The use of vaginal meshes has been an advance in the surgical management of women with pelvic organ prolapse. We reviewed the literature to synthesize the evidence regarding the infectious complications related to this new type of foreign body. We searched PubMed, current contents, and references of initially identified relevant articles and extracted data regarding the incidence, clinical manifestation, and management of vaginal mesh-related infections. The incidence of mesh-related infections and erosion ranged from 0 to 8%, and 0 to 33%, respectively, in the published studies. Various factors influence the development of vaginal mesh-related infectious complications such as the kind of biomedical material (e.g. filament structure, pore size) of the mesh, the type of procedure, the preventive measures taken, and the age and underlying comorbidity of the treated women. Non-specific pelvic pain, persistent vaginal discharge or bleeding, dyspareunia, and urinary or faecal incontinence are the most common manifestation of vaginal mesh-related infection. Clinical examination may reveal induration of the vaginal incision, vaginal granulation tissue, draining sinus tracts, and prosthesis erosion or rejection. Various pathogens have been implicated, including Gram-positive and Gram-negative aerobic and anaerobic bacteria. The management of mesh-related infections in women who underwent pelvic organ reconstruction is combined surgical and medical treatment. Although the use of vaginal meshes has become a new effective method of pelvic organ prolapse surgery clinicians should be aware of the various post-operative complications, including mesh-related infections.  相似文献

目的探讨类风湿性关节炎(RA)骨侵蚀征象与抗环瓜氨酸肽抗体(抗CCP抗体)和类风湿因子(IgM-RF)的相关性。方法回顾性分析106例类风湿性关节炎患者资料,分别对其关节骨侵蚀,血清抗CCP抗体及IgM-RF检测结果进行统计、分析。结果106例RA患者的X光片按修正的sharp评分法进行评分分组,有骨侵蚀组60例,其中抗CCP抗体阳性48例,IgM-RF阳性49例;无骨侵蚀组46例,其中抗CCP抗体阳性22例,IgM-RF阳性30例。结论RA骨侵蚀与抗CCP抗体阳性具有相关性(p<0.01),而与IgM-RF阳性无相关性(p>0.05)。  相似文献

目的:为普查女职工的妇科疾病及预防和早期发现宫颈癌进行本项工作,方法:通过普查的方式对成都木综厂的全体已婚女职工进行 妇科检查、阴道清洁度、白带查滴虫、霉菌、宫颈细胞学检查,结果:阴道滴虫的患病率平均为2%,阴道霉菌的平均感染率为10.65,宫颈糜烂的平均患病率为32.75,霉菌性阴道炎及宫颈糜烂患病率随年龄的增加而逐渐下降。宫颈细胞学检查巴氏Ⅲ以上者患者率为0.4%(4例)。结论:宫颈糜烂、霉菌性阴道炎及滴虫性阴道炎是妇女最常见的3种炎症,前2种炎症的患病率随年龄的增加而逐渐下降。  相似文献

Abstract

The objective of this study was to compare standard hand radiographs with in-office 0.2?T magnetic resonance imaging (MRI) in monitoring response to therapy in patients with rheumatoid arthritis (RA) who were receiving infliximab, to evaluate the frequency and location of erosions, and to determine if there were differences in outcome based on disease duration at baseline. Patients who satisfied the American College of Rheumatology criteria for RA and were receiving infliximab therapy were evaluated with a baseline and 1-year follow-up MRI. Magnetic resonance images were interpreted by two blinded, board-certified radiologists. Bone erosions were identified as well-defined defects extending through the cortical margin. The mean age of the 48 patients was 58.5 years. The median infliximab dosage was 4?mg/kg. Baseline data showed that 41 patients had abnormal MRIs. The mean time between the baseline and follow-up MRI examinations was 10.5 months. Follow-up MRI revealed regression in 11 patients. Thirty-one patients had both MRIs and radiographs. Magnetic resonance imaging was approximately twice as sensitive as radiography in detecting erosions at baseline. In-office MRI was useful in monitoring disease response after the initiation of infliximab treatment. Magnetic resonance imaging is potentially a very valuable diagnostic tool and prognostic indicator for use in patients with RA.  相似文献

ObjectiveDental erosion is a common oral condition caused by chronic exposure to acids from intrinsic/extrinsic sources. Repeated acid exposure can lead to the irreversible loss of dental hard tissues (enamel, dentine, cementum). Dentine can become exposed to acid following severe enamel erosion, crown fracture, or gingival recession. Causing hypersensitivity, poor aesthetics, and potential pulp involvement. Improving treatments that can restore the structural integrity and aesthetics are therefore highly desirable. Such developments require a good understanding of how acid demineralisation progresses where relatively little is known in terms of intertubular dentine (ITD) and peritubular dentine (PTD) microstructure. To obtain further insight, this study proposes a new in vitro method for performing demineralisation studies of dentine.MethodsAdvanced high-speed synchrotron X-ray microtomography (SXM), with high spatial (0.325 μm) and temporal (15 min) resolution, was used to conduct the first in vitro, time-resolved 3D (4D) study of the microstructural changes in the ITD and PTD phases of human dentine samples (∼0.8 × 0.8 × 5 mm) during 6 h of continuous acid exposure.ResultsDifferent demineralisation rates of ITD (1.79 μm/min) and PTD (1.94 μm/min) and their progressive width-depth profiles were quantified, which provide insight for understanding the mechanisms of dentine demineralisation.SignificanceInsights obtained from morphological characterisations and the demineralisation process of ITD and PTD during acid demineralisation would help understand the demineralisation process and potentially aid in developing new therapeutic dentine treatments. This method enables continuous examination of relatively large volumes of dentine during demineralisation and also demonstrates the potential for studying the remineralisation process of proposed therapeutic dentine treatments.  相似文献

ObjectivesThe aim of this in vitro study was to evaluate the effect of air-abrasion surface pre-treatment with bioactive glass (BAG) 45S5 on enamel surface loss after erosion/abrasion challenge.MethodsTwenty-four sound bovine incisors were used for this study. Four experimental groups (n = 12) were assigned as follows: Group 1 was the negative control group, Group 2 specimens were treated with a SnF2 gel (positive control group), Group 3 specimens were air-abraded with BAG 45S5 (ProSylc) and Group 4 received both treatments. The specimens were submitted to erosion/abrasion challenge using a common soft drink. Enamel surface loss was evaluated using an optical profilometer. Additionally, surface roughness (VSI method) and surface microhardness (Vickers method) changes were evaluated, as well as SEM and EDS analyses were performed on enamel surface. The data were statistically analyzed using one-way ANOVA and Tukey’s post-hoc test at a level of significance a = 0.05.ResultsSurface pre-treatment with BAG 45S5 reduced surface loss in comparison with negative control group (p < 0.001), which exhibited the highest surface loss of the experimental groups (p < 0.05). The positive control group (SnF2 treatment) and Group 4 specimens, which received both air-abrasion pre-treatment and SnF2 treatment, presented the lowest surface loss (p < 0.05), but did not show significant differences to each other (p = 0.65).SignificanceAir-abrasion pre-treatment with BAG 45S5 may be beneficial as an in-office preventive method for the limitation of enamel erosive wear induced by excessive consumption of soft drinks. The clinical significance of the results regarding this preventive method should be confirmed by clinical studies.  相似文献

Objectives

Most in vitro studies investigate the erosive process using relatively simple roughness parameters such as roughness average (Ra). In isolation, Ra may misrepresent the surface features. Further, few studies report baseline surface characteristics after sample preparation. This study aimed to test the hypothesis that measuring the bearing area parameters in addition to Ra may be useful when qualifying the surface of enamel at baseline and after an erosive challenge. The null hypothesis for this study was that the bearing area parameters provide no more useful information than Ra alone, when qualifying the surface of enamel at baseline and after an erosive challenge.

Methods

Enamel slabs (n = 20) were prepared from human (n = 2) and bovine (n = 4) incisor teeth and polished with 0.05 μm paste. Roughness average (Ra) and bearing parameters (MR1, MR2, Rpk, Rk, Rvk) were used to record baseline characteristics. Specimens were subjected to erosion with 1% citric acid solution for 1 min. Profilometric characteristics were recorded post-erosion, along with the maximum height changes within the profile. T-tests were carried out in order to compare baseline surface characteristics between tissue types. Post-erosion, analysis of variance (ANOVA) was used to test the effects of tissue type (bovine or human).

Results

There was no significant difference in Ra between human and bovine incisor enamel at baseline (human 0.11 μm, bovine 0.12 μm P > 0.05), and no significant difference was observed post-erosion (human 0.23 μm, bovine 0.20 μm P > 0.05). There were significant differences in bearing parameters at baseline and post-erosion that were not identified by the Ra measurement alone.

Conclusions

The results suggest that if Ra alone is measured, important differences in surface characteristics may be missed. The null hypothesis is rejected, and the recommendation is made that bearing parameters are included within profile measurements in order to further triangulate the results of surface analysis studies.

Clinical relevance

In isolation, Ra may misrepresent the surface features of a profile. These results have shown that the bearing parameters are an important and informative set of measurements. The recommendation is made that bearing parameters are included within profile measurements at baseline and post-erosion, in order to further triangulate the results of surface analysis studies.  相似文献

Objectives

Sn2+ has promising erosion-inhibiting properties in solutions, but little is known about respective effects in toothpastes. In addition, biopolymers might have protecting potential. Aim of this study was to investigate the effects of Sn2+ in toothpastes and of a biopolymer (chitosan) added to a Sn2+ formulation on erosion/abrasion.

Methods

Enamel samples were subjected to cyclic erosion procedures (10 days; 0.50% citric acid, pH 2.5; 6× 2 min/day), and brushing (2× 15 s/day, load 200 g) during immersion in slurries (2 min). The toothpastes were NaF formulations (NaF/1, NaF/2, NaF/3) and Sn2+ formulations (NaF/SnCl2, AmF/SnF2, AmF/NaF/SnCl2) and AmF/NaF/SnCl2 + 0.5% chitosan.Declared concentrations of active ingredients in toothpastes were 1400–1450 μg/g F and 3280–3500 μg/g Sn2+. Negative controls were erosion only and placebo, positive control was a SnF2 gel. Tissue loss was quantified profilometrically, Sn on enamel surfaces was measured by energy dispersive X-ray spectroscopy.

Results

Loss values (μm) for erosion only and placebo were 14.4 ± 4.5 and 20.2 ± 3.8, respectively, and 4.6 ± 1.9 for the positive control (p ≤ 0.001 each compared to erosion only). The other loss values were: NaF/1 16.5 ± 3.0, NaF/2 14.0 ± 2.7, NaF/3 12.6 ± 3.9, NaF/SnCl2 14.7 ± 5.1, AmF/SnF2 13.5 ± 4.8, AmF/NaF/SnCl2 12.4 ± 4.2, AmF/NaF/SnCl2 + chitosan 6.6 ± 3.5 (except NaF/1 all p ≤ 0.01 compared to placebo). AmF/NaF/SnCl2/chitosan was more effective than all other toothpastes (p ≤ 0.01 each). Sn on the enamel surface ranged between 1.3 ± 0.3 and 2.8 ± 0.04 wt.% with no obvious relationship with efficacy.

Conclusions

The NaF and Sn2+ toothpastes without chitosan exhibited similar anti-erosion and abrasion-prevention effects. The experimental Sn2+ formulation with chitosan revealed promising results similar to those of the positive control.

Clinical significance

NaF toothpastes offer a degree of protection against erosion/abrasion, which is likely sufficient for most subjects with average acid exposures. For patients with initial erosive lesions, however, more effective toothpaste is desirable. The combination of Sn2+ and a biopolymer appears promising in this context.  相似文献

Objectives

To investigate the dentine occlusion and acid resistance of dentifrices developed to treat dentine hypersensitivity.

Methods

This was a single centre, single blind, randomised, split mouth, four treatments, two period crossover, in situ study in healthy subjects. Subjects wore buccal intra-oral appliances each fitted with four dentine samples over four consecutive days with one study product applied per appliance; 8% strontium acetate in silica base, 1040 ppm sodium fluoride (Sensodyne® Rapid Relief), 8% arginine, calcium carbonate, 1450 ppm sodium monofluorophosphate (Colgate Sensitive Pro-Relief®), 1450 ppm sodium fluoride (control paste) and water. On days 3 and 4, two agitated grapefruit juice challenges (ex vivo) occurred for 1 min. At the end of each treatment day 1 dentine sample was removed from each appliance for scanning electron microscopy (SEM). The extent of tubule occlusion was measured using an examiner-based visual scoring index (three trained examiners).

Results

In total, 28 subjects ((12 males and 16 females with a mean age of 34.7 years (SD 8.41 years)) completed the study. On day 2, both test dentifrices demonstrated significantly better dentine tubule occlusion than water (p < 0.0001) and control paste (8% strontium p = 0.0003 and 8% arginine p = 0.0019). After 3 and 4 days of twice daily brushing with acid challenges on days 3 and 4 the strontium-based dentifrice demonstrated significantly better dentine occlusion than all other treatments (p < 0.0001).

Conclusions

Strontium acetate and arginine-based dentifrice result in statistically significant dentine tubular occlusion compared to controls, but the arginine-based dentifrice is more susceptible to acid challenge.

Clinical significance

Erosive beverages are an important aetiology in DH by exposing dentine tubules. Their consumption has increased significantly over the past decade in the UK. This 4-day in situ study investigated the properties of commercially available dentifrices designed to occlude dentine tubules and their resistance to an agitated acid challenge.  相似文献

Objective To establish the incidence of central venous catheter erosion in a patient cohort receiving total parenteral nutrition and to examine risk factors and complications of vascular erosion. Design and setting Review of prospectively collected intravenous nutrition service audit records in a tertiary university hospital. Results Records of 1,499 patients (2,992 catheters) were studied over the 14 year period 1991–2005. Fisher's exact test was used to determine statistical significance. Five erosions occurred, representing an incidence of 0.17% per catheter or 0.28 per 1,000 catheter days. One of the five patients died from ensuing complications. Mean time to onset of symptoms was 3.6 days following catheter insertion. Symptoms/signs included dyspnoea (n = 5), chest pain (n = 2) and pleural effusion (n = 5). Diagnosis was delayed by a mean of 1.6 days. Three erosions occurred in left subclavian catheters (n = 583); two in left internal jugular catheters (n = 453). None occurred in right-sided catheters (n = 1956). The relative risk of erosion occurring in left-sided catheters compared to right was 2.9 (95% CI 2.76–3.00; p = 0.009). There was no statistically significantly greater risk of vascular erosion in subclavian than internal jugular catheters (relative risk 0.9; p = 1.0). Older age was a statistically significant risk factor (p = 0.009); female sex was not (p = 0.18). Conclusion In patients receiving total parenteral nutrition via central venous catheters, erosion has an incidence per catheter of 0.17% and is more likely to occur in left-sided catheters and elderly patients.  相似文献
