ObjectiveWe address the hypothesis that the extraordinary sandstorm occurred on 22-24 February 2020 might have a role in the different cumulated incidence of COVID-19 cases between the islands of Tenerife and Gran Canaria, since it obliged to reduce significantly air traffic and forced to suspend all major carnival street events in all most locations.MethodWe performed a retrospective analysis of COVID-19 cases as to 1 April 2020 according to symptoms onset, weather-related data and Carnival events in Tenerife and Gran Canaria.ResultsThe sandstorm occurred on February 22-24, 2020, forced air traffic to close, reducing the influx of tourists to the Canary Islands and suspending carnival events in most places, except in Santa Cruz de Tenerife. Cumulated incidence as to 1 April was 132.81/100,000 in Tenerife, and 56.04/100,000 in Gran Canaria.ConclusionsThe suspension of Carnival events due to the sandstorm in the Canary Islands contributed to reduce differently the SARS-CoV-2 spread in Tenerife and Gran Canaria.  相似文献

重型病毒性肝炎血清甲状腺激素水平检测及临床意义   总被引:1,自引:0,他引:1  
目的 探讨重型病毒性肝炎患者血清甲状腺激素水平与肝损害程度关系及临床意义.方法 应用放射免疫法测定54例重型病毒性肝炎、41例急性病毒性肝炎患者及30例正常人血清甲状腺激素水平.结果 重型病毒性肝炎T3、T4、TSH水平显著低于急性病毒性肝炎及正常人(P<0.05);rT3则增高(P<0.05).T3、T4与血浆Alb呈正相关,L、TSH与PT,rT3与ALT呈负相关.重型病毒性肝炎死亡者T3、T4、TSH显著低于存活者(P<0.01).rT3则增高(P<0.01).结论 重型病毒性肝炎患者甲状腺激素水平可作为反映肝功能的敏感指标,对判断疾病严重程度、预后有重大价值.  相似文献

Bodysnatching was an illegal way to procure cadavers for anatomical dissection before the existence of effective anatomy legislation. As knowledge of anatomy was fundamental to medical practice, many famous nineteenth century physicians turned a blind eye to this activity or even participated. Sir William Osler, perhaps the most revered physician of all time, received his medical degree from McGill University in 1872 and then served as pathologist at Montreal General Hospital from 1874 to 1884, where he began a career which culminated in him becoming both the first Professor of Medicine at Johns Hopkins and then Regius Professor at Oxford. Quebec had been slow to enact effective anatomy legislation, and most of the cadavers in the McGill anatomy laboratory were resurrected; Osler's close friend and Anatomy Demonstrator Francis Shepherd was convicted of this offence on several occasions. In 1940, a letter was discovered in the archives at the McGill's William Osler Library, purportedly written in 1880, accusing Osler of being a grave robber while in Montreal. My paper dissects the letter to assess its credibility in the historical context of Osler's early life and the fact that Osler was a lifelong notorious practical joker with his own pseudonym, Dr. Egerton Yorrick Davis, who helped with his pranks. The provenance of the letter is analyzed and the paper explores, using historical context and forensic handwriting analysis, whether this letter is a revenge‐motivated practical joke played on him by one of his famous colleagues or represents Osler's last practical joke for posterity. Drawing its etymology from the Latin pestis (curse), plague, over the centuries, has been more dreaded by humankind than any other epidemic. The Apocalypse had recognized plague as the archetypal divine curse, ‘the power to kill over a fourth of the earth'. Plague is thus a particular topic of study, insofar that it is one of the rare epidemics that has had recurrent major consequences on demography and human societies. Its highly transmissible nature, the brutality of its action, its high pathogenicity, marked by strong lethality and great swiftness, and the complete absence of treatment options before the 20th century conferred on it a sinister aspect. Generating a series of severe demographic crises, well known in the Western world, it has necessarily influenced the evolution of societies at both the biological and cultural levels.  相似文献

Introduction and objectivesCOVID-19 is currently causing high mortality and morbidity worldwide. Information on cardiac injury is scarce. We aimed to evaluate cardiovascular damage in patients with COVID-19 and determine the correlation of high-sensitivity cardiac-specific troponin T (hs-cTnT) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) with the severity of COVID-19.MethodsWe included 872 consecutive patients with confirmed COVID-19 from February to April 2020. We tested 651 patients for high-sensitivity troponin T (hs-TnT) and 506 for NT-proBNP on admission. Cardiac injury was defined as hs-TnT > 14 ng/L, the upper 99th percentile. Levels of NT-proBNP > 300 pg/mL were considered related to some extent of cardiac injury. The primary composite endpoint was 30-day mortality or mechanical ventilation (MV).ResultsCardiac injury by hs-TnT was observed in 34.6% of our COVID-19 patients. Mortality or MV were higher in cardiac injury than noncardiac injury patients (39.1% vs 9.1%). Hs-TnT and NT-proBNP levels were independent predictors of death or MV (HR, 2.18; 95%CI, 1.23-3.83 and 1.87 (95%CI, 1.05-3.36), respectively) and of mortality alone (HR, 2.91; 95%CI, 1.211-7.04 and 5.47; 95%CI, 2.10-14.26, respectively). NT-ProBNP significantly improved the troponin model discrimination of mortality or MV (C-index 0.83 to 0.84), and of mortality alone (C-index 0.85 to 0.87).ConclusionsMyocardial injury measured at admission was a common finding in patients with COVID-19. It reliably predicted the occurrence of mortality and need of MV, the most severe complications of the disease. NT-proBNP improved the prognostic accuracy of hs-TnT.  相似文献

IntroductionThe blunt trauma victim management is still a matter of debate and comparing studies involving different emergency medical services and health care organization remains fictitious. Hence, the French Intensive care Recorded in Severe Trauma (FIRST) was conducted in order to describe the severe blunt trauma management in France. The present paper aimed at recalling the main results of FIRST study.MethodsThe FIRST study was based on a multicenter prospective cohort of patients aged 18 or over with severe exclusive blunt trauma requiring admission to university hospital care unit within the first 72 h and/or managed by medical-Staffed Emergency Mobile Unit (SMUR). Multiple data were collected about patient characteristics, clinical initial status, typology of trauma and the main endpoints were 30-day mortality.ResultsSixty-one percent of trauma patients were road traffic victims and 30% were domestic, sport or leisure trauma. Patients who benefited from medical pre-hospital management were globally more severely injured than those who received basic life support care by fire brigades. Therefore, they were delivered more aggressive treatment in the pre-hospital setting and the median time for their hospital admission was lengthened. However, their 30-day mortality was significantly reduced. The probability of death was also decreased when casualties were transported by SMUR helicopter directly to the university hospital. In the in-hospital setting, the performance of a whole-body computed tomography (CT) was associated with a significant reduction in the mortality risk compared with a selective CT.ConclusionThe FIRST study suggests the benefit of a medical management in the pre-hospital setting on the survival of trauma patients. The emergency physician (EP) expertise in the pre-hospital and initial hospital phases would lead to the concept of the appropriate care for the appropriate trauma patient. It also highlights the necessity to set up organized regional sectors of care and registries.  相似文献

Therapeutic hypothermia (TH) is considered a standard of care in the post-resuscitation phase of cardiac arrest. In experimental models of traumatic brain injury (TBI), TH was found to have neuroprotective properties. However, TH failed to demonstrate beneficial effects on neurological outcome in patients with TBI. The absence of benefits of TH uniformly applied in TBI patients should not question the use of TH as a second-tier therapy to treat elevated intracranial pressure. The management of all the practical aspects of TH is a key factor to avoid side effects and to optimize the potential benefit of TH in the treatment of intracranial hypertension. Induction of TH can be achieved with external surface cooling or with intra-vascular devices. The therapeutic target should be set at a 35 °C using brain temperature as reference, and should be maintained at least during 48 hours and ideally over the entire period of elevated intracranial pressure. The control of the rewarming phase is crucial to avoid temperature overshooting and should not exceed 1 °C/day. Besides its use in the management of intracranial hypertension, therapeutic cooling is also essential to treat hyperthermia in brain-injured patients. In this review, we will discuss the benefit-risk balance and practical aspects of therapeutic temperature management in TBI patients.  相似文献

The prognosis of severe trauma patients is determined by the ability of a healthcare system to provide high intensity therapeutic treatment on the field and to transport patients as quickly as possible to the structure best suited to their condition. Direct admission to a specialized center (“trauma center”) reduces the mortality of the most severe trauma at 30 days and one year. Triage in a non-specialized hospital is a major risk of loss of chance and should be avoided whenever possible. Medical dispatching plays a major role in determining patient care. The establishment of a hospital care network is an important issue that is not formalized enough in France. The initial triage of severe trauma patients must be improved to avoid taking patients to hospitals that are not equipped to take care of them. For this purpose, the MGAP score can predict severity and help decide where to transport the patient. However, it does not help predict the need for urgent resuscitation procedures. Hemodynamic management is central to the care of hemorrhagic shock and severe head trauma. Transport helicopter with a physician on board has an important role to allow direct admission to a specialized center in geographical areas that are difficult to access.  相似文献

Traumatic brain injuries are fairly sensitive to hypoxia. For patient with associated lung and brain traumas, different means used to improve oxygen blood level are poorly described.
