The results of percutaneous radiofrequency rhizotomy of lumbar spinal facets in 46 patients followed at least three months (mean 15 months) are reported and compared with those reported previously. Satisfactory pain relief three months after the procedure was achieved in 36.4 percent of patients without operations and in 41.7 percent of patients with operations other than fusion.No patient had previously undergone fusion.Treatment of low-back pain by using radio-frequency thermocoagulation of spinal facets is a simple, safe, and well-tolerated procedure. It can be used to relief of pain in spite of decreasing rates of success within the follow-up period.  相似文献

BackgroundBiliary papillomatosis is a rare type of papillomatosis. Depending on the location of the disease, radical surgical resection or liver transplants are the treatment of choice. These radical surgical options may be unrealistic in patients who are not surgical candidates or who are unwilling to consider surgery.AimsTo evaluate the effectiveness of endoscopic electrocoagulation for the biliary papillomatosis.MethodsIn this case series, we report six patients with unresectable biliary papillomatosis who underwent cholangioscopic electrocoagulation using needle knife and their clinic follow up information.ResultsAfter patients received cholangioscopy with electrocoagulation of the residual biliary papilloma, the daily T-tube drainage volume increased to 200–400 ml with improvement in the drainage content and significant relief of clinical symptoms, such as jaundice and abdominal pain.ConclusionThis method of using electrocoagulation to directly target and destroy tumor tissue is a safe and effective alternative for those with unresectable disease, and this method has shown to enhance T-tube drainage volume and improve patients’ overall clinical symptoms.  相似文献

Electrocoagulation is an effective treatment modality for localized cancer of the distal rectum. Proper selection remains the key to successful treatment. Of potentially curable patients with cancer of the rectum followed up for a median of five years, 69 per cent had no evidence of cancer at the end of the study period. Gross tumor morphology defined two distinct groups with regard to outcome after electrocoagulation. Ninety-two per cent of patients with polypoid/exophytic tumors as compared to 33 per cent of patients with ulcerative lesions had successful treatment. Based on these results, the authors believe that lesions that are exophytic represent early cancers with a low incidence of nodal spread and, as such, can be treated by electrocoagulation with confidence. As a palliative measure, the authors found electrocoagulation to yield equivocal results. Over the years, olive mill wastewater (OMW), which is generated from olive oil production has attracted considerable attention as one of the most polluted wastewaters due to its high concentrations of toxic compounds. It is a truism nowadays to guide research towards a novel and efficient technologies in order to treat this hazardous waste.In this paper, detoxification of OMW in an external loop airlift reactor (ALR) by electrocoagulation (EC) powered by photovoltaic solar system as a renewable and sustainable energy source was investigated. The effects of the operating parameters, such as electrolysis time, initial pH, current density (CD), and the axial position of the electrode were studied in continuous flow with two aluminum electrodes. A rate of abatement of about 79.24% for COD, 94.82% for polyphenols and 97.87% for dark color was observed within 40 min of treatment at 32.14 mA/cm2 current density, a position of the electrodes at 35 cm from the bottom of the riser compartment, and initial pH of 5.6. Furthermore, under these optimal conditions, the power supplied by photovoltaic cells led to amount of electrode dissolved (ELC) and specific energy consumption (SEC) of around 0.1118 kg/m3 and 9.86 kWh/m3, respectively. It has been proven that the EC treatment of OMW in ALR coupled with renewable energy source offers a low operation cost (0.2 USD/m3) compared to other treatment processes.  相似文献

ObjectiveParotidectomy is the most classic and unequivocal intervention for parotid neoplasm. The operative outcomes and postoperative complications of parotidectomy between harmonic scalpel and electrocautery gained more prominence in physician. In spite of much research work within the past years, there was an obvious lack of randomized controlled trial to resolve this question. Hence, a quantitative and qualitative meta-analysis was essential to evaluate the differences in these two types of hemostasis method.MethodThe major electronic databases, including Pubmed, Embase, Cochrane library, Google Scholar, China National Knowledge Infrastructure and Chinese Scientific and Technological Journal databases were using the key words “electrocautery”, “electrocoagulation”, “harmonic scalpel”, “ultrasonic scalpel”, “ultrasonic dissector”, “parotidectomy” and “parotid surgery”. 9 articles were included in our systematic review and meta-analysis. The operative time, intraoperative blood loss, hospital stay, salivary fistula and transient facial nerve paralysis were the outcome measures. Odds ratios (ORs) with 95% confidence intervals (CIs) were employed to evaluate the effect size for categorical outcomes and mean differences (MDs) with 95% confidence intervals (CIs) for continuous outcomes.ResultsIn our meta-analysis, there was a significant reduction in operation time [mean difference: ?20.97; 95%CI=(?24.02,?17.92); P < 0.00001], intraoperative blood loss [mean difference: ?20.75, 95%CI=(?22.32,?19.18); P < 0.00001], hospital stay [mean difference: ?0.83; 95%CI=(?1.10,?0.57); P < 0.00001], salivary fistula [ORs: 0.30, 95%CI=(0.08,1.14)] and transient facial nerve paralysis [OR:0.33, 95%CI=(0.19,0.58),P = 0.0001] in harmonic scalpel group compared with electrocautery group.ConclusionThis meta-analysis indicated that compared with electrocautery, harmonic scalpel (HS)was transcendent in the aspects of operative time, intraoperative blood loss, hospital stay, salivary fistula and transient facial nerve paralysis. The harmonic scalpel, as an efficient and useful instrument, was advocated in parotidectomy.  相似文献

目的 评价神经导航下经皮选择性半月神经节内靶点射频术治疗三叉神经痛的效果.方法 原发性三叉神经痛患者147例,性别不限,年龄32~99岁,VAS评分>8分.采用随机数字表法,将患者分为2组:C型臂组(C组,n =72)和神经导航组(N组,n=75).C组采用Hartel前入路穿刺法,C型臂引导下以卵圆孔为靶点进行穿刺;N组将头颅MRI影像传输至StealthStation手术导航系统进行重建,在半月神经节内确定靶点位置后设计穿刺径路和穿刺点.记录穿刺成功情况和穿刺及射频术相关不良事件发生情况.分别于术后1d、7d、1个月、6个月、12个月、24个月记录VAS评分,并根据VAS评分评价镇痛效果,术后1、24个月根据Barrow神经研究所评分系统评价治疗效果.结果 N组无一例患者发生穿刺相关不良事件及动眼神经损伤和耳鸣.与C组比较,N组首次手术穿刺成功率升高,术后不同时点镇痛有效率升高,治疗效果较好(P<0.05),神经定位时间和面部麻木发生率差异无统计学意义(P>0.05).结论 神经导航下经皮选择性半月神经节内靶点射频术治疗三叉神经痛的治疗效果好,复发率低,穿刺成功机率高,且并发症少.  相似文献

为探讨PPH加消痔灵注射配合电凝切术治疗混合痔的临床疗效,回顾性分析10年来对560例混合痔患者施行PPH加消痔灵注射配合电凝切术治疗的资料,观察其疗效。结果显示,560例患者中,治愈500例(89.3%),显效53例(9.5%),有效7例(1.2%),总有效率i00%。住院时间7~14d,平均(10±1)d。术后随访3个月至10年,吻合口出血3例,均经过再次缝扎止血后治愈;吻合口狭窄4例,予以局麻下行电凝切狭窄松解术后治愈。结果表明,PPH加消痔灵注射配合电凝切术治疗混合痔,术后患者痛苦小,恢复快,并发症少,肛门功能正常,肛门外观平整,近、远期未见复发,疗效确切。  相似文献

目的探讨结肠镜下分块分期高频电凝电切巨大肠息肉手术前后的护理及术中配合的有关问题.方法采用结肠镜下对74例巨大肠息肉患者进行分块分期高频电凝电切术.护士在术前做好病人和物品的准备,术中密切配合医生进行操作及术后的护理.结果74例巨大肠息肉患者中71例分块一次成功切除,3例分期切除,全部病例无出现大出血和肠穿孔等并发症.结论结肠镜检查是发现息肉的最好方法,也是治疗息肉的最佳手段.采用肠镜下切除巨大肠息肉具有效果好、创伤小、费用低等优点,大大减少了患者的痛苦和经济负担,使患者得到了有效的治疗,有良好的临床应用前景.  相似文献

高频微针电灼仪治疗24例中重度腋臭患者临床研究   总被引:1,自引:0,他引:1  
目的 评价使用高频微针电灼仪治疗腋臭的安全性及有效性.方法 收集2015年6月至2016年6月,北京协和医院皮肤科就诊的中重度腋臭患者24例,使用高频微针电灼仪(BodyTiteTM)进行单次治疗,治疗前后使用视觉-气味计分量表(VAS)评价腋下气味,生活质量SF-36量表评估健康相关生活质量,取患者腋下皮肤组织行病理学活检.结果 VAS量表评估显示,24例中22例达到并维持≥12周的症状缓解,平均气味评分下降率(VAS%)50% ~ 100%,1例治疗后12周腋臭复发,1例未获得临床缓解.SF-36量表评估显示,治疗前患者在社会功能、情感职能、精神健康领域得分[M(P0~P100)]分别为77.50(62.50~100.00)、66.67(33.30 ~ 100.00)、55.00(48.88 ~ 72.00),治疗后分别升至100.00(62.00 ~ 112.50)、100.00(33.30 ~ 110.00)、68.00 (48.00~ 80.00),治疗前后差异有统计学意义(均P< 0.05).组织病理显示,治疗后22例汗腺导管细胞出现明显变性、坏死,2例出现表皮损伤.安全性方面,1例治疗后出现单侧上肢疼痛,2例出现小面积皮肤烫伤样改变.术后平均恢复时间为7~ 14d.结论 高频微针电灼仪治疗腋臭临床有效率高,有良好的安全性,具有临床应用前景.  相似文献
