目的回顾性分析开窗支架型血管治疗近肾腹主动脉瘤(AAA)的初步结果。方法4例合并严重疾病无法行开放手术的近肾AAA患者,根据术前CT数据定制个体化开窗支架型血管。于全身麻醉下置人此支架型血管并置入肾动脉支架。结果术后即时造影示各分支血管血流通畅,支架形态良好,两例有少量近端I型内漏。术后3个月复查4例患者支架形态正常,两例I型内漏均消失,但另一名患者出现Ⅱ型内漏。结论应用个体化开窗支架型血管治疗近肾AAA近期效果良好。  相似文献

PurposeTo characterize the short-term results of a newly available self-expanding covered stent (Covera; CR Bard Peripheral Vascular Inc., Murray Hill, New Jersey) for the reconstruction of target vessels in complex aneurysms.Materials and MethodsFrom August 2017 to November 2018, this self-expanding covered stent was used in 17 patients (mean 72.6 ± 7.6 years of age) during endovascular aneurysm repair (EVAR) with hypogastric preservation (11.8%), branched EVAR (29.4%), fenestrated (F)-EVAR (17.6%), chimney + F-EVAR (11.8%), or chimney EVAR (29.4%). In more than 48 stented arteries (2.8 ± 1.1/patient), 25 were preserved using this self-expanding covered stent.ResultsAll target vessels were successfully preserved. There was no 30-day mortality and 1 in-hospital death. Intraoperative aneurysm exclusion was successful in 14 patients (82.4%) with a perioperative technical success rate of 82.4%. The actuarial survival rate was 93.8% at 6 months and 85.9% at 12 months. Aneurysm sac regression of >5 mm was observed in 4 cases (23.5%), and the sac remained stable in the remaining patients (13 cases [76.5%]). At 12 months, the primary clinical success rate was 76.5%, and assisted primary clinical success rate was 82.4%. No type 3 endoleak was related to a disruption of the reconstruction with the self-expanding covered stent.ConclusionsThis new self-expanding covered stent provides good short-term patency in chimneys, branches, or fenestrations. Larger series with long-term follow-up are required to determine if the stent can sustain the mechanical stress to which it will be submitted in these repairs.  相似文献

The outer blood-retinal barrier is composed of a monolayer of retinal pigment epithelium, Bruch's membrane and the choriocapillaris which is fenestrated. Endothelial proliferation and breaching of Bruch's membrane leads to the neovascular form of age-related macula degeneration (ARMD). The aim of this study was to generate an in vitro model that mimics more faithfully the phenotype of the choriocapillaris and the trilayer architecture in vitro. A trilayer culture model was generated with retinal pigment epithelium (ARPE-19) cell cultures on the epithelial surface of amniotic membrane and with human umbilical vein-derived endothelial cells on the other surface. A control model for the effect of retinal pigment epithelium on endothelial changes was generated with corneal epithelial cells replacing the ARPE-19. Both human umbilical vein-derived endothelial and ARPE-19 cells formed confluent monolayers on respective surfaces of the amnion. The human umbilical vein-derived endothelial cells in the trilayer became fenestrated when co-cultured with the ARPE-19 cells, but not with corneal epithelial cells, or when grown as monolayers on the amnion, showing a loss of fidelity of origin in the presence of ARPE-19 cells. These cells also revealed VE-cadherin and ZO-1 at cell-cell contacts from 24 h in the trilayer. The tight junctional molecules, occludin and ZO-1, were localized to cell-cell contact regions in the retinal pigment epithelium, both in the monolayer and in the trilayer system. Permeability of the trilayer was tested by using fluorescein and fluorescein-conjugated tracers under flow. At 72 h the trilayer severely restricted transfer of sodium fluorescein (NaF) (ten-fold reduction) whilst transfer of a 4 kDa FITC-conjugated dextran was virtually occluded, confirming a restrictive barrier. Ultrastructural studies showed the retinal pigment epithelium monolayer was polarized with microvilli present on the apical surface. Paracellular clefts showed numerous tight junctional-like appositions, similar to that seen on amnion alone. This study demonstrates that ARPE-19 and human umbilical vein-derived endothelial cells can be co-cultured on the amniotic membrane and that the resultant cross-talk leads to formation of a fenestrated endothelium, whilst maintaining a polarized restrictive epithelial layer. The fenestrated endothelial phenotype achieved in this human in vitro trilayer model is a first and offers an outer-retinal barrier which approaches the in vivo state and has potential for studies into induced junctional disruption, endothelial proliferation and migration: features of ARMD.  相似文献

Equine herpesvirus‐1 (EHV‐1) is a major equine pathogen causing respiratory diseases, abortions and severe neurological disorders. The basis of neurological disturbances is, as in other organs, infection of endothelial cells, followed by vasculitis, thrombosis and ischaemic damage of the parenchyma. Here, a murine model was used to explore the mechanism of entry to, and spread within the brain, the cell affinity of the agent and the modulating role of the immune defence, which are all factors governing the pathogenesis of the neurological disease. Because controversial views exist about these mechanisms, we undertook a neuropathological study with intranasally infected adult mice. EHV‐1 entered the brain through the olfactory neuroepithelium and along the olfactory nerves, and spread transsynaptically in rostro‐caudal direction, using olfactory and limbic neuronal networks. Exclusively neurons were infected. The cellular immune reaction exerted a restraining effect on virus dissemination. Following nasal infection, the olfactory route was the major pathway for virus entry and dissemination, involvement of the trigeminal nerve in virus spread seems much less probable. In the adult mouse brain EHV‐1 behaves as a typical neurotropic agent, using, similarly to other herpesviruses, the neuronal networks for dissemination. Vasculitis, the predominant type of lesion in natural infection, and endothelial cell positivity for EHV‐1 were detectable only in the lung. Thus, this agent exhibits in the mouse a dual affinity: it is neurotropic in the brain, and endotheliotropic in visceral organs. Consideration of pathogenetic aspects of equine and experimental murine EHV‐1 infections also helps a better understanding of human herpetic brain disease.  相似文献

目的分析Z型覆膜支架波峰数目对覆膜支架生物力学性能,尤其对径向支撑性能及柔顺性影响,为Z型覆膜支架设计以及临床选择提供理论支持。方法通过有限元方法,建立4种不同波峰数目(5、8、10、12)的Z型覆膜支架有限元模型,分别施加径向位移载荷和绕z轴40°转动变量以完成压握和弯折的模拟,并提取支架应力、径向支撑力、覆膜应变、横截面变形程度及弯矩作为评价参数。结果相比波峰数目较多的覆膜支架,压握过程中5波峰覆膜支架具有最大的径向力,提供良好的径向支性能;5波峰覆膜支架在弯折模拟中产生了68 N·mm的最小力矩及67.5%的最小弯折横截面变形程度,并且在弯曲过程中覆膜的拉伸应变较小。结论 Z型覆膜支架波峰数目对覆膜支架生物力学性能的影响显著。波峰数目相对较少的Z型覆膜支架在径向支撑性能及弯曲力矩、横截面形变程度等柔顺性评价方面表现出优势,有利于覆膜支架在血管内的固定贴合,对减少血栓和内漏等并发症具有积极意义。研究结果可以指导Z型覆膜支架优化设计及手术最优选择。  相似文献

目的探讨累及弓部分支的Stanford B型主动脉夹层患者原位开窗腔内治疗的临床护理对策。方法回顾性分析解放军总医院血管外科2012年1月至2013年7月收治的9例累及弓部分支的B型主动脉夹层近端锚定区不足患者采用腔内原位开窗技术治疗的临床资料,总结护理要点。结果所有患者支架置入均一次成功,夹层原发破口完全封闭,无内漏发生,围术期无死亡病例。1例患者出现声音轻微嘶哑,其余患者无相关并发症发生。9例患者的平均住院天数为(12±2)d,均痊愈出院。结论加强累及弓部分支的Stanford B型主动脉夹层近端锚定区不足患者原位开窗腔内治疗的围术期护理,有助于提高患者的手术耐受性、有效预防并发症的发生。  相似文献

The goals of future development of endovascular aortic stent grafting were towards extension of the proximal seal,preservation of vital branches,ease of deployment,and low profile delivery systems.Fenestrated aortic stent graft is a viable option for treating aortic aneurysms with short necks,and more than 5000 grafts have been implanted worldwide.The first device available,the Zenith fenestrated graft (Cook Medical,USA),involved placing a proximal tube fenestrated component,then accessing each target vessels from the contralateral side with a sheath,and bridging the connection with a covered stent.A bifurcated distal component is then placed to complete the endovascular repair.  相似文献

An 8‐year‐old boy with tetralogy of Fallot, pulmonary atresia, and major aortopulmonary collaterals status post complete repair including a fenestrated ventricular septal defect patch presented to the catheterization laboratory for fenestration closure. During the procedure, the catheterization wire was found to have an unusual intracardiac loop and was unable to be straightened within the heart. Three‐dimensional transesophageal echocardiography revealed the wire was looped around a right ventricular papillary muscle. The wire was readjusted, and the fenestration was successfully closed. Three‐dimensional imaging was essential in unambiguously defining the catheter course and assisting in fenestration closure.  相似文献

Purpose

The study purpose was to review the outcomes of patients treated for thoracoabdominal aortic aneurysms using endovascular repair with fenestrated and branched stent-grafts in a single center.

Methods

We reviewed the clinical data of the first 185 consecutive patients (134 male; mean age, 75 ± 7 years) treated for thoracoabdominal aortic aneurysms using fenestrated and branched stent-grafts. Graft design evolved from physician-modified endografts (2007-2013) to off-the-shelf or patient-specific manufactured devices in patients enrolled in a prospective physician-sponsored investigational device exemption protocol (NCT 1937949 and 2089607). Outcomes were reported for extent IV and extent I to III thoracoabdominal aortic aneurysms, including 30-day mortality, major adverse events, patient survival, primary target vessel patency, and reintervention.

Results

A total of 112 patients (60%) were treated for extent IV thoracoabdominal aortic aneurysms, and 73 patients (40%) were treated for extent I to III thoracoabdominal aortic aneurysms. Demographics and cardiovascular risk factors were similar in both groups. A total of 687 renal-mesenteric arteries (3.7 vessels/patient) were targeted by 540 fenestrations and 147 directional branches. Technical success was 94%. Thirty-day mortality was 4.3%, including a mortality of 1.8% for extent IV and 8.2% for extent I to III thoracoabdominal aortic aneurysms (P = .03). Mortality decreased in the second half of clinical experience from 7.5% to 1.2%, including a decrease of 3.3% to 0% for extent IV thoracoabdominal aortic aneurysms (P = .12) and 15.6% to 2.4% for extent I to III thoracoabdominal aortic aneurysms (P = .04). Early major adverse events occurred in 36 patients (32%) with extent IV thoracoabdominal aortic aneurysms and 26 patients (36%) with extent I to III thoracoabdominal aortic aneurysms, including spinal cord injury in 2 patients (1.8%) and 4 patients (3.2%), respectively. Mean follow-up was 21 ± 20 months. At 5 years, patient survival (56% and 59%, P = .37) and freedom from any reintervention (50% and 53%, P = .26) were similar in those with extent IV and extent I to III thoracoabdominal aortic aneurysms. Primary patency was 93% at 5 years.

Conclusions

Endovascular repair of thoracoabdominal aortic aneurysms can be performed with high technical success and low mortality and morbidity. However, the need for secondary reinterventions and continued graft surveillance represents major limitations compared with results of conventional open surgical repair. Long-term follow-up is needed before the widespread use of these techniques in younger or lower-risk patients.  相似文献
