Repetitive nerve stimulation (RNS) is a simple and rapid method for evaluation of neuromuscular transmission defects. Although the effect of exercise in conjunction with RNS is well recognized, it has not been standardized in actual patient and control groups. In a prospective study over a period of 1 year, the authors evaluated the effect of exercise in conjunction with RNS in comparison with conventional 3-Hz RNS at rest in the clinical setting. Fifty-four patients who were referred for possible neuromuscular transmission disorders, in addition to 35 healthy control subjects, were studied. Amplitude and area decremental responses with RNS at rest and after 20 seconds of maximal exercise at 1-minute intervals up to 3 minutes were evaluated. The use of RNS with exercise resulted in additional diagnostic yield of up to 36.4% compared with conventional 3-Hz RNS at rest. The standardized use of exercise with RNS is advocated for increasing its diagnostic yield in the neurophysiologic laboratory.  相似文献

Objective

It was previously reported that docosahexanoic acid (DHA) reduces TNF-α-induced necrosis in L929 cells. However, the mechanisms underlying this reduction have not been investigated. The present study was designed to investigate cellular and biochemical mechanisms underlying the attenuation of TNF-α-induced necroptosis by DHA in L929 cells.

Methods

L929 cells were pre-treated with DHA prior to exposure to TNF-α, zVAD, or Necrostatin-1 (Nec-1). Cell death and survival were assessed by MTT and caspase activity assays, and microscopic visualization. Reactive oxygen species (ROS) were measured by flow cytometry. C16- and C18-ceramides were measured by mass spectrometry. Lysosomal membrane permeabilization (LMP) was evaluated by fluorescence microscopy and flow cytometry using Acridine Orange. Cathepsin L activation was evaluated by immunoblotting and fluorescence microscopy. Autophagy was assessed by immunoblotting of LC3-II and Beclin.

Results

Exposure of L929 cells to TNF-α alone for 24 h induced necroptosis, as evidenced by the inhibition of cell death by Nec-1, absence of caspase-3 activity and Lamin B cleavage, and morphological analysis. DHA attenuated multiple biochemical events associated with TNF-α-induced necroptosis, including ROS generation, ceramide production, lysosomal dysfunction, cathepsin L activation, and autophagic features. DHA also attenuated zVAD-induced necroptosis but did not attenuate the enhanced apoptosis and necrosis induced by the combination of TNF-α with Actinomycin D or zVAD, respectively, suggesting that its protective effects might be limited by the strength of the cell death insult induced by TNF-α.

Conclusions

DHA effectively attenuates TNF-α-induced necroptosis and autophagy, most likely via its ability to inhibit TNF-α-induced sphingolipid metabolism and oxidative stress. These results highlight the role of this Omega-3 fatty acid in antagonizing inflammatory cell death.  相似文献

Measurement of the decremental muscle response to repetitive nerve stimulation (RNS) has low yields for the diagnosis of neuromuscular transmission defects compared with single fiber electromyography (SFEMG). We compared area and amplitude of muscle responses to RNS in 87 patients and 30 controls, using SFEMG as the reference standard. Decrement of response area provided additional diagnostic yields of 5.3% to 30% depending on the muscle examined and disease severity, and is recommended as a diagnostic adjunct to measurement of amplitude decrement during RNS.  相似文献

OBJECTIVE: To define the optimal nerve conduction study (NCS) technique of the pectoral nerves and evaluate its clinical utility. DESIGN: Prospective electrophysiologic study with healthy controls. SETTING: Electrophysiologic laboratory in a large general hospital. PARTICIPANTS: Thirty healthy controls and 10 patients with cervical root or brachial plexus pathologies. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURE: Correlation of pectoral NCS with electromyography and magnetic resonance imaging. RESULTS: For pectoral NCS, the mean values +/- standard deviation of onset latency, amplitude, and interside amplitude ratio (ratio of smaller over larger amplitude) were 2.01+/-0.22 ms, 11.75+/-2.21 mV, and .95+/-.04 mV, respectively. Subject age correlated significantly with both onset latency (r=.46, P<.001) and amplitude (r=-.34, P<.008). All 5 patients with brachial plexopathy had amplitude ratios below the normal limit of controls (.87). However, this was not seen for all 5 patients with cervical spondylotic radiculopathy. CONCLUSIONS: The pectoral NCS technique is feasible in healthy subjects. It is useful when differentiating brachial plexopathy from cervical root lesions.  相似文献

Introduction: Bell’s palsy is a commonly encountered paralysis of the facial nerve occurring worldwide. Prognosis for Bell’s palsy is good, but the proportion of patients with poor outcomes may reach 30%. Ultrasound (US) may provide a novel approach for evaluating and prognosticating Bell’s palsy, in comparison with known electrophysiological techniques. Methods: In this study, we measured the diameter of the distal facial (VII) nerve using US in patients with Bell’s palsy treated with prednisolone, in comparison with healthy controls. Blink reflex and VII nerve conduction studies were also performed. Studies were prospective and performed within 1 week of disease onset. Results: Our results have shown that diameter of the distal VII nerve is a good predictor of favorable (positive predictive value: 100%) and bad outcomes (negative predictive value: 77%) in Bell’s palsy at 3 months after clinical presentation. Furthermore, we also noted the lack of correlation of VII diameter with conventional VII nerve conduction studies (NCS) and blink reflex studies. US was superior to VII nerve conduction and blink reflex studies in outcome prediction. Conclusions: This first study utilizing US in Bell’s palsy highlights its role in outcome prediction and contributes to our understanding of recovery processes in this common neurological disorder.  相似文献

It is generally accepted that median sensory nerve conduction studies are more sensitive than motor nerve conduction studies in the electrodiagnostic evidence of carpal tunnel syndrome (CTS). This study was conducted to compare the sensitivities of various parameters of sensory nerve conduction studies in the diagnosis of CTS.This prospective study included 88 consecutive patients (151 hands) with CTS and 106 control subjects. CTS was diagnosed clinically by two neurologists. Median sensory nerve responses with wrist stimulation were determined. The onset and peak latencies, peak-to-peak amplitudes, negative peak duration, and area were measured. The differences between the peak and onset latencies were also calculated as a measure of waveform temporal dispersion. Among each measured parameter, values between the 2.5th and the 97.5th percentile range of the control subjects served as the normal limits.Among the 151 hands with suspected CTS, five (3.3%) had normal electrodiagnostic studies and 146 (96.7%) had at least one abnormal electrodiagnostic study. Among the 146 hands with an abnormality, 138 had abnormal onset latency, 143 had abnormal peak latency, and 88 had abnormal difference between peak and onset latency. In addition, 87 had abnormal amplitude, 70 had abnormal duration, and 59 had abnormal area. The sensitivity was 91.4% for onset latency, 94.7% for peak latency, 58.3% for difference between peak and onset latency, 57.6% for amplitude, 46.4% for duration, and 39.1% for area.Our study shows that in patients with CTS, the most sensitive sensory nerve conduction parameter is the peak latency. Studying various additional sensory nerve conduction parameters did not significantly increase the diagnostic yield.  相似文献

Short-segment nerve conduction studies were performed in 17 limbs with clinical features suggestive of ulnar neuropathy at the elbow. Recording from flexor carpi ulnaris yielded 93% sensitivity, compared with 71.4% when recording from abductor digiti minimi. The rationale underlying the technique is discussed. This approach should be of value as a diagnostic adjunct in technically challenging cases of ulnar neuropathy at the elbow.  相似文献

Radial neuropathy most commonly occurs as a result of external compression at the spiral groove region. The posterior antebrachial cutaneous nerve (PACN) conduction study was performed in 15 consecutive patients with radial palsy. Unilateral PACN abnormalities were present in 11 patients. A normal PACN study was correlated with clinical improvement at 3 months. Conversely, PACN abnormality was correlated with radial motor axon loss and a poorer prognosis. The PACN study is a simple adjunct which provides additional information relating to the diagnosis and prognosis of radial lesions.  相似文献

Since plasma exchange (PE) and intravenous immunoglobulin (IVIg) have been widely used in treatment for Guillain–Barré syndrome (GBS), early relapse and treatment-related fluctuation have been a potential problem, but little is known about the mechanism of relapse and fluctuation. We describe a patient who had GBS with treatment-related fluctuation. A 37-year-old Japanese man exhibited acute distal-dominant weakness in upper limbs after upper respiratory infection. His cranial nerve system was normal and muscle weakness was limited to upper limbs. Anti-GT1a IgG was strongly positive and anti-GQ1b IgG was also detected in his serum. Muscle weakness responded well to double-filtration plasmapheresis (DFPP) followed by IVIg, but relapsed 45 days after the initial treatment. Although repeated treatments were effective, the patient showed additional minor deterioration twice. Motor nerve conduction velocities (MCVs) corresponded to the muscle weakness, but elevated level of cerebrospinal fluid (CSF) protein remained and anti-ganglioside antibody titers steadily decreased throughout the clinical course. These findings indicate that the clinical fluctuation was not due to changes in the production of anti-ganglioside antibodies but presumably to the transient beneficial effects of DFPP/IVIg and the outlasting inflammatory response in peripheral nerves.  相似文献

Repetitive hypoglossal nerve stimulation in myasthenia gravis.   总被引:1,自引:0,他引:1  
OBJECTIVES: To assess the diagnostic efficacy of repetitive nerve stimulation (RNS) of the hypoglossal nerve in patients with myasthenia gravis (MG) and bulbar symptoms (dysphagia, dysarthria). METHODS: Twenty patients with MG and 25 normal controls had RNS of the hypoglossal nerve. All patients also had single fibre electromyography (SFEMG) of the orbicularis oculi and RNS with recordings of the nasalis, trapezius and abductor pollicis brevis muscles. RESULTS: All patients had positive SFEMG studies. Nine patients with bulbar symptoms had positive hypoglossal RNS, including 3 with negative RNS recordings in other muscles. Eleven patients with no bulbar symptoms showed negative hypoglossal RNS, including two with positive RNS recordings from other muscles. CONCLUSIONS: Abnormal RNS of the hypoglossal nerve correlates well with bulbar dysfunction and further characterises the extent of neuromuscular transmission defect in MG patients.  相似文献
