Because of its neurocognitive enhancing effects, Gingko biloba has emerged as amongst the most commonly used herbal products. We report a liver transplant recipient with potentially life-threatening toxicity resulting from Gingko biloba use. Seven days after a second liver transplantation for recurrent hepatitisB virus infection, subphrenic hematoma was documented in a 59-year-old Korean patient. Failure to control bleeding with CT-guided drainage necessitated exploratory laparotomy for the evacuation of a large subphrenic hematoma. Three weeks later, an episode of vitreous hemorrhage was documented. Unbeknownst to his care providers, the patient had been consuming Gingko biloba throughout the postoperative period. No further bleeding episodes occurred after the cessation of Gingko biloba use. Unrecognized use of herbal products may be associated with serious side effects and adverse clinical sequelae in transplant recipients. Given their increasing popularity, the use of herbal products should be routinely sought as part of the history in transplant recipients.  相似文献

Total global biodiversity is estimated at between 3 and 500 × 106 species of prokaryote and eukaryote organisms spread across 70 or more phyla. The marine macrofauna alone are estimated between 0.5 and 30 × 106 species and represents a broader range of taxonomic diversity than that found in the terrestrial environment, which has been the traditional source of natural products. With a typical eukaryote possessing 50,000 genes, the global marine macrofauna are the source of 2.5 × 1010 to 1.5 × 1012 primary products and an associated extensive range of secondary products. However, only a few thousand novel compounds from marine organisms have been described. These compounds have proven unique in chemical and pharmacological terms but, as yet, no therapeutic agents have resulted. Given a broader drug discovery strategy, and facilitated by technological advances, it is predicted that the characterisation of marine chemical diversity will be accelerated. Strategies for drug discovery from the virtually untapped chemical diversity of marine organisms are discussed. © 1994 Wiley-Less, Inc.  相似文献

A food frequency of consumption questionnaire was completed by 137 diabetic outpatients attending the University of Wales Hospital in Cardiff, to provide information about the use of special dietary products.
Seventy-four per cent of the diabetics used special dietary products, the most popular of which were artificial sweeteners (45%) and preserves (47%), followed by squash (34%), sweets (31%) and chocolate (31%). Twenty per cent of diabetics consumed biscuits and tinned fruit. Cake and other products (e.g. jelly), were used by less than 10% of the respondents. Over half of all the diabetics consumed one or more products on a daily basis. The use of special products bore no significant relationship to the sex of the respondents, nor to the duration of the diabetes. However, a significantly higher proportion of the Insulin Dependent Diabetics (IDDM) group used dietary products compared with the Non-Insulin Dependent Diabetics (NIDDM) group. This can be explained largely by the differences in age between the diabetics; the under-18-year-old age group (who were all IDDM respondents) were the greatest users of sweets, chocolate and squash.
Forty-three per cent of diabetics who did not use special food products cited at least one reason for non-use. The reasons included dietetic advice (NIDDM respondents only), high cost, poor palatability, lack of availability and unsuitability for other members of the family.  相似文献

目的 回顾性研究终末期肾脏疾病(end stage renal disease,ESRD)血液透析病人原发疾病构成及其相关因素。方法 收集ESRD血液透析病人1108例,分析其原发疾病构成、年龄、性别、透析治疗持续时间、转归、生存率和死亡原因。结果 301例病人透析时间超过3个月,其中43.9%仍在进行血液透析治疗,最长者已存活13年;13.0%病人已行肾脏移植;19.9%转至外院治疗;7.3%改为腹膜透析;1.3%因经济困难放弃治疗;14.6%病人死亡。ESRD维持性血液透析病人1年生存率为93.53%,3年生存率为68.92%。5年生存率62.51%。死亡原因为心血管事件占54.5%,脑血管意外占20.5%。结论 本组病人以中、老年为主,男性多见。ESRD血液透析病人主要原发疾病为慢性肾小球肾炎,主要死亡原因为心血管事件和脑血管意外。  相似文献

利用1,1,6-四苯基-2,4-已二炔-1,6-二醇(I)的包结能力,可简单而迅速地从蛇床子中选择分离出有效成分欧前胡素(2)和花椒毒素(3),收率分别为0.054%(2)和0.002%(3)。本文用UV,IR,1HNMR和13CNMR验证了2和3的化学结构,并用HPLC检验了2和3的纯度。X-射线衍射晶体结构分析表明主体分子I与组分2靠氢键形成包结物,它们的摩尔比为1:2,氢键长为2.8612A。包结物晶体属单斜晶系,空间群为已P21/C,晶胞参数:a=15.468(5)A,b=8.595(3)A,c=18.663(7)A,β=93.64(5)°,Z=4和R=0.088。  相似文献
