During the past 10–15 years, Regional Health Care Networks (RHCN) have been established in many regions throughout the world. RHCN build on well-known techniques, methodologies and appropriate standards. Most of the European Countries today have set up IT strategic plans that focus on the establishment of RHCN. The benefits of having access to all relevant information are tremendous and contribute to cost-effective and coherent health services. By the rapid spread and use of Internet, technology has made it possible to interconnect all kinds of applications. In 2000, the most experienced regions in Europe joined PICNIC, a European project to develop the Next Generation Regional Health Care Networks and to support their new ways of providing health and social care. The previous generation of Regional Health Care Networks supported the interconnection of applications by transfer of messages. Messaging is an effective means of integration for isolated high-specialised systems that only need to exchange data. This service will continue to be one of the most important services in the future health care networks. However, tighter coupling may be desirable in some instances to avoid replicating the same functionality in several applications. In other words, certain services can be common and used by a number of applications instead of building that service inside each application. These common services are called middleware services. In PICNIC (http://www.medcom.dk/picnic), a new middleware Collaboration IT service has been identified and developed. This service allows the end users to perform real-time clinical collaboration, with exchange of text, structured data, voice and images across the limits of a single region. A clinical collaboration is associated with the shared clinical context to provide a record of relevant clinical information and facilitates synchronous as well as asynchronous collaboration. This new IT service builds on the increasing popularity of instance messaging and presence systems that facilitate smooth transition between synchronous and asynchronous interaction. The new Collaboration IT service is expected to have a strong impact on the practice of health care in the next generation of Regional Health Care Networks.  相似文献

本文选择固定和新鲜尸体下肢标本57侧,采用局部解剖、动脉铸型、动脉造影和标本透明等方法,对膝关节动脉血液供应作了初步观察。动脉关节支起点变异和共干形式,以右侧居多。膝区各动脉均有特殊的形态和一定的分布区域,在每一局部几个血液供应来源中,总有一个主次关系和层次关系。膝区动脉按口径大小,可以分成3组。口径的变化除了可以评价局部血液供应丰富的程度外,也是衡量相邻动脉大小的指征。整个膝区动脉血液供应配布比较均匀对称。供应膝关节的各动脉起点的高度是恒定的,相互间保持一定关系。本文还就纯关节支和肌关节支的概念、动脉吻合、局部“乏血管区”、髌前动脉网与髌骨血液供应的关系等进行了简单讨论。  相似文献

The demand for home care in the Netherlands is growing and changing. A decreasing rate of institutionalization, combined with an increasing number of elderly people with special age-related, often complex health problems, has led to new groups of patients, to be cared for in their home settings. This requires a strong coordination of services between suppliers from the primary and secondary health care sectors. In order to promote coordinated care, Dutch health care providers are building inter-organizational networks. The development of such arrangements was the target of 12 demonstration projects, selected by the Ministry of Health for the National Home Care Programme. This article presents some of the results of an evaluation study of the Programme, with special attention given to the question of the extent to which such inter-organizational networks contribute to the balance between quality and cost.  相似文献

Cognitive science continues to make a compelling case for having a coherent, unique, and fundamental subject of inquiry: What is the nature of minds, where do they come from, and how do they work? Central to this inquiry is the notion of agents that have goals, one of which is their own persistence, who use dynamically constructed knowledge to act in the world to achieve those goals. An agentive perspective explains why a special class of systems have a cluster of co‐occurring capacities that enable them to exhibit adaptive behavior in a complex environment: perception, attention, memory, representation, planning, and communication. As an intellectual endeavor, cognitive science may not have achieved a hard core of uncontested assumptions that Lakatos (1978) identifies as emblematic of a successful research program, but there are alternative conceptions according to which cognitive science has been successful. First, challenges of the early, core tenet of “Mind as Computation” have helped put cognitive science on a stronger foundation—one that incorporates relations between minds and their environments. Second, even if a full cross‐disciplinary theoretic consensus is elusive, cognitive science can inspire distant, deep, and transformative connections between pairs of fields. To be intellectually vital, cognitive science need not resemble a traditional discipline with its associated insularity and unchallenged assumptions. Instead, there is strength and resilience in the diverse perspectives and methods that cognitive science assembles together. This interdisciplinary enterprise is fragile and perhaps inherently unstable, as the looming absorption of cognitive science into psychology shows. Still, for many researchers, the excitement and benefits of triangulating on the nature of minds by integrating diverse cases cannot be secured by a stable discipline with an uncontested core of assumptions.  相似文献

ABSTRACT

The advancement of DNA technology comes with the increased sensitivity of amplification systems, where DNA traces are routinely detected without a known biological source. These systems also have increased discriminating capacity, providing larger likelihood ratios (LRs) when a corresponding DNA profile is observed. Questions in court are shifting from identity to transfer mechanism, where the presence of an individual’s DNA is conceded by both parties, but the activities that led to its deposition is in dispute. One way of handling propositions developed at the activity level is with the use of graphical structures known as Bayesian Networks (BNs). The following is an evaluation of a case, given activity level propositions, through the application of BNs. Alternative case findings will be explored for the given scenario to show the potential value of the DNA evidence for different outcomes within the broader case context.  相似文献

BackgroundRevision total knee arthroplasty (rTKA) can be complex, with greater costs to the treating hospital than primary TKA. A rTKA regional network has been proposed in England. The aim of this work was to accurately quantify current costs and reimbursement for the rTKA service and to assess whether costs are proportional to case complexity at a tertiary referral centre within the National Health Service (NHS).MethodsA review of all rTKA performed at our institution over two consecutive financial years (2017–2019) was performed. Cases were classified according to the Revision Knee Complexity Classification (RKCC) and by mode of failure; “infected” and “non-infected”. Financial data was acquired through Patient-Level Information and Costing System (PLICS). The primary outcome was the financial difference between tariff and cost per episode. Comparisons between groups were analysed using analysis of variance and two-tailed unpaired t-test as appropriate.Results159 patients underwent 188 rTKA procedures. Length of stay and cost significantly increased between complexity groups (p < 0.0001) and for infected revisions (p < 0.0001). All groups sustained a mean deficit but this significantly increased with revision complexity (from £1,903 to £5,269 per case) and for infected revisions. The total deficit to the Trust for the two-year rTKA service was £667,091.ConclusionsThe current level of NHS reimbursement are inadequate for centres that offer rTKA and should be more closely aligned to case complexity. An increase in the most complex rTKA at major revision centres will undoubtedly place an even greater strain on the finances of these units.  相似文献

This article illustrates how urban inner-city trucking milieux may influence STI/BBI/HIV acquisition and transmission risks for U.S. long-haul truckers, as well as their social and risk relationships. Using mixed methods, we collected ethnoepidemiological and biological data from long-haul truck drivers and their risk contacts in inner-city trucking milieux in Atlanta, Georgia, United States. Key findings indicate that within the risk-endemic environment of distressed inner-city areas, diverse trucking risk milieux can amplify STI/BBI/HIV risk for multiplex networks of truckers. Inner-city neighborhood location, short geographic distance among risk contacts, and trucker concurrency can potentially exacerbate transmission via bridging higher-risk individuals with lower-risk populations at disparate geographic and epidemiological locations.  相似文献

The primary progressive aphasias (PPA) are a heterogeneous group of language-led neurodegenerative diseases resulting from large-scale brain network degeneration. White matter (WM) pathways bind networks together, and might therefore hold information about PPA pathogenesis. Here we used diffusion tensor imaging and tract-based spatial statistics to compare WM tract changes between PPA syndromes and with respect to Alzheimer's disease and healthy controls in 33 patients with PPA (13 nonfluent/agrammatic PPA); 10 logopenic variant PPA; and 10 semantic variant PPA. Nonfluent/agrammatic PPA was associated with predominantly left-sided and anterior tract alterations including uncinate fasciculus (UF) and subcortical projections; semantic variant PPA with bilateral alterations in inferior longitudinal fasciculus and UF; and logopenic variant PPA with bilateral but predominantly left-sided alterations in inferior longitudinal fasciculus, UF, superior longitudinal fasciculus, and subcortical projections. Tract alterations were more extensive than gray matter alterations, and the extent of alteration across tracts and PPA syndromes varied between diffusivity metrics. These WM signatures of PPA syndromes illustrate the selective vulnerability of brain language networks in these diseases and might have some pathologic specificity.  相似文献
