现代观念与电视剧创作

转型期的文学艺术总体格局出现了引人注目的变化 ,电视剧已成为百姓日常文化消费的主要对象 ;在艺术风格上呈现世俗化、平民化趋向的电视剧创作 ,同样要“代表先进文化的前进方向” ,这就要求创作者更新并优化观念。现代观念的涵义是丰富的 ,与时俱进的 ;一些电视剧的缺失都可以从中找到原因。而近几年涌现的一批在大众化、精品化方面取得重大进展的多种题材的电视剧 ,关键在于观念的突破带动了思想和艺术的创新     

浅析命门穴之“门户”作用

影响出国维和待命士兵应付方式的相关因素

目的 了解参加维和部队士兵在出国前的心理应付方式,探讨应激性心理障碍与应付方式的关系。方法 采用应付方式问卷量表（CSQ）,艾森克（EPQ）个性问卷量表和生活事件量表,Zung焦虑自评量表（SAS）、抑郁自评量表（SDS）,对维和待命士兵进行测评,分组进行对照分析。结果 年龄与“自责”、“幻想”因子呈负相关,文化程度与“求助”因子呈负相关,军龄与“解决问题”呈正相关,与“自责”呈负相关;正性事件分值与“求助”、“合理化”因子呈正相关,负性事件分值与“自责”、“退避”呈正相关,生活事件总值与“自责”、“退避”呈正相关。结论 士兵应付方式与年龄、军龄、文化程度、性格具有相关性,焦虑抑郁组应付方式和对照组有较大差异。在士兵中进行应付方式的教育和训练,有益于平战时的心理健康。     

排毒养颜胶囊对非酶体系产生的·OH及的清除作用

目的 :通过对活性氧自由基的清除作用研究排毒养颜胶囊排毒养颜的部分作用机理。方法 :用化学发光法测定该中药方剂水浸提液对非酶体系产生的超氧阴离子自由基 (O ·2 )和羟自由基 (·OH)的清除作用。结果 :该浸提液清除O ·2 的SC5 0 为 2 7× 1 0 5 g·ml 1 ,相当于 1 0ng·ml 1 SOD的活性 ;清除·OH的SC5 0 为 6 5× 1 0 5 g·ml 1 ,相当于 1× 1 0 5 mol·L 1 硫脲的活性。当浸提液浓度较高时 ,它对O ·2 和·OH的清除率均可达 90 %以上。结论 :这种较强的抗自由基活性可能是该中药方剂排毒养颜的部分作用机理     

命门动静观——兼论中医关于内分泌学的早期发现

明代命门学说的形成是中国古代医家为探索生命的起源及其内在规律所作的伟大尝试,反映出中国医药学早期对人体内分泌功能的发现和认识[1]。明代医家关于命门的解剖位置及其功能的描述,与现代医学肾上腺的位置和生理功能基本相合。"命门动静观"——即命门是五脏整合的生殖调节中枢,平时处在半关闭的状态,静以蓄养,为生殖作物质和能量方面的储备;激发态下命门全面开放,整合脏腑,使之同步;奇经八脉为命门开放、整合五脏的通道。命门为生命之原,相火之主,具有阴阳相济的特性,因而有别于五脏、十二经脉,反映出太极层次下生命活动的内在规律和调节机制。这项研究澄清了长期以来"肾命不分"的混淆。命门学说研究的深入为在生命的高层次上探索中西医结合之路开辟了广阔的前景。     

Tc-99m diethylenetriamine pentaacetic acid (DTPA): Is it reliable for assessment of methotrexate-induced cumulative effect on renal filtration in rheumatoid arthritis patients?

Methotrexate[MTX] is commonly employed as the initial DMARD used for treatment of Rheumatoid arthritis[RA]. We aimed to contribute to the safety profile of MTX by assessing its cumulative effect on renal filtration. Fifty two RA adult females with normal base-line serum creatinine and GFR at the initial diagnosis of the disease were included. Group-1[G1] included 30 patients[mean age 40.4 ± 4.4 years] on MTX and NSAIDS, while 22 RA patients[mean age 38.5 ± 8.2 years] who received NSAIDs only served as the control group[G2]. Renal function was assessed by GFR-measurement using Technetium diethylenetriamine-pentaacetic acid[Tc-99 m-DTPA] at the point of the study time corresponding to disease duration. 21/30[70%] in G1 showed reduced GFR compared to 6/22[27.3%] in G2[P0.007] with 3.3 ± 0.5% annual reduction of GFR. Reduced GFR in G1 showed significant negative correlation with age[r = ?0.396, P = 0.005], MTX-cumulative dose[r = ?0.263, P = 0.049], MTX-intake duration[r = ?0.293, P = 0.031] and NSAID-intake duration[r = ?0.344, P = 0.014]. Low dose MTX has a slow cumulative effect on renal filtration manifested by GFR reduction over time that could be monitored by Tc-99 m DTPA.     

Reducing the probability of falsely elevated HbA1c results in diabetic patients by applying automated and educative HbA1c re-testing intervals

IntroductionToo frequent HbA1c measurements may lead to unnecessary treatment modifications of diabetic patients. The aim of this study was to estimate the percentage of falsely elevated HbA1c results in two hospitals, Landeskrankenhaus/Uniklinikum Salzburg (LKH) and Landesklinik St. Veit (STV), as well as to retrospectively investigate the effect of an automated and an educative 60-day re-testing interval (RTI).MethodsThe amount of estimated falsely elevated results (eFER), based on odds calculated using the baseline and the follow-up values and the time between these measurements, the number of HbA1c re-testings within 60 days as well as the overall number of ordered and performed HbA1c analyses were calculated. In LKH, an automated algorithm cancelling inappropriate HbA1c testing was applied, and in STV, educational actions were taken.ResultsBefore RTI-implementation, eFER were 0.9% and 2.1% and within-60-days-re-testing were 15.0% and 7.4% of cases in LKH and STV, respectively. After RTI-implementation, these numbers decreased to 0.2% (p < .001) and 1.8% (p = .869) and within-60-days-re-testing decreased to 1.1% (p < .001) and 3.6% (p = .003) in LKH and STV, respectively. Median monthly HbA1c measurements decreased by 15.8% (p < .001) and 21.1% (p = .002) in LKH and STV, respectively.ConclusionBoth the educational and the automated 60-day-RTI were proven to be efficient in reducing overall HbA1c measurements, re-testing within 60 days and eFER.     

Effect of sexual diary keeping and self-evaluation on female sexual function and depression: A pilot study

Objectives The aim of the trial was to assess the effect of self-evaluation and sexual diary keeping on female sexual function and depressive symptoms in women diagnosed with sexual dysfunction.

Methods A single-arm non-randomised trial included 30 women (53 ± 7 years of age) with female sexual dysfunction (Female Sexual Function Index [FSFI] < 27) and a stable partnership duration of 5–40 years. Female sexual function was assessed by sexual, psychological and gynaecological history taking and validated questionnaires including the FSFI, Female Sexual Distress Scale (FSDS) and Hamilton Depression Scale (HDS), before and after 4 weeks of sexual diary keeping.

Results A subjective improvement in communication of sexual problems was reported by 60% of participants; no participants reported any worsening of communication. FSFI and FSDS scores were, respectively, 18.0 ± 7.7 and 22.0 ± 10.0 at baseline and 20.2 ± 7.2 and 20.6 ± 11.5 after 4 weeks. HDS score decreased from 6.0 ± 4.0 at baseline to 4.4 ± 2.7 after 4 weeks (p = 0.042).

Conclusions Self-evaluation and sexual diary keeping may improve aspects of sexual life, such as couple communication, without a direct effect on variables measured with validated questionnaires on different domains of sexual function.     

论《内经》中的“守神”“守机”“守气”

在针刺过程中,医者不能只拘守手部形式,针刺关键在于"治神"。治神是医者手法的核心,贯穿于整个施治过程中。以神"侯气"、"调气",进而"调神"、"调形体"。其理论在《内经》中已有叙述,具体包括:守神、守机和守气。这三者是对针灸医生针刺手法的要求,也是衡量针灸医生水平高低的标准之一。然而针道易陈而难入,针灸医生须认真钻研针刺手法和心法。针刺治神在于医者必须"心领神会",方能感悟。
