Effect of Electroacupuncture on Spermatogenesis in Rats with Oligozoospermia of Insufficiency of Shen (Kidney) Essence Syndrome

Objective: To assess the effect of electroacupuncture(EA) on expression of cytoskeletal proteins from Sertoli cells(SCs) and spermatogenesis in rats with oligozoospermia of insufficiency of Shen(Kidney)essence syndrome(OIKES).Methods: Twenty healthy male Sprague-Dawley rats were randomly assigned to four groups using a random number table: control,tripterygium glycosides(TG) treatment,sham and EA groups(n=5 in each group).A rat model of OIKES was established by oral gavage with TG.The EA group was treated with TG and received EA at Shenshu(BL 23) and Zusanli(ST 36) acupoints for 20 min,once daily for 30 days,while the sham group received EA at identical acupoints with skin penetration without stimulation.After 30 days,the ?nal body weight and coef?cients for the testis and epididymis were calculated and sperm parameters were measured.Immunohistochemical analyses were performed to detect expression of vimentin and α-tubulin in SCs and proliferating cell nuclear antigen(PCNA) immunoreactivity in germ cells.Apoptosis in germ cells was quanti?ed by the transferase biotin-dUTP nick end labeling assay.Results: Compared with the control group,the final body weight and testis/epididymis coefficients of rats in the TG-treated group were not significantly different,but the sperm count and motility were lower(P0.05).Expressions of vimentin and α-tubulin were also signi?cantly weaker(P0.01).The PCNA immunoreactivity of germ cells was decreased(P=0.059),whereas the apoptotic index of germ cells was increased signi?cantly(P0.01).In contrast,EA at BL 23 and ST 36 acupoints signi?cantly improved the ?nal body weight as well as the sperm count,concentration and motility(P0.01 or P0.05).EA increased expression of vimentin and α-tubulin in SCs markedly,and signi?cantly enhanced PCNA immunoreactivity with decreased apoptosis in germ cells(P0.01 or P0.05).Conclusions: EA at BL 23 and ST 36 acupoints has protective effects on spermatogenesis in rats with OIKES.This effect seems to be achieved by attenuating TG-induced disruption of cytoskeletal protein in SCs.

沙苑子为我国常用中药,药用历史悠久。通过查阅本草文献,对沙苑子名称、基原、功效、产地及伪品5个方面进行考证,得知其名称演变大致经过"白蒺藜""沙苑蒺藜""沙苑子"3个阶段;历代本草记载沙苑子基原为扁茎黄芪的干燥成熟种子,古今应用品种基本一致;沙苑子补肾固精功效的记载自宋《本草衍义》起逐渐清晰明确;沙苑子道地产区自宋时起为陕西沙苑,明清时期产地逐渐转到陕西潼关、山西等地,建国后以陕西省为主向北扩散,以陕西潼关为佳;沙苑子建国前有马薸子、色微绿伪品及紫云英子等混伪品,建国后有紫云英、猪屎豆、华黄芪和直立黄芪的种子等混伪品。为沙苑子正本清源,并为其进一步深入开发提供参考。     

培养检验医师队伍的对策探讨

随着现代检验医学的发展,传统医学检验正在向现代化检验医学的转变,急需加强检验与临床的沟通,医师型检验人才应运而生。目前,由于相关的管理体制,人文教育,工作职责不清晰等原因,检验医师队伍发展缓慢,仍存在很多值得改善和探索的地方。因此,我们应督促检验医师队伍,要具备较高的人文精神,掌握扎实的专业知识,参与完善的培训体制,明确工作职责,提高检验医师学习临床知识,掌握和运用医学检验知识,同时加强对临床与检验医学综合运用的能力,向临床医师提供医学检验知识,更好的与临床医生沟通解决临床问题,加强对检验医师的宣传与认识,共同促进检验医师队伍的规范化发展。     

阿戈美拉汀联合乙哌立松治疗青年慢性紧张性头痛的疗效分析

目的 探讨阿戈美拉汀联合乙哌立松治疗青年慢性紧张性头痛(Chronictention-typeheadache,CTTH)的疗效及安全性.方法 80例青年慢性紧张性头痛患者依据是否倒班随机分为对照组和治疗组,对照组口服乙哌立松治疗,治疗组在对照组基础上联合阿戈美拉汀治疗,评估2组患者治疗前匹兹堡睡眠量表评分(PSQI)评估治疗6周后两组的头痛疗效以及不良反应.结果 倒班人员的入组前PSQI的总评分、睡眠质量、入睡时间、催眠药物及睡眠障碍因子分高于非倒班人员;治疗组的疗效优于对照组,有统计学意义(P＜0.05);治疗组内,倒班患者疗效优于非倒班患者(P＜0.05).两组不良反应无明显差异.结论 阿戈美拉汀联合乙哌立松治疗慢性紧张性头痛的的疗效确切,安全性高.     

上海市普通外科病人静脉血栓栓塞症防治管理规范(2020版)

静脉血栓栓塞症(VTE)是指各种病因(如静脉血滞缓、静脉壁血管内膜损伤或血液高凝等)导致血液在静脉系统不正常凝集的一组疾病,包括肺动脉栓塞(PE)和深静脉血栓形成(DVT)。上海市普通外科临床质量控制中心自2018年发布《上海市普通外科住院病人静脉血栓栓塞症防治管理规范》(以下简称2018版《规范》)至今已2年,推动了上海市普通外科围手术期血栓防治工作的推广,同时积累了上海在血栓防治方面的临床数据。截止至2019年9月,上海市二级以上医院已有88%建立VTE防控体系,在病史中包含了VTE风险评估表,并且大部分医院已有电子病案的嵌入或纸质病史的记录;84%的医院有VTE和PE的联合会诊团队;92%医院的普通外科已按照2018版《规范》为住院病人评估VTE风险,74%的科室在病人手术后再次评估VTE风险,38%的科室在出院前会再次为病人评估VTE风险。上海市普通外科在临床实践中防治VTE的理念和实践有了很大进步,但与先进国家和医院的防治实践相比仍有较大差距。     

Unexpected Return for Follow-up During the First Year of Multidisciplinary Care May Be Predictive of Rapid Deterioration of Renal Function

Multidisciplinary predialysis education and team care (MDC) may slow the decline in renal function in patients with chronic kidney disease (CKD). However, associations between unexpected return during MDC and progression of renal dysfunction have not been characterized in patients with CKD. Our study aimed to determine the association between exacerbation of renal dysfunction and the frequency of unexpected return during follow-up.A total of 437 patients with CKD receiving multidisciplinary care between January 2009 and June 2013 at the Shin-Kong Wu Ho-Su Memorial Hospital were included in this retrospective observational cohort study, and multiple imputations were performed for missing data. The predictor was the frequency of unexpected return for follow-up during the first year after entering MDC. Main outcome was monthly declines in estimated glomerular filtration rates (eGFR). Moreover, the demographic data, comorbidities, history of medication, and routine laboratory data for patients with CKD were collected.Among all patients, 59.7% were male, the mean age at initiation of MDC was 69.4 ± 13.2 years, and the duration of follow-up was 21.4 ± 3.3 months. The subjects were divided into 2 groups according to frequencies of follow-up (≤4 and > 4 visits) during the 1st year of MDC. The patients with CKD were regularly followed up every 3 months as a part of MDC in our hospital, and patients who returned for more than 4 follow-up visits were included in the unexpected return group. In crude regression analyses, unexpected return was significantly associated with higher monthly declines of eGFR (β = 0.092, 95% confidence interval, 0.014–0.170). This association remained after adjustments for multiple variables, and subgroup analyses of unexpected return showed that male gender, older age, CKD stage 1 to 3, hypertension, history of coronary artery disease, and use of renin–angiotensin system blockade were significantly associated with declines in renal function.In conclusion, unexpected return for follow-up during the 1st year of MDC was significantly associated with the deterioration of renal function.
