Aims

This paper sets out to explore the unconscious meanings and functions of the “witch-child” aetiology appearing in the discourse of families from sub-Saharan Africa (in the present instance Bamileke), and its possible links with an “incestuel” (as opposed to incestuous) family dynamic. We have sought to show the possible points of transition between concepts that belong to different resisters, using a singular clinical situation which is nevertheless just one of the possible manifestations of the link between the witch-child and an “incestuel” family climate. From this particular issue, we also seek to show the permanent articulation between psyche and culture, and the way each casts light on the other. This dimension appeared to us to be particularly important for the reflection of the clinician, for a finer understanding of the clinical setting, and thus for the psychotherapeutic process enacted with these families.

Method

In order to explore the potential of the “witch-child” representation to put certain “unspeakable” histories occurring in an “incestuel” climate into words and images, we looked for the presence of unconscious fantasies common to these two entities, clinical and anthropological. Alongside, using one main clinical situation, we sought to show how these two different registers can connect, or even overlay one another. The history of Leo and his mother seemed to us to be the best suited to demonstrate the subject's attempts at a metaphorisation process. The study of these two dimensions enabled us to form a few hypotheses on the recourse to this singular aetiology.

Results

It can be seen that in certain situations reference to the witch-child can be seen as an attempt to put an “incestuel” experience fraught with numerous confusions into words. It can however be noted that the child thus pointed to as being a witch-child may not be the subject caught up in the “incestuel” atmosphere. Indeed, the two individuals concerned often belong to different generations. Reference to this cultural entity then serves as a means to put words on an unrepresented experience, and is in fact an attempt to find a psychic remedy. It should however be emphasised that this process of translation-figuration often fails, unless another person is there to receive the different languages in which it is expressed. Otherwise, the confusion that the subject is trying to escape merely increases. The careful study of these families thus shows that their relationship with the group from which they originate, the Bamileke, also reflects their pathological functioning, since a traditionally patrilinear structure is replaced by a family organisation centred on the rejection of otherness, placing the sole mother–daughter relationship at the centre of filiation, and thus taking on the mask of a matrilinear structure.

Discussion

These various elements suggest the need to take account of the articulations between psyche and culture. It is clear that it is important to hear the subject on the multiplicity and the complexity of his/her different components, and that the understanding of psychopathological situations can be enhanced by the contributions of anthropology. Humans should be reduced neither to their unconscious functioning, nor to their particular culture. This also shows that in some instances the position of a subject in relation to the group can be an indicator of a pathological family functioning.

Conclusion

First of all, this work shows the impossibility of putting a name on an “incestuel” family functioning and the ensuing confusion. Resorting to the aetiology of the witch-child appears here as a metaphor of this type of functioning, on account of the equivalent unconscious fantasies that underpin the two dynamics. However, only the articulation of the two registers, culture and psyche, in the setting of work on meaning, will enable the subjects concerned to embark on an elaboration. It is here that, by way of metaphorisation, this work cane enable a process of translation, figuration and individuation to occur.  相似文献

Objective: The clinical value of IgG anticardiolipin antibody in patients with Behçet's disease with or without vascular thrombosis was evaluated.   Methods: IgG isotype of anticardiolipin (aCL) antibody was assessed in 40 Behçet's disease (BD) patients with venous or arterial thrombosis, 40 BD patients without venous or arterial thrombosis and 80 healthy subjects as controls. The levels of IgG aCL were determined by an indirect ELISA method. Color Doppler Sonography, Magnetic Resonance Imaging and conventional angiography were the procedures used for other clinical evaluations. Results: Out of 40 patients with vascular thrombosis, 20(50%) were positive for low to moderate level of IgG aCL. In patients without thrombosis 22(55%) were positive for low to moderate level of IgG aCL while in none (0%) of the healthy subjects the IgG aCL was positive, neither low nor moderate. The number of patients with headache but having a normal cerebral magnetic resonance imaging (MRI), was higher in anticardiolipin positive patients without vascular thrombosis as compared to those with vascular thrombosis, (P = 0.001). Arthritis was noticed in both patents groups. 15% of aCL positive patients without thrombosis had arthritis as compared to none in aCL negative patients without thrombosis (P = 0.02).   Conclusion: The results of this study indicate that although the frequency of IgG aCL was found to be higher in Iranian patients with BD in comparison with the previous reports, except in arthritis the observed elevated IgG aCL does not correlate with clinical disease manifestations, or vascular thrombotic complications.  相似文献

This study was designed to verify the factorial structure of a French version of the State-Trait Anxiety Inventory for Children (STAI-C). This inventory being one of the most widely utilised to measure children's anxiety levels, it was believed necessary to obtain statistical clarification concerning this translation and its construction in order to extend its use. The present French version of the STAI-C was administered to 242 eight- to ten-year-old children. The results showed excellent internal consistency for both state anxiety and trait anxiety scales of the inventory. Furthermore, the three-factor model obtained in the original version of the test was confirmed, demonstrating good psychometric properties of this French version. It is hoped that the present translation, here supported by preliminary statistical results, will allow, once further tested, a greater use of the State-Trait Anxiety Inventory for Children in French-speaking environments.  相似文献

ObjectiveThe aim of this study was to adapt and validate the Tunisian version of the Oswestry Disability Index (ODI) within a Saudi Arabian population.MethodThe translation of items 8 and 10 taken out of the Tunisian version was conducted according to Beaton's method. Adaptations were made after a pilot study on 100 patients. The validation study included 100 patients suffering from chronic low back pain aged 18 to 65 years old. Intra-observer reliability was assessed using the intra-class coefficient (ICC). Spearman rank correlation coefficient, the Kruskall–Wallis test and factor analysis were used to evaluate construct validity (convergent and divergent validity). Internal consistency was assessed by Cronbach's alpha coefficient.ResultsOne hundred Saudi patients were included in the study. Intra-observer reliability was excellent (ICC: 0.99). The correlations of the index with the VAS pain scale (r = 0.708), the Roland–Morris Low Back Pain Disability (r = 0.656), and the Quebec Back Pain Disability Scale (r = 0.792) suggest good construct validity. Factor analysis unveiled two main factors explaining a cumulative percentage variance of 63.5%. The first factor represents static activities and the second factor represents dynamic activities.ConclusionThe Arabic version of the ODI adapted to the Saudi population has high metrological qualities. Further studies assessing its responsiveness to change should be conducted.  相似文献

Aim

The aim of this study was to interpret and validate a French version of the Oswestry disability index (ODI), using a cross-cultural validation method. The validity and reliability of the questionnaire was assessed in order to ensure the psychometric characteristics.

Method

The cross-cultural validation was carried out according to Beaton's methodology. The study was conducted with 41 patients suffering from low back pain. The correlation between the ODI and the Roland-Morris disability questionnaire (RMDQ), the medical outcome survey short form-36 (MOS SF-36) and a pain visual analogical scale (VAS) was assessed.

Results

The validity of the Oswestry questionnaire was studied using the Cronbach Alpha coefficient calculation: 0.87 (n = 36). The significant correlation between the ODI and RMDQ was 0.8 (P < 0.001, n = 41) and 0.71 (P < 0.001, n = 36) for the pain VAS. The correlation between the ODI and certain subscales (physical functioning 0.7 (P < 0.001, n = 41), physical role 0.49 et bodily pain 0.73 (P < 0.001, n = 41)) of the MOS SF-36 were equally significant. The reproducibility of the ODI was calculated using the Wilcoxon matched pairs test: there was no significant difference for eight out of ten sections or for the final score.

Conclusion

This French translation of the ODI should be considered as valid and reliable. It should be used for any future clinical studies carried out using French language patients. Complimentary studies must be completed in order to assess its sensitivity to change in the event of any modifications in the patients functional capacity.  相似文献
