ObjectiveTo explore the effect of a PEEK material‐based external fixator in the treatment of distal radius fractures with non‐transarticular external fixation.MethodsThere were 48 patients in this prospective comparative study. They were divided into two groups according to the materials used: the PEEK group and the titanium group. Wrist dorsiflexion, palmar flexion, pronation, supination, radial deviation, ulnar deviation, grip strength of the palm on the affected side, kneading force, Visual Analogue Scale/Score (VAS), Disabilities of the Arm, Shoulder, and Hand (DASH) score, operation time, frequency of fluoroscopy procedures, and X‐ray results were compared between the two groups. Functional recovery was evaluated at the last follow‐up according to the wrist joint evaluation criteria.ResultsThe baseline data were comparable between the two groups, and no significant differences were found in age, sex, fracture types (P > 0.05). There was no significant difference between the two groups in the results of DASH, grip strength, and recovery of pinch force and wrist function (dorsiflexion, clavicle, ulnar deviation, deviation, pronation, and supination) (P > 0.05). Normal limb function was achieved in the two groups of patients at an average of 6 weeks after surgery, and there was no significant difference in X‐ray examination radial height (10.60 ± 1.59 vs 11.00 ± 1.53, P = 0.687), radial inclination (1.11 ± 0.24 vs 1.12 ± 0.24, P = 0.798), volar tilt (10.33 ± 2.13 vs 10.00 ± 2.08, P = 0.660), ulnar variance (20.87 ± 3.00 vs 20.38 ± 3.04, P = 0.748), and step‐off persistence (1.73 ± 0.69 vs 1.68 ± 0.72, P = 0.425) between the two groups (P > 0.05). However, the operation time (54.80 ± 12.20 vs 85.23 ± 15.14, P = 0.033) and number of fluoroscopy procedures (36.93 ± 6.89 vs 64.77 ± 9.74, P = 0.000) in the PEEK group were significantly reduced compared with those in the titanium group.ConclusionCompared with the traditional titanium external fixator, the PEEK composite external fixator has advantages, such as a shorter operation time and fewer fluoroscopy procedures when used to treat different types of distal radius fracture.  相似文献

ObjectiveThis study aimed to investigate the effects of long-term use on the retention force and wear behavior of double crown systems.MethodsBased on a common double crown design sixty pairs of telescopic crowns were fabricated and divided into six groups, each consisting ten samples: “Gold standard” cast gold alloy primary and secondary crown (GG) and cast non-precious alloy (NN), computer-aided design (CAD)/computer-aided manufacturing (CAM)-milled zirconia primary crown and galvanoformed secondary crown (ZG), CAD/CAM-milled non-precious alloy primary and secondary crown (CC NN), CAD/CAM-milled zirconia primary crown and non-precious alloy secondary crown (CC ZN) and CAD/CAM-milled zirconia primary crown and polyetheretherketone (PEEK) secondary crown (CC ZP). In the constant presence of artificial saliva, all samples were subjected to 10,000 joining-separation cycles at a velocity of 120 mm/min. Wear was analyzed by reflected light microscopy and confocal microscopy before and after artificial aging.ResultsRetention force losses were observed in each group after long-term use, with significant losses in the groups ZG and CC ZP (pZG = 0.01, pCC ZP = 0.049). During artificial aging, no significant differences in pull-off force were recorded for groups GG, NN and CC ZN. Regarding wear, merely the Y-TZP primary crowns of the CC ZP group displayed no surface changes.SignificanceAll tested production methods and material combinations seem to be suitable for clinical practice. CAD/CAM technology allows similarly predictable results to be achieved as the gold standard. Confocal microscopy is recommended for surface examinations of double crowns.  相似文献

目的利用有限元分析法比较聚醚醚酮/羟基磷灰石/碳纤维复合材料(75PEEK/10HA/15CF)与钛合金的生物力学。方法建立C4~C6有限元模型,于C5~C6间植入75PEEK/10HA/15CF或钛合金人工椎间盘和椎间融合器,计算在前屈、后伸、侧弯和旋转时临近椎体、椎间盘的wonMises应力变化和C5~C6节段的活动度及植入物上的应力分布。结果在正常情况下,钛合金人工椎间盘置换模型在前屈时C5椎体、C4~5椎间盘平均won Mises应力改变率分别为75PEEK/10HA/15CF人工椎间盘模型的1.50倍和1.67倍;在侧弯时C6椎体的平均won Mises应力改变率为75PEEK/10HA/15CF人工椎间盘置换模型的1.33倍。融合器模型,在前屈时C5椎体、C4~5椎间盘应力改变率前者为后者的1.48倍和1.87倍;在侧弯时C6椎体应力改变率,前者为后者的1.67倍。钛合金植入物的最大应力为75PEEK/10HA/15CF的4.5倍,并出现应力集中。结论与钛合金相比,75PEEK/10HA/15CF能更好地将负荷传递,增加融合率,能有效地减少临近椎体的应力,减少植入物沉降的发生。  相似文献

PurposeThe purpose of study was to investigate the long-term effect of staining and/or cleansing solutions on the color stability of two non-metal removable partial denture materials.MethodsOne hundred disks (25×3 mm) of polyoxymethylene (POM) and polyetheretherketone (PEEK) were immersed in water, wine, coffee, cleanser and combo bath, simulating normal daily use. Color parameters in the CIELAB system was measured every 30 cycles up to 240 using a contact colorimeter and color differences estimated using ΔEab and ΔE00 formulas. Two-way repeated measures ANOVAs and regression analyses were performed at α = 0.05.ResultsRegression analysis indicated a strong R2 between color changes and number of cycles, for both materials. Tests of within-subjects effects for the ΔEab revealed significant differences among cycles and between the materials in the wine and coffee baths (p < 0.001). Significant materialXcycles interactions were also recorded with all staining baths. ΔE00 values were lower than ΔEab up to 63.6%. Tests within and between-subjects effects for the ΔE00 gave similar but not the same with ΔEab results.ConclusionsΔE00 found to correlate well with ΔEab. Long term exposure of both materials showed a progressive discoloration in all except control baths. POM discolored more than PEEK in coffee, and combo baths but not in cleanser. Discoloration was smaller in combo bath (where a cleanser was also used) indicating the effectiveness of a cleanser to prevent long term discoloration of both materials.  相似文献

PurposeAdjacent segment degeneration or fracture of the vertebral body was commonly reported in rigid fusion. Use of semirigid instruments such as PEEK rod system could be an alternative treatment. However, the biomechanical implications of using PEEK rod systems are not well understood. Purpose of this study was to compare a PEEK rod fixation system to traditional titanium rod fixation via a finite element analysis.MethodsA lumbar spine model from L2–L5 vertebral bodies was constructed. A fusion model, created by modifying the intact lumbar model, was used to simulate anterior interbody and posterolateral lumbar fusion. Loading was applied through flexion, extension, lateral bending, torsion.ResultsThe greatest increase in stress was estimated at the upper disc adjacent to the titanium rod with interbody fusion. The lower increase in stress on adjacent segments occurred with PEEK rod fixation without fusion and noninstrumented posterolateral lumbar fusion models. With the same fusion or nonfusion procedures, the stress on discs and facet joints of adjacent segments in the PEEK rod group decreased by 5–25% of that in the titanium rod group for all loading conditions.ConclusionIn comparison with rigid fixation, some potential advantages of using PEEK rod systems include a reduced stress on adjacent segment disc and facet joint, and the elastic ability of PEEK rod fixation allows for a greater range of motion, which may reduce the incidence of clinical complications seen with rigid fusion devices.  相似文献

Objective

The present retrospective study was conducted to compare the clinical and radiographic outcomes in patients undergoing anterior cervical discectomy with fusion (ACDF) using carbon fiber reinforced polymer (CFRP) cages, or allograft.

Methods

We retrospectively reviewed cases of ACDF using allograft in 20 patients, and CFRP in 19 who had sequential radiographs before and after surgery, and at 1 year.

Results

There were no apparent significant differences between the 2 groups in age (p = 0.057), gender (p = 0.635), or complications (p = 0.648). At 12 months, there were no cases of construct failure, and fusion appeared to have been achieved in patients of both groups. Lordosis was increased significantly in both groups after surgery (p < 0.001 in allograft and p = 0.025 in CFRP), and was maintained up until 1 year (p < 0.018 in allograft and p = 0.05 in CFRP) without a difference between groups (p = 0.721). Anterior interbody height was significantly increased (p < 0.001 in both groups at each time points) after surgery, without a significant difference between groups (p > 0.21). This increase in height was greatest in magnitude immediately after surgery, and declined with the passage of time. There was no detectable health-related quality of life difference between allograft and CFRP group after surgery (p > 0.05).

Conclusion

The present study demonstrates that CFRP cages appear to have comparable fusion rates, restoration of lordosis and disc space height, and complication rates to patients who undergo ACDF with allograft.  相似文献

As part of the experiments herein, the mechanical properties of specimens made of poly-ether-ether-ketone (PEEK) material using 3D printing technology were determined. Two populations of specimens were investigated, the first of which contained an amorphous structure, while the other held a crystal structure. The studies also investigated the influence of the print directionality on the mechanical properties obtained. Static tensile, three-point bending, and impact tests were carried out. The results for the effect of the structure type on the tensile properties showed that the modulus of elasticity was approximately 20% higher for the crystal than for the amorphous PEEK form. The Poisson’s ratios were similar, but the ratio was slightly higher for the amorphous samples than the crystalline ones. Furthermore, the studies included a chemical PEEK modification to increase the hydrophilicity. For this purpose, nitrite and hydroxyl groups were introduced into the chain by chemical reactions. The results demonstrate that the modified PEEK specimens had worse thermoplastic properties than the unmodified specimens.  相似文献

Research on Laser Powder Bed Fusion (L-PBF) of polymer powder feedstocks has raised over the last decade due to the increased utilization of the fabricated parts in aerospace, automotive, electronics, and healthcare applications. A total of 600 Science Citation Indexed articles were published on the topic of L-PBF of polymer powder feedstocks in the last decade, being cited more than 10,000 times leading to an h-index of 46. This study statistically evaluates the 100 most cited articles to extract reported material, process, and as-built part properties to analyze the research trends. PA12, PEEK, and TPU are the most employed polymer powder feedstocks, while size, flowability, and thermal behavior are the standardly reported material properties. Likewise, process properties such as laser power, scanning speed, hatch spacing, powder layer thickness, volumetric energy density, and areal energy density are extracted and evaluated. In addition, material and process properties of the as-built parts such as tensile test, flexural test, and volumetric porosity contents are analyzed. The incorporation of additives is found to be an effective route to enhance mechanical and functional properties. Carbon-based additives are typically employed in applications where mechanical properties are essential. Carbon fibers, Ca-phosphates, and SiO2 are the most reported additives in the evaluated SCI-expanded articles for L-PBF of polymer powder feedstocks. A comprehensive data matrix is extracted from the evaluated SCI-index publications, and a principal component analysis (PCA) is performed to explore correlations between reported material, process, and as-built parts.  相似文献

In spinal oncology traditional titanium implants could significantly impair evaluation of postoperative imaging because of artifacts, potentially affecting proper planning and execution of radiotherapy and adequate radiological follow-up to rule out progression of the disease. This is why carbon fiber reinforced (CFR)-PEEK implants have been developed for spinal fixation. The advantages of this system include fewer artifacts on imaging, potentially improving the execution and quality of radiotherapy, with also a reduced scattering effect to neighboring tissues.A comparative clinical and radiological study between new CFR-PEEK and standard titanium implants is described. Data recorded for each case included patient demographics, clinical, radiological and surgical data, intra- and postoperative complications, follow-up information. The goal of this study was to verify the safety and effectiveness of CFR-PEEK devices compared to standard titanium implants.A total number of 78 patients were reviewed. 36 patients underwent CFR-PEEK fixation, while titanium implants were used for 42 patients. Functional recovery was obtained in both groups and registered at last follow-up in terms of axial pain and neurological status. No significative differences were found between the two groups in terms of post-operative clinical complications and hardware-related complications.CFR-PEEK implants constitute a feasible and effective way to restore stability in metastatic spine tumors. This study found a non inferior favorable profile in terms of intraoperative and postoperative complications and functional recovery, compared to titanium. Further prospective studies are needed to clarify the potential oncological advantage of their radiolucency.  相似文献
