脂肪抽吸术对胰岛素敏感性及脂质代谢的影响

目的了解脂肪抽吸术对胰岛素抵抗与脂质代谢的影响,并探讨其临床意义。方法对20例接受腹部脂肪抽吸术者术前及术后2个月的血清三酰甘油、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇,胰岛素敏感性进行测量并进行比较。结果术后与脂肪抽吸术前比较,术后胰岛素敏感性增强（P〈0.01）,血清总胆固醇与低密度脂蛋白胆固醇水平下降。结论腹部脂肪抽吸术在去除大量皮下脂肪的同时,对胰岛素抵抗、脂质代谢等心脑血管危险因素也有非常有益的影响。     

肿胀局麻技术的临床应用

目的探讨肿胀技术在体表外科手术中的应用价值。方法随机选择体表肿物与先天性畸形、体表瘢痕、乳房整形、脂肪抽吸、腹壁整形、除皱、隆胸、褥疮、面瘫及颅骨电烧伤等２３０例体表外科手术应用肿胀技术，成人１６４例，完全采用肿胀法局麻技术，儿童６６例，在全麻下应用肿胀技术。并与相应手术非肿胀法下施行进行了比较。结果脂肪抽吸术中利多卡因用量可达３０～６０ｍｇ／ｋｇ体重。肿胀技术具有易于进行解剖间隙的分离、减少术中失血、增强麻醉效果、增加手术安全性和术后恢复快等优点。结论肿胀技术可推广到体表外科手术中广泛应用，对儿童更为适宜，对减少输血造成的疾病具有重要意义。     

Evacuation of hematomas using liposuction technology: Technique and literature review

Established nonexpanding hematomas can be successfully treated with minimal morbidity using standard liposucstion techniques at the bedside or in an outpatient setting under local anesthesia. The authors presents a series of eight patients and discuss current concepts of dealing with this common and distressing surgical complication.     

改良腹壁整形术

目的为进一步增进脂肪抽吸的效果,解决脂肪抽吸后皮肤松弛、皱纹增多、腹部仍有膨隆等的不足.方法在腹壁整形治疗中,联合采用脂肪抽吸术和腹壁整形术.首先根据腹部脂肪堆积的位置和高低程度的不同,均匀抽吸脂肪.完成脂肪抽吸后,根据术前皮肤、腹肌的松弛情况,再行腹壁整形术,沿腹肌筋膜表面分离,缩紧腹肌,向下拉动腹壁皮肤,确定多余皮肤的切除.结果自2003年至今,共治疗10例,手术效果良好,腹部形态明显改善,受术者满意.结论施行腹壁整形治疗时需要考虑腹部皮肤、皮下脂肪、腹壁肌肉以及腹内脂肪等多方面的情况,综合评估.单纯的脂肪抽吸只能解决皮下脂肪的堆积膨隆;而结合传统腹壁整形术和脂肪抽吸术的特点,针对腹部具体条件,有针对性的联合采用脂肪抽吸和腹壁整形等不同的手术方法,才能获得更为理想的腹壁整形效果.

Liposculpturing by Syringe Technique in Obesity

The author recommends liposuction by syringe technique instead of by the lipoaspirator machine. This technique has been applied to obese patients who were further stimulated to lose weight by dieting.     

Superficial liposuction: A new application of the technique for aged and flaccid skin

From 1982 to 1991, 2500 patients underwent liposuction to thin the superficial areolar layer of fat. The patients were of all ages and had different types of skin. From the results the author concludes that the new superficial liposculpting, based on the controlled scar retraction of the thin cutaneous adipose flap remaining after the treatment, allows for very good results even in very flaccid and aged skin. A digital molding of the residual fat also improves the contouring. So-called cellulite is finally meliorated.Presented at the First and Second International RAPS Congress, Sao Paulo, Brasil, March 1989 and 1991 and at the Nineth LSNA Meeting, Seattle, Washington, September 1991     

Syringe liposculpture: A two-year experience

Syringe liposcupture is a method that combines two relatively new techniques of plastic surgery: syringe liposuction and fat grafting. We can reshape the face and the body by removing localized fat deposits and reinjecting this fat where needed. When we do not reinject, we call the technique reduction liposculpture. In 1989 we introduced a new technique—superficial syringe liposculpture—to treat patients with flaccid skin, superficial irregularities or depressions, cellulite, and liposuction sequelae. The technique combines syringe liposculpture, superficial liposuction, and our method of treating skin irregularities by breaking the fibrous adherences and injecting fat superficially.     

Two arcane areas in liposuction: The banana and the sensuous triangle

Two new approaches in suction lipectomy of the buttocks region are described: liposuction of the banana and liposuction of the sensuous triangle. The banana is the highest part of the posterior thigh just below the buttocks crease. It appears only in certain individuals and appears as a buldge under the buttocks crease. The preferred approach to liposuction of the banana is discussed as well as a theory as to its etiology. A common complication of liposuction of this area is ptosis of the buttocks crease. Methods of treatment of this complication are also discussed. The second topic addressed is liposuction of the sensuous triangle which is at the junction of the lateral buttocks, lateral thigh, and posterior thigh. The result of suctioning this area is a more athletic-appearing buttocks region.Presented at the 7th Annual Meeting of the Lipoplasty Society of North America, San Francisco, California, October 29, 1989
