笔者分析了三级综合医院开展社区护理的必要性和可行性;阐述了三级综合医院护士在开展社区护理中承担的教育者和咨询者、服务者、协调者和合作者、观察和研究者多种角色;并提出三级综合医院护士在开展社区护理中所发挥的作用,包括:(1)向社区提供经验丰富的专科护士,提高社区护理质量;(2)为社区护士的培养提供师资队伍和培训基地;(3)开设日间病房,为病人提供方便的治疗及护理;(4)建立双向转诊的社区卫生服务网络,为社区提供持续的优质服务;(5)开展社区护理研究。 相似文献

There is much controversy surrounding the issue of nurses in the role of patient advocate. Cancer patients are vulnerable due to their diagnoses, the anxiety that it generates, the nature of the disease and subsequent treatment and may have diminished autonomy as a result. It is questionable whether the nurse is able to promote the best interests of the patient in this situation. Models of advocacy and the nurse-patient relationship are discussed. The main arguments for taking on the role of patient advocate include: reference to long periods of time spent with the patient and the chance to build a relationship; the duty to care and the duty to do no harm. Criticisms of the appropriateness of the advocacy role for nurses include: inability for the patient to choose his or her nurse; promotion of self interests; charges of paternalism; challenges from other health care professionals and multiple and/or conflicting obligations. It is concluded that perhaps the most ethical way of caring is by empowering patients and promoting selfadvocacy. This concept needs to be further explored. 相似文献

盖洛普Q12对护士工作满意度的影响及应用研究 总被引:10,自引:0,他引:10
目的探讨什么样的工作环境最能激发护士的才干和热情以及盖洛普Q12运用在护理人性化管理中对营造一个良好的基层工作环境的作用。方法选用盖洛普Q12调查表,随机抽查某院16个病区的350名护士,采用自设的病人满意度调查表分别调查了这些病区的病人满意度。结果在333份有效问卷中对盖洛普Q12选择“非常满意”的占总选项的39.21%,选择“比较满意”占总选项的41.39%,选择“较不满意”占总选项的5.86%,选择“不满意”占总选项的13.49%,护士满意度和病人满意度有正的直线相关关系,随着护士满意度的提高,病人满意度增高。结论盖洛普Q12运用在护理人性管理中对营造一个积极、高效的基层工作环境是非常有效的,它们是管理中的12个重点。护士长如果能悉心关注这12个方面,使护士们对所有12个问题都作出肯定的回答,那么护士工作的满意度、患者满意度及医院的效益将会得到有效的提高。 相似文献

目的探讨专科护士实践模式的可行性及其对妊娠期糖尿病孕妇妊娠结局的影响。方法将577例妊娠期糖尿病孕妇按时间段分为对照组323例,干预组254例。对照组在常规产检时接受血糖管理,干预组在常规产检的基础上,采用妊娠期糖尿病专科护士实践模式对其进行全程管理。比较两组妊娠结局。结果干预组胰岛素使用率,患者入院次数和住院时间显著少于或短于对照组(均P0.01)。两组孕妇的分娩孕周、分娩方式、整个孕期体质量增长比较,差异无统计学意义(均P0.05)。在新生儿方面,新生儿出生时体质量、巨大儿、新生儿低血糖发生率结局指标比较,差异无统计学意义(均P0.05)。结论妊娠期糖尿病专科护士实践模式可行;通过该模式可降低胰岛素使用率,减少入院次数和住院天数,从而可减少医疗开支。 相似文献

Nurses working with people with learning disabilities are sometimes exposed to provoking behaviour such as unpredictable and violent actions, spitting and sexual harassment. Eight nurses at a group dwelling in Sweden were interviewed about their experiences when caring for a person who acts provokingly. Narrative interviews were conducted and interpreted using a method inspired by Ricoeur. Three themes were formulated which describe nurses' feelings of being tormented, disrupted and helpless. Nurses describe themselves as being deeply humiliated by physical violence, spittle, sexual invective and actions on the part of the patient. They respond more to being spat on and the sexual invective than to the patient's physical violence. The frequent humiliations could be seen as a symbolic language connected to defilement and 'evil' and therefore as having a deep symbolic meaning for the nurses. These results were interpreted and reflected on in the light of a theoretical framework from an affliction perspective in order to achieve a deeper understanding of the text. This paper shows that an affliction perspective is important when searching for the meaning of caring for a patient who acts provokingly. The study indicates that the goal in a provoking care situation must be to see the provoking patient, as he appears to the nurses in order to glimpse the goodness concealed behind the provoking facade. 相似文献
