喉腔及喉咽部平滑肌肉瘤3例报告

为了解发生于喉腔及喉咽部的平滑肌肉瘤的临床特点，报告１例喉咽部平滑肌肉瘤和２例喉腔平滑肌肉瘤患者早期症状都不明显，且无特异性，临床表现与该部常见鳞癌极为相似，术前明确诊断有一定困难。     

Differentiated thyroid cancer invading the hypopharynx: A case report

Most thyroid cancers (90–95%) are well differentiated. Well differentiated cancers of the thyroid are usually confined to the thyroid capsule, making them amenable to isolated thyroid resection. Invasion of the upper aerodigestive tract by these cancers is infrequent and hypopharyngeal invasion is still rare. We report a 51 year old man with thyroid cancer invading the hypopharynx, who was successfully managed with complete resection along with a partial pharyngectomy. He is asymptomatic and disease free eighteen months after surgery. We advocate aggressive surgical extirpation of thyroid carcinoma invading the upper aerodigestive tract.     

全喉切除发音重建术后喉咽部颈段食管影像学研究

目的:研究全喉切除发音重建术后患者咽部、食管在发音时的作用机制、影像学表现。方法:采用全喉切除发音重建术后1年以上随访病人,利用频闪动态喉镜、纤维喉镜、食道钡餐透视下观察发音时分流口及食管粘膜振动情况并照像记录。结果:发音时喉刚部及食管粘膜均振动,参与发音,发不同音时食管振动及扩张方式不一,但声源位于我们设计的分流口。结论:本发音方法设计合理,发音流畅,进食时无明显病人不能耐受之呛咳。     

Adenosquamous Carcinoma of Hypopharynx with Intestinal-Phenotype

Adenosquamous carcinomas of the head and neck (ADSCs) are rare locally aggressive malignancies characterized by the presence of two distinctive components, a squamous cell carcinoma and an adenocarcinoma. The immunophenotype of the glandular component of ADSCs has only been rarely studied but has been reported as being positive for keratin 7 (CK7) and carcinoembryonic antigen (CEA) and negative for keratin 20 (CK20). Herein, we report a case of an ADSCs of the hypopharynx composed of a superficial squamous cell carcinoma and an adenocarcinoma with an intestinal phenotype. The patient was a 62 year-old male with a T2 N0 M0 squamous cell carcinoma (SCC) of uvula and palate and a T1 N0 M0 of right hypopharynx. The ADSCs of the hypopharynx was composed of a minimally invasive SCC and an adenocarcinoma with tubulo-glandular and cribriform architecture. The neoplastic glands were positive for CK7, CK20, CDX2, CEA and Villin. The patient underwent radiotherapy to both tumors and remains well with no evidence of recurrent disease 19 months after treatment. To the best of our knowledge, this is the first report of an ADSCs of the head and neck with an intestinal phenotype in its glandular component.     

下咽埋藏性异物的临床处理策略

目的总结下咽埋藏性异物的诊治经验及探讨手术径路的选择。方法回顾武汉市第一医院耳鼻咽喉科近6年就诊的12例下咽埋藏性异物患者的临床资料,对患者临床表现、异物种类、异物埋藏部位、取出或手术方式、病情转归等进行总结分析,所有患者随访半年以上。结果12例患者中,6例经口内镜下直接切开取出,或在等离子刀或CO2激光辅助下切开取出;3例行颈侧入路取出;1例电子喉镜下成功取出;1例自行消失;1例未做处理、异物在观察期内包埋机化。结论下咽埋藏性异物的诊断及定位依赖于影像学检查,取出方式取决于异物埋藏的部位,首选经口内镜下取出,必要时行颈侧切开径路。     

鼻咽纤维喉镜下咽喉部异物取出术

目的 :探讨检查及治疗喉咽和喉部隐匿或可疑性异物的新手段。方法 :回顾分析用鼻咽纤维喉镜检查治疗 4 8例喉咽及喉部隐匿或可疑性异物的临床资料。结果 :4 8例中 4 6例异物用纤维喉镜一次取出 ,另2例为局部炎性反应 ,经消炎治疗后痊愈。结论 :鼻咽纤维喉镜是检查和治疗喉咽及喉部隐匿性异物的首选方法     

A schwannoma mimicing hypopharyngeal carcinoma

Hypopharyngeal schwannoma is quite rare, with only nine cases presented in the literature to our knowledge. The presentation itself requires a distinction from malignant hypopharyngeal disease, which is usually the initial diagnosis. A transcervical approach and laryngomicrosurgery are two possible operating methods for the treatment of hypopharyngeal schwannoma. The selection of the surgical method depends on the size of the tumor. For this case, we preferred the transcervical approach without tracheotomy. In contrast to previous case reports, we were unable to trace the original nerve.
