A multitude of athletic injuries occur when the various tissues that make up the human body experience stresses and strains that exceed their material strength. The precise amount of stress and strain that any given tissue can withstand is determined by the mechanical properties and resultant strength of that particular tissue. These mechanical properties are directly determined by an individual’s physiology and acute regulation of these properties. A number of theoretical frameworks for athletic injury occurrence have been proposed, however, a detailed conceptual framework for injury aetiology that considers the interplay between the physiological and mechanical factors and outlines the causal pathways to tissue damage and injury is needed. This will guide injury research towards a more thorough investigation of causal mechanisms and understanding of risk factors. Further, it is important to take into account the considerable differences in loading patterns which can result in varying injury outcomes such as acute stress-related, strain-related, or overuse injury. Within this article a simplified conceptual model of athletic injury is proposed along with a detailed, evidence-informed, conceptual framework for athletic injury aetiology that focuses on stress-related, strain-related, and overuse injury. 相似文献

A series of three World Café events centred on the topic of developing an Educational Framework for Urological Nursing [EFUN] were held by the British Association of Urological Nurses [BAUN], the European Association of Urology Nurses [EAUN] and the Australian and New Zealand Urological Nursing Society [ANZUNS] between 2017 and 2019. About 376 urology nurses participated in these “conversations that matter” and generated 1047 individual response items that were grouped into themes to assist the three associations to take the creation of an EFUN to the next level. Areas explored centred on four aspects: what any agreed educational framework for urology nursing should contain; the academic level at which education should be provided; who should be recruited as collaborators on writing an educational framework and, lastly, just how any emergent framework should be used. Analysis of the conversational data indicate that there exists within the urological nursing community a collective wisdom regarding their educational needs and how these needs should be met. 相似文献

In their article in this journal “Is well-being U-shaped over the life cycle?” Blanchflower and Oswald (Blanchflower, D.G., & Oswald, A.J. (2008). Is well-being U-shaped over the life cycle? Social Science & Medicine, 66, 1733–1749) report the results of an ambitious cross-national study of age and well-being and conclude that in most societies studied the well-being of adults tends to be high in young adulthood and old age and lowest around age 40. I argue that the appearance of this U-shaped curve of well-being is the result of the use of inappropriate and questionable control variables. The most clearly inappropriate control variable is marital status, the control of which to a large extent accounts for the U-shaped curve. Most researchers who have studied the relationship between being married and being happy believe that happiness affects marital status (happier people are more likely to marry and stay married), and of course a variable that is affected by the dependent variable should not be included as a control variable in a simple recursive model. Such control variables as income and education are suspect because the relationship between them and well-being is likely to be partially spurious, and such variables as race and whether or not persons lived with both parents at age 16 should not be controlled, because they cannot affect or be affected by age. Finally, year of survey should not be controlled because of the age-period-cohort conundrum, which makes including age, period, and cohort all as predictor variables in a regression inappropriate (and impossible if the three variables are measured precisely and comparably). The only clearly appropriate control variable is birth cohort, and when only it is controlled, the regression data become estimates of how the well-being of persons has actually changed as they have gone through the life course. I argue that such estimates are much more useful than the counterfactual abstractions provided by Blanchflower and Oswald (Blanchflower, D.G., & Oswald, A.J. (2008). Social Science & Medicine, 66, 1733–1749), and I conclude that those authors (or someone else) could make a very important contribution by redoing their analyses with birth cohort as the only control variable. Hepatitis C virus (HCV) establishes persistent infection in the majority of infected individuals. The currently accepted hypothesis of immune evasion by antigenic variation in hypervariable region 1 (HVR1) of glycoprotein E2 does not however, explain the lack of subsequent immune recognition. Here, we show that the N-terminal region of E2 is antigenically and structurally similar to human immunoglobulin (Ig) variable domains. E2 is recognized by anti-human IgG antibodies and also possesses common amino acid (aa) sequence features of the conserved v-gene framework regions of human Ig light chains in particular but also heavy chains and T cell receptors. Using a position specific scoring system, the degree of similarity of HVR1 to Ig types correlated with immune escape and persistence in humans and experimentally infected chimpanzees. We propose a unique role for threshold levels of Ig molecular mimicry in HCV biology that not only advances our concept of viral immune escape and persistent infection but also provides insight into host-dependent disease patterns. 相似文献

BACKGROUND: General practice in the UK is undergoing a period of rapid and profound change. Traditionally, research into the effects of change on general practice has tended to regard GPs as individuals or as members of a professional group. To understand the impact of change, general practices should also be considered as organisations. AIM: To use the organisational studies literature to build a conceptual framework of general practice organisations, and to test and develop this empirically using case studies of change in practice. This study used the implementation of National Service Frameworks (NSFs) and the new General Medical Services (GMS) contract as incidents of change. DESIGN OF STUDY: In-depth, qualitative case studies. The design was iterative: each case study was followed by a review of the theoretical ideas. The final conceptual framework was the result of the dynamic interplay between theory and empirical evidence. SETTING: Five general practices in England, selected using purposeful sampling. METHOD: Semi-structured interviews with all clinical and managerial personnel in each practice, participant and nonparticipant observation, and examination of documents. RESULTS: A conceptual framework was developed that can be used to understand how and why practices respond to change. This framework enabled understanding of observed reactions to the introduction of NSFs and the new GMS contract. Important factors for generating responses to change included the story that the practice members told about their practice, beliefs about what counted as legitimate work, the role played by the manager, and previous experiences of change. CONCLUSION: Viewing general practices as small organisations has generated insights into factors that influence responses to change. Change tends to occur from the bottom up and is determined by beliefs about organisational reality. The conceptual framework suggests some questions that can be asked of practices to explain this internal reality. 相似文献
