PurposeAccording to the social determinants of health framework, income inequality is a potential risk factor for adverse mental health. However, few studies have explored the mechanisms suspected to mediate this relationship. The current study addresses this gap through a mediation analysis to determine if social support and community engagement act as mediators linking neighbourhood income inequality to maternal anxiety and depressive symptoms within a cohort of new mothers living in the City of Calgary, Canada.MethodsData collected at three years postpartum from mothers belonging to the All Our Families (AOF) cohort were used in the current study. Maternal data were collected between 2012 and 2015 and linked to neighbourhood socioeconomic data from the 2006 Canadian Census. Income inequality was measured using Gini coefficients derived from 2006 after-tax census data. Generalized structural equation models were used to quantify the associations between income inequality and mental health symptoms, and to assess the potential direct and indirect mediating effects of maternal social support and community engagement.ResultsIncome inequality was not significantly associated with higher depressive symptoms (β = 0.32, 95%CI = −0.067, 0.70), anxiety symptoms (β = 0.11, 95%CI = −0.39, 0.60), or lower social support. Income inequality was not associated with community engagement. For the depression models, higher social support was significantly associated with lower depressive symptoms (β = −0.13, 95%CI = −0.15, −0.097), while community engagement was not significantly associated with depressive symptoms (β = 0.059, 95%CI = −0.15, 0.27). Similarly, for the anxiety models, lower anxiety symptoms were significantly associated with higher levels of social support (β = −0.17, 95%CI = −0.20, −0.13) but not with higher levels of community engagement (β = 0.14, 95%CI = −0.14, 0.41).ConclusionThe current study did not find clear evidence for social support or community engagement mediating the relationship between neighbourhood income inequality and maternal mental health. Future investigations should employ a broader longitudinal approach to capture changes in income inequality, potential mediators, and mental health symptomatology over time. 相似文献

Summary The purpose of this study was to compare two ways of estimating both fatigue and fitness indicators from systems model of the effects of training on performance. The model was applied to data concerning the training of a hammer thrower. The variations in performance were mathematically related to the successive amounts of training. The model equation was composed of negative (NF) and positive (PF) functions. The NF and PF were associated with the fatigue and fitness estimated in previous studies. Using another method, fatigue and fitness indicators were estimated from a combination of NF and PF. The influence of training on performance was negatively associated with fatigue (NI), and positively to fitness (PI). The changes in performance were well described by the model in the present study (r = 0.96,N = 19,P<0.001). Significant correlations were observed between NF and NI (r = 0.93,P < 0.001) on the one hand and between PF and PI (r = 0.90,P < 0.001) on the other. The absolute values and the time variations of PI and NI were closer to the change in performance than NF and PF. The NF and PF were accounted for mainly by the accumulation of amounts of training. On the other hand, NI and PI were accounted for rather by the impact of these amounts of training on performance. 相似文献

Work undertaken to investigate the effect on EEG responses of ocular artefact removal by proportional electro-oculogram subtraction
using the least-squares method is reported. The ocular artefact model and the least-squares approach to estimation of the
model parameters and hence of the EEG waveform, including the response, are described. Results are presented to show that
the response shapes become modified if ocular artefact removal is implemented recursively. This happens because the response
is incorrectly treated as part of the random background EEG. The solution is to incorporate a model of the response within
the ocular artefact removal algorithm. The results of tests on simulated data confirm these conclusions. The results of the
incorporation of response models in removing ocular artefacts from CNV recordings are presented. Even with modelling recursive
ocular artefact removal is found to modify the shape of the CNV, and so reliable results can only be obtained if the removal
is implemented nonrecursively. Evidence is given that it is advisable to remove DC levels from the recorded data. 相似文献

Summary Since the work of Pauwels, the forces exerted on the coxofemoral joint during walking have been studied either in different spatial planes (frontal, sagittal and horizontal) or by three-dimensional spatial analysis. Starting from the findings of our own studies, our aim was to compare the two methods of analysis (two-dimensional and three-dimensional) in order to provide a better understanding of the benefits and limitations of each method. In pursuit of this aim, we studied the pressure forces exerted on the coxofemoral joint, using a geometric plane technique following a method similar to that of Pauwels [20], and with a three-dimensional modelling technique using the finite element method. The material, taken from the published literature, was the same in both our studies. The results are expressed in terms of the size and orientation of the pressure force exerted on the coxofemoral joint during the monopodal weightbearing phase of walking. A comparison of these two methods of analysis clearly demonstrates the simplicity of two-dimensional analysis (which must incorporate as a minimum the frontal plane and the sagittal plane) and the richness of the three-dimensional analysis. The latter method, by appropriate manipulation of the information obtained, provides a starting point for computer simulations performed with the aim of testing a biomechanical or therapeutic hypothesis.
Biomécanique de la hanche : les sollicitations à la marche
Résumé Depuis Pauwels, les sollicitations exercées sur l'articulation coxofémorale au cours de la marche ont été étudiées soit dans les différents plans de l'espace (frontal, sagittal et horizontal), soit par des analyses spatiales tridimensionnelles. A partir d'études personnelles, nous avons voulu comparer ces deux méthodes d'analyse (bidimensionnelle et tridimensionnelle) afin de dégager au mieux les apports et les limites de chacune. Pour cela, nous avons étudié les sollicitations en pression s'exerçant sur l'articulation coxofémorale, d'une part selon une étude géométrique plane dont la méthodologie s'apparente à celle de Pauwels [20], d'autre part selon une modélisation tridimensionnelle par la méthode des éléments finis. Le matériel, emprunté à la littérature, est commun à nos deux études. Les résultats intéressent l'intensité et l'orientation de la force en pression exercée sur l'articulation coxofémorale pendant la phase d'appui monopodal de la marche. La comparaison des deux types d'analyse met en valeur la simplicité de l'analyse bidimensionnelle (qui doit combiner le plan frontal et le plan sagittal au minimum) et la richesse de l'analyse tridimensionnelle qui, par la manipulation des informations obtenues, ouvre sur les simulations informatiques, en vue de tester une hypothèse biomécanique ou thérapeutique.相似文献

The paper presents a mathematical analysis of the contributions of flow and mass transport to a single reactive event at a
blood vessel wall. The intent is to prepare the ground for a comprehensive study of the intertwining of these contributions
with the reaction network of the coagulation cascade. We show that in all vessels with local mural activity, or in “large”
vessels (d>0.1 mm) with global reactivity, events at the tubular wall can be rigorously described by algebraic equations under
steady conditions, or by ordinary differential forms (ODEs) during transient conditions. this opens up important ways for
analyzing the combined roles of flow, transport, and coagulation reactions in thrombosis, a task hitherto considered to be
completely intractable.
We report extensively on the dependence of transport coefficient kL and mural coagulant concentration Cw on flow, vessel geometry, and reaction kinetics. It is shown that for protein transport, kL varies only weakly with shear rate
in large vessels, and not at all in the smaller tubes (d<10−2 mm). For a typical protein, kL∼10−3 cm s−1 within a factor of 3 in most geometries, irrespective of the mural reaction kinetics. Significant reductions in kL (1/10–1/1,000) leading to high-coagulant accumulation are seen mainly in stagnant zones vicinal to abrupt expansions and
in small elliptical tubules. This is in accord with known physical observations. More unexpected are the dramatic increases
in accumulation which can come about through the intervention of an autocatalytic reaction step, with Cw rising sharply toward infinity as the ratio of reaction to transport coefficient approaches unity. Such self-catalyzed reactions
have the ability to act as powerful amplifiers of an otherwise modest influence of flow and transport on coagulant concentration.
The paper considers as well the effect on mass transport of transient conditions occasioned by coagulation initiation or pulsatile
flow. During initiation, instantaneous flux varies with diffusivity and bulk concentration, favouring the early adsorption/consumption
of proteins with the highest abundance and mobility. This is akin to the ‘Vroman effect’ seen in narrow, stagnant spaces.
The effect of flow pulsatility on kL has the potential, after prolonged cycling, of bringing about segregation or accumulation of proteins, with consequences

Modelling asthma epidemics on the relationship between air pollution and asthma emergency visits in Barcelona, Spain 总被引:1,自引:0,他引:1
We have investigated different methods of controlling for asthma epidemics in the time series regression of the relationship between air pollution and asthma emergency visits in Barcelona, Spain. The relationship between air pollution and asthma emergency room visits was modelled using autoregressive Poisson models. We examined the effect of using no control by epidemics, and modelling asthma epidemics with a single dummy variable, six dummy variables, and a dummy variable for each epidemic day. Air pollution coefficients increased when controlling asthma epidemics with six dummy variables instead of a single variable. They further increased when autocorrelation was allowed for. Standard errors were relatively unaffected when either the epidemics or the autocorrelation were included in the model. Black smoke, nitrogen dioxide and ozone were statistically significant associated to asthma emergency visits after using six dummy variables to control for asthma epidemics. We have shown that different models, including different confounding variables, give markedly different estimates of the effect of a pollutant on health. Care is needed in the interpretation of such models, and careful reporting so it is clear how the confounding variables have been modelled. 相似文献

A two-component dynamic model was used to describe the ventilatory response to sustained hypoxia in humans. One component
(Xs) represents the stimulating effects of hypoxia and the other component (Xd), the hypoxic ventilatory decline. The total ventilatory response to hypoxia is represented by the sum of the two components.
A nonlinearity is included to account for the nonlinear steady-state ventilatory response to hypoxia. A sensitivity analysis
of the model indicates that, with a step change in
as the input, all the parameters can be estimated from the data except for the nonlinearity. The relative sensitivity of
the parameters from the model analysis was confirmed in an experimental study. However, comparing steps into hypoxia versus
steps out of hypoxia we found a decrease in the gains of both components. The most likely explanation for the decrease in
the gains is that the combination of Xs and Xd is not entirely additive. Other models may be required to completely describe the ventilatory response to inputs more complex

ObjectiveThe aim of the present work was to use a semi-mechanistic pharmacokinetic–pharmacodynamic (PK/PD) model developed from in vitro time–kill measurements with P. aeruginosa to compare different pharmacodynamic indices derived from simulated human avibactam exposures, with respect to their degree of correlation with the modelled bacterial responses.MethodsA mathematical model of the effect of ceftazidime–avibactam on the growth dynamics of P. aeruginosa was used to simulate bacterial responses to modelled human exposures from fractionated avibactam dosing regimens with a fixed ceftazidime dosing regimen (2 or 8 g q8h as a 2-h infusion). The relatedness of the 24-h change in bacterial density and avibactam exposure parameters was evaluated to determine exposure parameter that closely correlated with bacterial growth/killing responses.ResultsFrequent dosing was associated with higher efficacy, resulting in a reduction of avibactam daily dose. The best-fit PD index of avibactam determined from the simulation was fT > CT of 1 mg/L avibactam and q8h was the longest dosing interval able to achieve 2-log kill: 41–87% (3.3 h to 7.0 h out of 8-h interval, respectively). The avibactam exposure magnitude required to achieve a 2-log kill in the simulations was dependent on the susceptibility of the bacterial isolate to ceftazidime.ConclusionsAvibactam activity in combination with ceftazidime against multidrug resistant P. aeruginosa correlated with fT > CT. Setting a threshold avibactam concentration to 1 mg/L, superimposed over a simulated human-like exposure of ceftazidime, achieved at least 2-log kill for the clinical dose of 500 mg q8h avibactam as a 2-h infusion, depending on the minimum inhibitory concentration of ceftazidime alone. 相似文献
