目的探讨胼胝体梗死的临床表现、病因及鉴别诊断特点。方法对2005年7月收治的1例53岁男性胼胝体梗死患者的临床表现、影像学特点、病因机制及其治疗过程进行回顾分析。结果临床主要表现为发作性黑蒙、言语不利,既往有高血压、糖尿病、脑梗死、吸烟、饮酒史,体格检查以失用为主要表现。头部MRI检查可见左侧脑室旁、胼胝体梗死,右侧基底节、脑桥陈旧性腔隙性梗死;脑血管造影检查显示为多发性血管狭窄,其中以左侧大脑中动脉、右侧颈内动脉及基底动脉最为严重。经颈内动脉内膜剥离术及颈内动脉支架植入术治疗,临床症状缓解。结论失用可以是胼胝体梗死的主要表现,其病因是在脑动脉粥样硬化基础上的血流动力学改变,患者预后良好。 相似文献

AIM: To investigate the participation of adenosine receptors in the adenosine 5'-triphosphate (ATP)-induced relaxation in the corpus cavernosum penis (CCP) of rabbits. METHODS: The ATP-induced relaxation was assessed on the noradrenaline precontracted CCP of rabbits in the presence and absence of 8-(3-chlorostyryl)caffeine (CSC); an adenosine A(2A) receptor antagonist; alloxazine and MRS1754; adenosine A(2B) receptor antagonists; and ARL67156, an inhibitor of ecto-nucleoside triphosphate diphosphohydrolases. RESULTS: Adenosine and ATP relaxed the noradrenaline precontracted CCP of rabbits in a concentration-dependent manner. The adenosine- and ATP-induced relaxations were suppressed by alloxazine and MRS1754, but not by 8-(3-chlorostyryl)caffeine. ARL67156 potentiated the ATP-induced relaxation but not the adenosine-induced one. MRS1754 suppressed the ATP-induced relaxation potentiated by ARL67156. CONCLUSIONS: The above results suggest that, in the CCP of rabbits, the adenosine receptor mediating adenosine-induced relaxation is of the A(2B) receptor and the ATP directly causes relaxation through the A(2B) receptor on the CCP. 相似文献

目的 观察淫羊藿苷 (ICA)对家兔阴茎海绵体平滑肌的舒张效应及机制。方法 采用离体平滑肌张力记录法。结果 ①淫羊藿苷、西地那非 (sildenafil,Sil)、罂粟碱 (Pap)及氨力农 (amrinone ,Amr)均可浓度依赖性地引起舒张效应 ,EC50 分别为 0 2 ,11 2 ,31 6和 5 0 1μmol/L。②NO合酶抑制剂N 硝基左旋精氨酸甲酯 (L NAME ) 5μmol/L可将ICA引起的最大舒张效应由 6 0 %降低到 9% ,而NO前体左旋精氨酸 (L Arginine) 2 0 μmol/L ,可部分恢复ICA的最大舒张效应到 4 0 % (P <0 0 5 )。③ICA (1,10 μmol/L)增强Ach引起的舒张效应 ,最大效应由 6 0 %分别升高到 6 7%和 80 % (P <0 0 5 )。④ICA (3,10 μmol/L)能使高K (2 0~ 12 0mmol/L)去极化引起的最大收缩反应分别降低到 88%和 81% (P <0 0 5 )。⑤在经 1μmol/L阿托品和 5 μmol/L胍乙啶预先处理的标本上 ,电场刺激可以引起海绵体平滑肌频率依赖性的舒张效应。ICA (1、 10 μmol/L)能增强电场刺激引起的舒张效应 ,最大效应由对照水平的4 1%分别升高到 5 9%和 6 3% (P <0 0 5 )。结论 ICA可以直接舒张阴茎海绵体平滑肌 ,其壮阳作用机制与影响NO cGMP信号通路有关。 相似文献

Since 1903, Marchiafava‐Bignami disease has been recognized as a rare syndrome with focal demyelination and necrosis in the corpus callosum, which is usually found in chronic alcoholics. It extends into the neighboring white matter and occasionally as far as the subcortical regions. We report a Japanese patient with Marchiafava‐Bignami disease associated with alcohol abuse, who had traveled around Western Europe, North America and China for more than 30 years. As he suffered extreme delirium in the early stages we administered a low dose (10 mg) of mianserin hydrochloride. He was very irritable and uncooperative on admission, after 20 days his delirium had disappeared and his temper had become very calm and mild. After 40 days, his intelligence level increased substantially as measured by various neuropsychological tests. 相似文献

RATIONALE: A small percentage of patients with idiopathic generalized epilepsy (IGE) do not respond to medical therapy. Generalized tonic-clonic (GTC) seizures are especially debilitating and can be associated with severe injuries. The benefit, safety and effect of corpus callosotomy (CC) in patients with IGE have not been studied. METHODS: We reviewed patients with presumed IGE who underwent CC between 1991 and 2000. Criteria for selection included history, examination, brain imagining, interictal and ictal EEG. All patients had refractory and debilitating tonic-clonic seizures (GTCS) and had failed four or more antiepileptic drugs. Seizure frequency was calculated per month over the last year and pre-operative baseline was compared to last follow-up using paired t-tests. IQ, executive function, language and verbal, non-verbal memory and quality of life (QOL) was compared before and after surgery. Serial EEGs after surgery were reviewed. RESULTS: There were nine patients (seven men), mean age 37.9 (range: 22-49), mean IQ 87.3 (range: 75-107). All had anterior CC. Mean follow-up time was 5.4 years (range: 0.6-10.3 years). One patient died from sudden death in epilepsy 9 months after surgery. There was a significant reduction of GTC seizures from 6.3 to 1.1 (p<0.005). Four patients had more than 80% and eight more than 50% reduction. Of five patients with absence seizures, two became seizure free and one had more than 80% reduction and two worsened slightly, and of three with myoclonic seizures one had more than 90% reduction. One patient had completion of the CC with improvement of myoclonus and absence seizures, but not of GTC seizures and suffered a disconnection syndrome. Another had right frontal focal resection without improvement after new seizures of focal onset. Cognitive testing showed a good outcome (improved or no change) in all cognitive domains. Post-surgical EEG showed new focal slowing and sharp waves. There was no change in QOL. CONCLUSION: CC can be effective in reducing GTC, absence and myoclonic seizures in patients with refractory IGE. These findings suggest that interhemispheric communication of the cerebral cortices plays an important role in the generation of seizures in IGE. The in-vitro oestradiol (E2) and progesterone (P) production by corpora lutea (CL) obtained at sterilization from 30 untreated women and 43 women treated with norethisterone (NET) 300 micrograms daily was measured. The CL were obtained at different stages of the luteal phase in the untreated women [luteinizing hormone (LH) 0 to +3, n = 7; LH +4 to +7, n = 7; LH +8 to +11, n = 9; LH +12 to menses, n = 7] and on days LH +8 to +11 or cycle days 22 to 26 in the NET-treated women. In the treated women, four types of ovarian reaction were identified. Four women showed ovarian reaction Type A (completely inhibited ovarian activity), 14 women Type B (marked follicular activity, but no luteal function), 12 women Type C (normal follicular activity, followed by insufficient luteal function) and 13 women Type D (apparently normal follicular and luteal activity). The CL were incubated in Eagle's medium with and without stimulation by human chorionic gonadotrophin (HCG) for 2 and 4 h. In the untreated women, P and E2 production increased significantly with both incubation time and stimulation by HCG throughout the luteal phase, except in the late luteal phase (LH +12 to menses) where P increased (P less than 0.01) only after 4 h stimulation by HCG. 目的 :检测大鼠阴茎海绵体中磷酸二酯酶 5 (PDE5 )mRNA的表达 ,进而探讨小檗碱 (berberine ,Ber)的分子作用机制。 方法 :通过逆转录酶链反应 (RT PCR)技术检测大鼠阴茎海绵体中PDE5mRNA的表达。 结果 :大鼠阴茎海绵体中有PDE5A1和PDE5A2mRNA的表达 ,以PDE5A2为主要的异构酶。与内参照 β 肌动蛋白相比 ,对照组、Ber孵育 1和 3h组的PDE5A1及PDE5A2基因mRNA的相对表达量分别为 :0 .2 2± 0 .0 2 ,0 .4 1± 0 .0 1 ;0 .1 5± 0 .0 1 ,0 .34± 0 .0 2 ;0 .1 0± 0 .0 1 ,0 .1 2± 0 .0 1。与对照组相比 ,PDE5A1、PDE5A2的mRNA表达 ,在Ber孵育 1h组降低了 32 %和 1 7%,3h组降低了 5 5 %和 71 %,其中尤以应用Ber 3h后PDE5A2的mRNA减少最为明显 (n =5 ,P <0 .0 1 )。 结论 :Ber对NO cGMP信号通路的下游关键酶 (PDE5 )具有一定的调控作用 ,尤其是抑制PDE5A2的mRNA表达 ,为Ber治疗勃起功能障碍的分子机制之一。 相似文献
