Rhabdomyolysis is a syndrome of skeletal muscle injury with release of cellular constituents such as potassium,phosphate,urate and intracellular proteins such as myoglobin into the circulation,which may cause complications including acute kidney injury,electrolyte disturbance and cardiac instability.Abnormal liver function tests are frequently observed in cases of severe rhabdomyolysis.Typically,there is an increase in serum aminotransferases,namely aspartate aminotransferase and alanine aminotransferase.This raises the question of liver injury and often triggers a pathway of investigation which may lead to a liver biopsy.However,muscle can also be a source of the increased aminotransferase activity.This review discusses the dilemma of finding abnormal liver function tests in the setting of muscle injury and the potential implications of such an association.It delves into some of the clinical and experimental evidence for correlating muscle injury to raised aminotransferases,and discusses pathophysiological mechanisms such as oxidative stress which may cause actual liver injury.Serum aminotransferases lack tissue specificity to allow clinicians to distinguish primary liver injury from muscle injury.This review also explores potential approaches to improve the accuracy of our diagnostic tools,so that excessive or unnecessary liver investigations can be avoided. 相似文献

Immunochromatography (ICG) is highly used in clinical settings for rotavirus (RV) diagnosis. The specificity of the tests differs by brand type and is not 100%, therefore its use when the prevalence of the disease is low (i.e. in vaccinated children) may result in a proportion of false positive diagnoses.In some areas, vaccine effectiveness studies or surveillance is done using ICG. Our objective was to estimate the validity of ICG test in vaccinated children, and estimate the number of false positive results in the Valencian Region of Spain, where all RV infections are diagnosed using ICG and are not confirmed by PCR.Population based registries were used to identify all results from the RV antigen tests performed between January 2008 and June 2012 in children under 37 months. Hospitalization and vaccination status of the patients were obtained by linking different databases through a unique identification number. The Positive Predictive Value of the ICG test depending on the vaccination status of the child, hospitalization and the rotavirus season was estimated by a Bayesian model of latent classes.Of the 48,833 tests with valid results, 9429 were done in vaccinated children, and of those 3963 (42%) during the rotavirus season. The prevalence of positive results in vaccinated varied from 2.9 to 21.4% of the tests depending on the hospitalization and seasonality. The estimated PPV also varied from 27.1 to 84.6% when stratified by these two parameters. Globally it is calculated that approximately 267 out of the 520 (51.3%) positives in vaccinated children were false positive tests.The large percentage of false positives, due to an excessive number of tests in vaccinated and out of the RV season, if interpreted as vaccine failures, can cause a loss of confidence in the vaccine and lower the estimates of vaccine effectiveness. 相似文献

Dengue vaccination would enhance the control of dengue, one of the most frequent vector-borne viral diseases globally. CYD-TDV is the first dengue vaccine to be licensed, but global uptake has been hampered due to its use being limited to seropositive persons aged 9 years and above, and the need for a 3-dose schedule. The Partnership for Dengue Control (PDC) organized a meeting with key opinion leaders and stakeholders to deliberate on implementation strategies for the use of CYD-TDV. New data have emerged that support the shortening of the primary schedule from a 3 to 2 dose schedule, extending the age range below 9 to 6 years of age, and expanding the indication from endemic populations to also include travelers to endemic areas. Cost-effectiveness may improve with the modified 2-dose regimen and with multiple testing. Strategies to implement a dengue vaccination program have been developed, in particular school-based strategies. A range of delivery scenarios can then be considered, using various settings for each step of the intervention. However, several challenges remain, including communication about limiting the use of this vaccine to seropositive individuals only. Affordability will vary from country to country, as will government commitment and community acceptance. Well-tailored communication strategies that target key stakeholders are expected to make up a significant part of any future dengue vaccination program. 相似文献

Determining aortic stenosis (AS) severity is clinically important. Calculating aortic valve (AV) area by means of the continuity equation assumes a circular left ventricular outflow tract (LVOT). The full impact of this assumption in calculating AV area is unknown. Predictors of noncircular LVOT shape in patients with AS are undefined.In 109 adult patients with AS who underwent multiplanar transesophageal echocardiography, we calculated AV area by means of the standard continuity method and by a modified method involving planimetric LVOT area.We found 54 circular, 37 horizontal-oval, 8 vertical-oval, and 10 irregular LVOTs. Area derived by direct planimetry correlated better with the modified than the standard continuity method (r=0.89 vs r=0.85; both P=0.0001). Valve areas of patients with mild, moderate, or severe AS by planimetry were more often mischaracterized with use of the standard than modified method (29 vs 18; P <0.0001). Horizontal-oval AV area derived by planimetry (1.28 ± 0.55 cm2) was underestimated by the standard method (1.05 ± 0.47 cm2; P=0.001), but not by the modified method. Congenital AV morphology and low cardiac index were the only multivariate predictors of horizontal-oval shape. Low cardiac index was the only predictor of noncircular shape.More than half our patients with AS had noncircular LVOTs. Using the modified method reduces mischaracterizations of AS severity. Congenital AV morphology and low cardiac index predict horizontal-oval or noncircular shape. 目的 比较不同药敏试验方法检测替加环素对耐碳青霉烯肺炎克雷伯菌(CRKP)的敏感性。初步了解辽宁地区CRKP对替加环素的耐药情况,为临床合理用药提供依据。方法 回顾性收集辽宁省内多家大型三甲医院2011年1月-2016年12月分离的CRKP共269株,采用微量肉汤稀释法、KB纸片扩散法、Vitek-2仪器法及E试验法检测替加环素对CRKP的敏感性。结果 微量肉汤稀释法、E试验法、Vitek-2仪器法测定替加环素MIC50和MIC90分别为(0.5/1)、(0.25/0.5)和(0.5/4)μg/mL。按美国食品与药品监督管理局(FDA)和欧洲药敏试验委员会(EUCAST)判读标准,微量肉汤稀释法测得CRKP对替加环素的敏感率为97.4%/93.3%,中介率为2.2%/4.1%,耐药率为0.4%/2.6%。E试验法敏感率最高,为100%/98.9%,Vitek-2法耐药率最高,为8.6%/13.4%,纸片扩散法的中介率最高,为11.9%/46.5%。与微量肉汤稀释法比较,E试验法的基本一致率(EA)和分类一致率(CA)均≥90%,但存在重大误差(VME),分别为0.4%/1.9%;Vitek-2法EA仅为61.7%,CA为87.4%/72.1%,且大误差(ME)为6.3%/7.4%,无VME;纸片扩散法CA仅为85.9%/46.5%,小误差(mE)为1.9%/2.6%,ME为0.7%/5.6%。结论 辽宁地区绝大多数CRKP对替加环素仍保持较高的敏感性。对于CRKP、E-试验法、Vitek-2仪器法和纸片扩散法均不适合单独检测替加环素敏感性,可考虑联合检测,若结果不一致,均应参考微量肉汤稀释法。 相似文献

着地方式:指的是人体在跑步着地阶段足部接触地面的方式,一般分为3种方式:分别为足跟着地(fore foot strike),跟骨先接触地面;全足着地(mid foot strike),全脚掌着地,即足跟与前足同时接触地面;前足着地(rear foot
strike):前足部首先接触地面。
背景:成年人跑步着地方式一直是国内外学者研究的重点,而幼儿跑步的着地方式也是不容忽视的内容。
目的:运用生物力学方法探究幼儿在跑步过程中,不同着地方式下的运动学和动力学指标的差异,为幼儿正确的跑步着地方式提供科学依据。
方法:在北京市海淀区某公立幼儿园中随机抽取幼儿74名,按年龄分为3岁组、4岁组、5岁组,采用BTS红外动作捕捉系统、Kistler三维测力台和VIXTA录像解析系统同步采集幼儿跑步过程中不同着地方式下的运动学、动力学数据;运用Anybody
5.2仿真建模软件计算下肢肌肉力量指标。试验前向受试者父母详细解释并签署知情同意书,试验方案符合北京师范大学的相关伦理要求。
结果与结论:①3岁组全足着地的比例最高,足跟着地的比例最低,5岁组全足着地的比例最低,足跟着地的比例最高;前足着地者的蹬伸时间大于足跟着地(P < 0.01)和全足着地(P < 0.05);②着地时刻,踝屈曲角度足跟着地者大于前足着地(P < 0.01)和全足着地者(P < 0.05),全足着地者大于前足着地(P < 0.05);前足着地者髋内收-外展角度、最大髋内收-外展角、髋内-收外展的关节变化量及最大膝内收-外展角速度大于足跟着地(P < 0.01)和全足着地者(P < 0.05);前足着地者的踝屈伸最小值大于足跟着地者(P < 0.05),而最大髋内收-外展角速度小于足跟着地者(P < 0.05);③足跟着地和全足着地者的腓骨短肌、腓骨长肌、第三腓骨肌的肌力大于前足着地者(P < 0.05),前足着地者的股中间肌、股外侧肌下束、股外侧肌上束、股内侧肌下束、股内侧肌上束、股内侧肌中束肌力均大于足跟着地(P < 0.01)和全足着地者(P < 0.05);④结果提示:在3-6岁阶段,幼儿多采用足跟或全足着地模式进行奔跑,以满足自己在跑步过程的稳定性,随着年龄的增长,逐渐出现前足着地方式的跑步模式;前足着地能够动用更多髋关节和膝关节额状面的运动来维持人体运动中的稳定,足跟着地和全足着地能够动用更多的小腿前侧和后侧的肌力,而前足着地动用更多的大腿前侧肌力。
