目的 探讨有效且经济的治疗肺透明膜病的方案.方法 将体重为1 300~2 500 g确诊为肺透明膜病的Ⅰ、Ⅱ级早产儿随机分为A、B两组,两组基础治疗相同.A组(54例)尽早应用NCPAP,不用PS.B组(26例)尽早用PS后,仍需用NCPAP治疗.结果 A组治愈率96.2%,B组治愈率96.1%.两组比较无显著差异(P>0.05).但B组治疗费用平均多400~4 500元.结论 NCPAP治疗肺透明膜病疗效可靠,治愈率高,且较B组价格便宜.并发症相对少,操作简便,尤其适合在贫困地区及城市较低收入家庭的患儿中应用.  相似文献

胃癌10和11组淋巴结转移及其清扫   总被引:5,自引:0,他引:5  
目的:研究胃癌脾门淋巴结(No.10)和脾动脉周围淋巴结(No.11)转移规律,进一步探讨No.10和11清扫的必要性和方法。方法:1991年-2000年132例行全胃切除,D2以上淋巴结清扫的胃癌患者,回顾性研究临床病理资料,包括性别、年龄、肿瘤部位、大小、浸润深度、病理类型、其他淋巴结转移等对No.10和11转移的影响,比较全胃切除和全胃联合脏器切除的并发症发生率。结果:胃癌具有较高的No.10或11淋巴结转移率(18.9%),Logistic回归分析表明,胃癌部位、病理类型、浸润深度和大小弯淋巴结转移五项临床病理指标影响No.10和U转移率。联合脏器切除的并发症发生率(32%)明显高于单纯全胃切除(11.2%),联合肢体尾切除增加肠下脓肿发生率,而脾切除并不增加全胃切除的危险性。结论:No.10和11在胃癌有较高的转移率和特定的转移规律,预防性和治疗性的清扫实属必要,联合左侧肢体尾加脾切除增加手术危险性,应严格掌握的适应证。而保留胰腺,切除脾血管和脾清扫No.10.和ll淋巴结合理可靠。  相似文献

Interactions between rabbit-γ-immunoglobulins and model membranes (lipid monalayers, planar lipid bilayers, liposomes) have been investigated. No significant interaction was observed with immunoglobulins. However, immunoglobulins dialysed first vs aqueous buffer having pH 2 or 3 and then dialysed against pH 7 buffer presumably adopt a new conformation which allows their bindings to model membranes. This binding is hydrophobic and the immunoglobulin region interacting with the lipid acyl chains is probably located in the heavy chain, as suggested by labelling in this region by a photosensitive probe previously incorporated into the lipid hydrophobic core. Cleavage at the hinge region by papain or pepsin, or heating above 38°C, induces the loss of the hydrophobic conformation responsible for hydrophobic bindings. The binding capacity of immunoglobulins heated above 38°C is restored after momentary dialysis at pH 2. The possible existence of two Ig isomers is discussed in relation to the mechanism of γ-immunoglobulin passage through the endoplasmic membrane and fixation into the plasma membrane.  相似文献

The actions of the neurotoxic amino acids folate and kainate have been compared on ortho-and antidromic responses evoked in CA1, CA3 and the dentate gyrus of slices of rat hippocampus maintained in vitro. Both in CA1 and the dentate gyrus superfusion of these acids caused an increase in amplitude of the population spike discharging from an excitatory postsynaptic potential which either remained unaffected or was reduced. In the CA3 region kainate and folate had broadly similar actions to enhance the probability of cell firing to synaptic excitation, and also caused epileptiform discharges to occur spontaneously or in response to electrical stimulation. Spontaneous and evoked population bursts in CA3 did not persist in low calcium/high magnesium medium indicating their dependence on intact synaptic transmission; spontaneously occurring bursts in CA1 were eliminated with the latter treatment or when the axonal connections between it and CA3 were cut. Following folate superfusion the commissural-evoked response in CA3 showed large and variable shifts of the latency which were dependent on the stimulus intensity and its timing after a spontaneous population discharge. Although all of the effects of folate were reproduced by bicuculline, no evidence for a decreased recurrent inhibition in CA1 was obtained although this was observed with kainate. The finding that folate and kainate produced their effects in the absence of a detectable effect on the antidromic population spike suggests a mechanism of action other than neuronal depolarization. The implications of these data for the neurotoxic mechanism(s) and the receptor homologies of folate and kainate are discussed.  相似文献

Metabolic pathways leading to lipid biosynthesis in four different developmental stages of Schistosoma mansoni were explored and quantified by incubation in the presence of labeled precursors in a chemically defined medium. At the schistosomulum stage and in male, female, or paired worms, glycerol and oleate incorporation into neutral lipids, mainly in the form of triacylglycerols, was greater than into phospholipids, whereas in 11-and 15-day-old worms, synthesis mainly led to phospholipids. Incorporation into phospholipids was recovered largely in phosphatidylcholine, and distribution into other phospholipids depended on the developmental stage. Incorporation of choline and ethanolamine into their respective phospholipids represented up to 15% of the parasitic phospholipid content. The formation of phosphatidylcholine by phosphatidylethanolamine methylation occurred mainly in the immature parasitic stages. Inositol incorporation was also measurable, whereas [14C]serine incorporation was low or undetectable. Addition of 1-palmitoyl-2-[14C]oleyl phosphatidylcholine revealed a very high uptake of this phospholipid by the immature stages but further metabolism was not detectable. In contrast, adult S. mansoni were completely unable to take up or absorb this exogenous phospholipid. The most striking aspect of this study was the relatively high metabolic activity in 11-day-old worms and the lower but sustained activity on day 15 and at the schistosomulum stage. By comparison, biosynthetic activity in adult S. mansoni, on which research studies have been focused until now, was very low. We also discuss the participation of lipid metabolism in the constant renewal of the membrane complex which is essential to parasitism by S. mansoni.  相似文献

The perinatal period of brain is characterized by dynamic changes in structure and high propensity for epilepsy. Animal models have shown that alterations of AMPA receptor (AMPAR) assembly or function may be related to seizure-induced cell damage, long-lasting impairments in brain development and seizure threshold. However, effects of earlier epileptiform discharges on AMPAR composition and sub-cellular distribution remain understudied. In this study, we analyzed age-dependent variation of relative GluR1 and GluR2 protein levels in primary cultured rat cortical neurons at 7 DIV, 12 DIV, 17 DIV and 21 DIV. By inducing a single event of epileptiform activity at 6 DIV, we tested the effects of early-life seizure-like insults on AMPAR subunit distribution. We found a significant increase in synaptosomal membrane GluR1 expression in magnesium-free (MGF) medium-treated neurons at each time point detected (p < 0.05), while GluR2 expression increased at 7 DIV, and declined at 17 DIV and 21 DIV respectively (p < 0.05). That is, a trend of high GluR1 with much lower GluR2 expression on the surface membrane of epileptiform discharges experienced neurons over time in culture was presented. These findings in an in vitro model of early-life seizure may inform rodent models of epilepsy, as well as the cellular mechanism involved in epilepsy-associated brain dysfunction.  相似文献

Summary: Bis(hydroxy)telechelic bisphenol A polycarbonate (PC) was prepared via melt polycondensation of bisphenol A (BPA) and diphenyl carbonate (DPC) using lanthanum(III ) acetylacetonate as a catalyst for transesterification. Subsequently, the polycarbonate was converted to a bifunctional macroinitiator for atom transfer radical polymerization (ATRP) with the reagent, α‐chlorophenylacetyl chloride. The macroinitiator was used for the polymerization of styrene (S) and methyl methacrylate (MMA) to give PSblock‐PC‐blockPS and PMMA‐block‐PC‐block‐PMMA triblock copolymers. These block copolymers were characterized by NMR and GPC. When styrene and methyl methacrylate were used in large excess, significant shifts toward high molecular weights were observed with quantitative consumption of the macroinitiator. Several ligands were studied in combination with CuCl as the ATRP catalyst. Kinetic studies reveal the controlled nature of the polymerization reaction for all the ligands used.

The immunologic and physiologic status of a group of symptomatic and asymptomatic pigeon breeders was studied in an attempt to define the immunologic events occurring in pigeon breeders' disease. Antibody activity to antigen(s) present in pigeon dropping extract (PDE) and pigeon serum (PS) was detected in the serum of both symptomatic and asymptomatic breeders. Antibody activity, however, tended to be greater in the symptomatic pigeon breeders. When subjects were challenged with PS via aerosol, serum complement activity became depressed only in asymptomatic patients. Cellular hypersensitivity to antigens present in PDE was detected in vitro in peripheral lymphocyte populations of 4 of 5 symptomatic breeders and in none of the asymptomatic breeders; cellular hypersensitivity to antigens in PS was not demonstrated in any of the individuals tested. These findings indicate that cell-mediated hypersensitivity, as well as humoral immunologic processes, may be involved in the pathogenesis of the hypersensitivity pneumonitis found in pigeon breeders.  相似文献

[目的]探讨雌激素调节蛋白(PS2)在乳腺浸润性导管癌中的表达及其与临床病理因素间的关系。[方法]采用免疫组化方法检测155例乳腺浸润性导管癌中ER、PR和PS2的表达。[结果]全组PS2表达阳性率38.1%。在ER阳性和阴性病例中,PS2阳性率分别为48.6%和29.4%(χ2=5.977,P<0.05);在PR阳性和阴性组中,PS2阳性率分别为50.8%和28.9%(χ2=7.664,P<0.01)。PS2的表达与病人年龄、月经状况、肿瘤大小无关(P>0.05);而与腋淋巴结状况(P<0.001)、临床分期(P<0.05)有关。此外,PS2的表达在民族间也存在一定差异,但无统计学意义。[结论]乳腺浸润性导管癌中PS2的表达与ER、PR呈正相关关系,并与腋淋巴结状况和临床分期有关。PS2可以作为预测乳腺癌预后的一项重要指标。  相似文献
