ObjectivesTo investigate the subcortical somatosensory evoked potentials (SEPs) to electrical stimulation of either muscle or cutaneous afferents.MethodsSEPs were recorded in 6 patients suffering from Parkinson’s disease (PD) who underwent electrode implantation in the pedunculopontine (PPTg) nucleus area. We compared SEPs recorded from the scalp and from the intracranial electrode contacts to electrical stimuli applied to: 1) median nerve at the wrist, 2) abductor pollicis brevis motor point, and 3) distal phalanx of the thumb. Also the high-frequency oscillations (HFOs) were analysed.ResultsAfter median nerve and pure cutaneous (distant phalanx of the thumb) stimulation, a P1-N1 complex was recorded by the intracranial lead, while the scalp electrodes recorded the short-latency far-field responses (P14 and N18). On the contrary, motor point stimulation did not evoke any low-frequency component in the PPTg traces, nor the N18 potential on the scalp. HFOs were recorded to stimulation of all modalities by the PPTg electrode contacts.ConclusionsStimulus processing within the cuneate nucleus depends on modality, since only the cutaneous input activates the complex intranuclear network possibly generating the scalp N18 potential.SignificanceOur results shed light on the subcortical processing of the somatosensory input of different modalities.  相似文献

BackgroundThe objective of this study was to compare complication rates between patients undergoing reverse shoulder arthroplasty (RSA) after a prior open reduction and internal fixation (ORIF) for proximal humerus fracture (PHF) to those undergoing RSA as a primary treatment for PHFs, glenohumeral osteoarthritis, or rotator cuff tear arthropathy (CTA).MethodsPatients who underwent RSA between 2015 and 2020 were identified in the Mariner database. Patients were separated into 3 mutually exclusive groups: (1) RSA for osteoarthritis, rotator cuff tear, or CTA (Control-RSA); (2) RSA as a primary treatment for PHF (PHF-RSA); and (3) RSA for patients with prior ORIF of PHFs (ORIF-RSA). Ninety-day medical and 2-year postoperative surgical complications were identified. In addition, patients in the PHF-RSA group were subdivided into those undergoing RSA for PHF within 3 months of the fracture (acute) vs. those treated greater than 3 months from diagnosis (delayed). Multivariate regression was performed to control for differences in comorbidities and demographics.ResultsA total of 30,824 patients underwent primary RSA for arthritis or CTA, 5389 patients underwent RSA as a primary treatment for a PHF, and 361 patients underwent RSA after ORIF of a PHF. ORIF before RSA was associated with an increased risk of overall revision (odds ratio [OR] 2.45, P = .002), infection (OR 2.40, P < .001), instability (OR 2.43, P < .001), fracture (OR 3.24, P = .001), minor medical complications (OR 1.59, P = .008), and readmission (OR 2.55, P = .001) compared with the Control-RSA cohort. RSA as a primary treatment for PHF was associated with an increased risk of 2-year revision (OR 1.60, P < .001), infection (OR 1.51, P < .001), instability (OR 2.84, P < .001), and fracture (OR 2.54, P < .001) in addition to major medical complications (OR 2.02, P < .001), minor medical complications (OR 1.92, P < .001), 90-day emergency department visits (OR 1.26, P < .001) and 90-day readmission (OR 2.03, P < .001) compared with the Control-RSA cohort. The ORIF-RSA group had an increased risk of periprosthetic infection (OR 1.94, P = .002) when compared with the PHF-RSA cohort. There were no differences in medical or surgical complications in the RSA-PHF cohort between patients treated in an acute or delayed fashion.ConclusionRSA following ORIF of a PHF is associated with increased complications compared with patients undergoing RSA for nonfracture indications. Prior ORIF of a PHF is also an independent risk factor for postoperative infection after RSA compared with patients who undergo RSA as a primary operation for fracture. The timing of RSA as a primary operation for PHF does not appear to impact the rates of postoperative medical and surgical complications.  相似文献

目的:探讨快速康复外科理念联合内窥镜椎间盘切除术治疗腰椎间盘突出症的疗效。方法:选取2018年4月—2019年4月本院收治的86例腰椎间盘突出症患者,随机分为两组,每组43例,对照组通过内窥镜椎间盘切除术治疗,观察组予以快速康复外科理念联合内窥镜椎间盘切除术治疗,对比两组疗效和并发症发生情况。结果:观察组并发症发生率(4.65%)较对照组(18.60%)低,差异有统计学意义(P 0.05);观察组下床活动时间、住院时间较对照组短,术后7天的疼痛评分较对照组低,差异有统计学意义(P 0.05);术后3个月,观察组的Oswestry功能障碍指数问卷表评分较对照组低,日本骨科协会评估治疗分数较对照组高,差异有统计学意义(P 0.05)。结论:探讨快速康复外科理念联合内窥镜椎间盘切除术治疗腰椎间盘突出症可减少并发症,缩短术后恢复时间,进而促进患者功能有效康复。  相似文献

目的:分析针刀与针灸配合应用于腰椎间盘突出症患者中的临床治疗效果。方法:在我院针灸康复科2017年12月-2018年12月治疗的腰椎间盘突出症患者中选取64例,并严格按照系统随机分配原则分为对照组和观察组,各32例。其中对照组采用针灸进行治疗,观察组采用针刀与针灸配合治疗,观察对比两组患者治疗后临床效果和治疗前后运动功能评分(Fugl-Meyer)、腰椎日本骨科学会(Japanese Orthopaedic Association,JOA)评分、视觉模拟评分法(Visual Analogue Scale,VAS)评分以及临床症状改善情况。结果:两组患者治疗前Fugl-Meyer、VAS、JOA评分差异无统计学意义(P>0.05),治疗后对照组VAS评分高于观察组,Fugl-Meyer、JOA评分低于观察组,且对照组总有效率低于观察组,对照组腰骶疼痛评分和腿疼痛评分均低于观察组,直腿抬高度高于观察组,差异具有统计学意义(P<0.05)。结论:予以腰椎间盘突出症患者针刀与针灸配合治疗效果显著,效果优于单一使用针灸治疗,可有效缓解患者疼痛感,改善临床症状,值得推广。  相似文献

目的 分析腰椎间盘突出症手术疗效不佳的原因,探讨提高疗效的方法。方法 随访本院1993-2003年腰椎间盘突出症行后路髓核摘除术病例236例,对术后腰腿痛症状未缓解或症状消失后再复发者复查X片、CT或MRI了解病因。结果手术疗效不佳者25例。疗效不佳的主要原因为残留髓核再突出、术后脊柱不稳倾向增加、对侧隐窝狭窄认识不足、神经根损伤、极外侧型椎问盘突出漏切、感染、术后椎管内瘢痕黏连等。全椎板切除术较椎板问开窗术更易发生椎管内瘢痕黏连(P<0.01)。结论首次手术时应尽可能取净髓核组织,对动力位摄片发现有椎问不稳倾向者行后外侧植骨或椎体问植骨融合,常规探查并妥善处理侧隐窝,牵拉神经根时间不应过长且用力轻柔。行腰椎间盘CT和MRI扫描时,应注意包括椎间盘相邻上下椎体的1/3部,以防止遗漏极外侧型椎问盘突出,尽量采用椎板间开窗术式以减少脊柱创伤,术中间断冲洗,术毕持续负压引流,以减少术后黏连,术中应严格无菌操作。  相似文献

目的:研究CT引导下胶原酶注射颈椎间盘溶解术治疗颈椎间盘突出症的价值。方法:对15例颈椎间盘突出症患者在CT引导下行胶原酶注射颈椎间盘溶解术。结果:15例均成功,随访1~16个月,优良率93.3%,未见并发症发生。结论:CT引导下行胶原酶注射颈椎间盘溶解术治疗颈椎间盘突出症安全有效,值得进一步研究。  相似文献

目的探讨下腰痛患者腰椎终板Modic退变、椎间盘退变及CT引导下腰椎间盘造影疼痛激发试验的相关性.方法对45例下腰痛患者常规行腰椎X线和MR检查,分别按Modic终板退变标准(0~3级)与Pearce椎间盘退变标准(Ⅰ~Ⅴ级)对终板和椎间盘进行评估.在CT引导下对45例患者中的40例(120个椎间盘)进行造影和疼痛激发试验,并按Dallas椎间盘造影分级系统(DDD)测评椎间盘退变程度.采用SPSS 11.5统计学软件分析腰椎终板Modic退变、椎间盘退变与腰椎间盘造影疼痛激发试验之间的相关性.结果40例下腰痛患者的腰椎终板Modic分级与椎间盘退变Pearce分级存在较强的相关性(Pearson x^2=43.326,P=0.000),与椎间盘造影疼痛激发试验有显著相关性(Pearson x^2=27.858,P=0.000);椎间盘退变Pearce分级与CT椎间盘造影椎间盘退变Dallas分级也呈较强的相关性.结论腰椎终板Modic退变分级与椎间盘退变Pearce分级密切相关,而与椎间盘疼痛激发试验有显著相关性,提示终板Modic退变可能是下腰痛的原因之一.  相似文献

目的探讨经皮髓核摘除术(PLD)联合医用臭氧注射治疗腰椎间盘突出症的临床效果及可行性。方法临床与影像检查确诊为腰椎间盘突出25例,先行PLD,后行椎间盘内及椎旁注射臭氧。结果25例病人治疗后经一年随访,治愈21例,有效24例,无效1例,无一例并发症。结论PLD联合医用臭氧治疗腰椎间盘突出症,安全可靠,可改善治疗效果,减少不良反应,值得推广应用。  相似文献

目的探讨经皮激光椎间盘减压术(percutaneous laser disc decompression,PLDD)治疗椎间盘源性腰痛的疗效。方法2002年6月~2004年12月我院对36例椎间盘源性腰痛,采用英国DIOMED公司半导体激光仪,激光功率15W,每个激光脉冲持续1s,间隔1s,照射能量800~1200J。VAS评分评价治疗效果。结果手术时间15~60min,平均30min。32例出现“疼痛复制效应”。36例随访6~36个月,平均11个月,32例有效(术后VAS评分改善≥3分18例,≥分14例),4例无效,有效率88.9%(32/36)。结论PLDD治疗椎间盘源性腰痛安全、有效、微创。  相似文献

Choleragenoid (cholera toxin B-fragment; CTB) is an anterograde, retrograde and transganglionic neuronal tracer. We describe a method for detecting CTB-labeled neuronal cell bodies, neurites and boutons at the ultrastructural level, using postembedding immunogold techniques on freeze-substituted Lowicryl HM20™ embedded nervous tissue. Primary afferents and motoneurons were labeled by injection of CTB in the dorsal ramus of the C2 spinal nerve of the rat. Following fixation with paraformaldehyde (4%) and glutaraldehyde (0.25%), tissue sections from the spinal cord C2 segment were freeze-substituted and embedded in Lowicryl HM20™ and subsequently processed with postembedding immunocytochemistry for CTB and glutamate. Immunogold particles indicating CTB immunoreactivity were found over primary afferents and motoneurons. In primary afferents in the central cervical nucleus (CCN) and motor nuclei, immunogold labeling was seen in boutons over vesicle-containing axoplasm and to a lesser extent over axoplasm devoid of vesicles, but not over mitochondria or axolemma. In motoneurons, immunogold particles were seen over the Golgi apparatus in the soma and over lysosomes in both soma and dendrites. Quantification of glutamate-like immunoreactivity in 20 CTB-labeled and 20 CTB-negative boutons in the neuropil was found similar, indicating that CTB does not interfere with the immunocytochemical detection of neuronal epitopes such as the transmitter substance glutamate.  相似文献
