Congenital Heart Disease in Adolescents With Gluteal Muscle Contracture

Gluteal muscle contracture (GMC), presented with hip abduction and external rotation when crouching, is common in several ethnicities, particularly in Chinese. It remains unclear that the reasons why these children are weak and have no choice to accept repeated intramuscular injection. Here, we found some unique cases which may be useful to explain this question.We describe a series of special GMC patients, who are accompanied with congenital heart disease (CHD). These cases were first observed in preoperative examinations of a patient with atrial septal defect (ASD), which was proved by chest X-ray and cardiac ultrasound. From then on, we gradually identified additional 3 GMC patients with CHD. The original patient with ASD was sent to cardiosurgery department to repair atrial septal first and received arthroscopic surgery later. While the other 3 were cured postoperative of ventricular septal defect (VSD), tetralogy of fallot (TOF), patent ductus arteriosus (PDA), respectively, and had surgery directly.The study gives us 3 proposals: (1) as to CHD children, it is essential to decrease the use of intramuscular injection, (2) paying more attention to cardiac examination especially cardiac ultrasound in perioperative period, and (3) taking 3D-CT to reconstruct gluteal muscles for observing contracture bands clearly in preoperation. However, more larger series of patients are called for to confirm these findings.     

The influence of processed total non-forward and non-motile sperm count on the outcome of artificial insemination with the husband’s semen

BackgroundArtificial insemination with the husband’s semen (AIH) is an economical and noninvasive method of infertility treatment. However, AIH’s pregnancy rate is much lower than in vitro fertilization (IVF) as its multiple and complex uncertainty factors. Semen quality has been one of the main factors which affect the pregnancy outcome of AIH.MethodsThe relevant parameters of 1,142 AIH cycles were retrospectively studied, including the general parameters and the semen quality parameters among clinical pregnancy, biochemical pregnancy, non-pregnancy group, age, infertility duration, infertility type, body mass index (BMI), cycle count, morphology in previously semen examination, and semen quality parameters on the day of AIH.ResultsThe statistically significant difference was only found on processed total non-forward and non-motile sperm count (N-TFMSC). The mean processed N-TFMSC in the biochemical pregnancy group was 6.37±4.27 million, significantly higher than the other two groups (vs. 4.40±3.15 million or vs. 4.48±3.60 million, P<0.05). The study was then divided into two groups according to processed N-TFMSC, Group 1 ≤5.0 million, and Group 2 >5.0 million. A statistical increase in biochemical pregnancy rate was observed when the processed N-TFMSC was >5.0 million (2.72% vs. 0.90%).ConclusionsProcessed N-TFMSC may be one of the independent factors on AIH’s outcome; it should be given equal attention the same as processed total forward motile sperm count (TFMSC).     

逍遥散治疗经前期综合征69例

逍遥散出自《太平惠民和剂局方》，由当归、白芍药、柴胡、茯苓、白术、炙甘草、薄荷、煨姜组成。经前期综合征为临床常见的妇科病症，笔者近年来采用逍遥散加减治疗经前期综合征69例，疗效满意，现报道如下。     

异搏定防治流行性出血热初期急性肾功能衰竭的疗效观察

在综合疗法的基础上加用异搏定3～5天治疗流行性出血热(EHF)发热后期病人41例,在尿蛋白转阴、越期率,特别是越少尿期平明显优于对照组,但对 BUN 水平的影响与对照组无异,异搏定对防治 EHF 急性肾衰具有一定疗效。     

授权科室成员质量控制对护理质量的影响

目的探索授权科室成员进行护理质量控制的效果.方法本着"质量控制,人人有责"原则,授权科室成员参与质量控制,并将科室成员划分为5个质控小组,即护理服务组、重患手术组、文件书写组、安全消毒组和基础护理组.各组负责科室各环节护理质量控制.比较质量控制组成立前后的效果.结果质量控制组成立后护士的"三基"理论水平、护理质量、患者满意率均高于成立前,差异有显著性意义(均P<0.05).结论授权科室成员进行质量控制,是全面提升护理质量的有效管理方法.     

残胃癌临床诊治分析

目的分析和总结残胃癌的外科诊断、治疗及预后情况。方法对1990年以来收治的35例残胃癌的发病率、临床表现、治疗情况和预后进行回顾性分析。结果首次胃切除以胃溃疡为主,占65.7%,首次手术BillrothⅡ式占82.9%(29/35)。残胃癌早期缺乏特异的症状,主要为上腹部隐痛不适,首次手术距临床诊断残胃癌的间隔时间平均为16.3年。本组残胃癌手术切除率及根治性切除率分别为77.1%和54.3%,根治性切除患者1、3、5年生存率分为78.9%、52.6%、36.8%,Ⅰ期、Ⅱ期患者的5年生存率为72.7%。结论胃良性病行手术治疗时以BillrothⅠ式为首选,定期胃镜检查是早期诊断残胃癌的关键,根治性切除是治疗残胃癌的有效方法及影响残胃癌预后的重要因素。     

α－脂蛋白测定质量控制、标本贮存及其正常参考值

分次使用国产ＥＬＩＳＡ药盒测定血清（浆）Ｌｐ（α）浓度过程中，进行质控、标本贮存等实验，并建立Ｌｐ（α）浓度水平的正常参考值。结果：①血清Ｌｐ（α）测值平均组内变异５．１８％、组间９．８６％，实验室间标准质控变异率０．１８％；②标本存放于—２５℃３１０天或者冻融３次后Ｌｐ（α）测值均无明显改变；③Ｌｐ（α）的血清与血浆值之间高度相关（ｒ＝０．９７４，Ｐ＜０．００１，ｎ＝６０）；④血清Ｌｐ（α）浓度在本室的正常参考值为２０４．３±１２６．４ｍｇ／Ｌ，并且呈正偏态分布，经对数或平方根变量交换均可使其正态化。     

股骨假体周骨折治疗方法的探讨

目的 探讨股骨假体周骨折不同处理方法的临床应用价值。方法 对16例股骨假体周骨折分别采用钢丝、钢板螺钉、环抱器固定和更换长柄假体植入方法处理．总结分析其相关临床资料。结果 16例病人术后随访4～14个月．平均6个月，全部病例骨折愈合，功能恢复良好。髋关节功能评定：90分以上3例、80～89分9例、70-79分3例、63分1例，优良率达75％。结论 股骨假体周骨折类型不同，处理方法应不同：股骨颈端到小粗隆下缘的劈裂骨折，采用钢丝捆扎；全劈裂骨折移位明显，更换较小型号的假体，再用钢丝捆扎固定并植骨；小粗隆下缘到股骨干髓腔狭窄段的上缘骨折，选用长柄假体植入再用钢丝捆扎，假体远端加锁固定或选用环抱器固定；股骨干髓腔狭窄段以下的骨折，采用钢板固定或环抱器固定并植骨。     

大白鼠皮肤Ⅱ度、Ⅲ度碱烧伤模型复制

目的 根据大白鼠皮肤碱烧伤创面早期病理组织学观察复制Ⅱ、Ⅲ度碱烧伤模型,研究碱烧伤的临床治疗.方法 Wistar纯种健康大白鼠20只,以不同浓度2mol/l、2.5mol/l、5mol/lNaOH,作用时间分别为30秒、45秒、60秒、75秒,涂抹于脱毛后的表皮.结果 2mol/L(60s)、2.5mol/L(45s),5mol/L(30s)即可直接造成Ⅱ度～深Ⅱ度烧伤;2mol/L(75s)、2.5mol/L(60s)、5 mol/L(45s)即可造成Ⅲ度烧伤,其烧伤深度与NaOH溶液浓度和作用时间呈正比.结论 Ⅱ度、Ⅲ度碱烧伤均为渐进性烧伤,与文献中介绍的潜拙样损伤似乎不同.另外,不同浓度NaOH溶液在相同时间对皮肤组织的损伤及同一浓度NaOH溶液在不同时间对大白鼠皮肤的损伤病理学变化均有差异.
