The introduction of new techniques such as epicardial mapping and programmed stimulation of the heart has made it possible to unravel most of the mysteries surrounding ventricular preexcitation. They helped us to understand the mechanisms of the arrhythmias that frequently are found in these patients and led to better-directed therapeutic interventions. The purpose of this article is to review this information and to indicate how it can be used in the treatment of the patient with preexcitation.  相似文献

Antimurine antibody formation following OKT3 therapy   总被引:1,自引:0,他引:1  
OKT3 is an IgG2a murine monoclonal antibody directed against the CD3 antigen receptor of human T lymphocytes. A major concern with OKT3 treatment in solid organ transplant recipients is the development of antimouse antibody, which may preclude retreatment with this agent. We have administered OKT3 on 215 occasions (150 renal, 34 hepatic, 26 cardiac, 5 pancreatic) in 179 patients between April 1982 and December 1988. The mean duration of treatment was 10.5 days (range, 2-22 days). Antimouse antibody data were analyzed on the most recent 133 treatment courses where the antibody status was available pretreatment. Determination of antimouse antibody production was elicited by ELISA technology at days 0, 7, 14, and 28 of OKT3 treatment. Patients were categorized according to the antibody response as follows: (a) absence of antibody; (b) low titer (1:100); or (c) high titer (greater than or equal to 1:1000). Our earlier experience has demonstrated that retreatment with OKT3 is successful in groups a and b. The development of antimurine antibodies was analyzed with regard to the following parameters: (1) The duration of OKT3 treatment; (2) treatment type (prophylactic, primary, or secondary); (3) primary treatment or retreatment; (4) concomitant immunosuppressive regimen (double or triple therapy); (5) dosage of concomitant immunosuppressive drugs; and (6) transplant organ type. The following results were obtained. (1) Duration of treatment had no effect on antibody production (11.0 days in antibody negative and 10.0 days in antibody positive). (2) There was no difference in antibody formation rates for the first treatment of OKT3 when it was used as prophylaxis (26%), primary (19%), or secondary (27%) therapy. (3) Antibody formation rate with first treatment was 29%; with retreatment, patients who were antibody negative following first treatment became positive in 28% of cases, and retreated patients who were low titer positive following first treatment converted to high titer in 57% of cases. (4) Antibody formation was higher in patients receiving double immunosuppressive therapy (36%) than in those receiving triple immunosuppressive therapy (21%) during OKT3 treatment. (5) Concomitant immunosuppression was lower in the antibody-positive group during OKT3 therapy: steroids, 61 mg/day vs. 52 mg/day; azathioprine, 89 mg/day vs. 66 mg/day; CsA, 317 mg/day vs. 186 mg/day. (6) Antibody formation rates were lower in non-renal transplants following first treatment with OKT3 (liver 17%, heart 17%, kidney 28%); this reflects the higher doses of concomitant immunosuppressive therapy used in nonrenal transplants.(ABSTRACT TRUNCATED AT 400 WORDS)  相似文献

It recounts the initiative made by Strathclyde Region to co-ordinate and integrate pre-five provision, administratively and at a service level. We review a ten-year experience in treating 60 patients with multiple myeloma. Infectious episodes occurred in 33 patients. Urinary tract infections caused by gram-negative organisms were the most frequent infections, and most of these were the result of catheterization. Pneumonia due to Streptococcus pneumoniae was encountered infrequently. This series confirms the emergence of gram-negative bacilli as the predominant pathogens in patients with multiple myeloma, and emphasizes the risk of instrumentation in these patients.  相似文献

Proximal femoral focal deficiency, an uncommon congenital anomaly, necessitates early radiologic classification for surgical planning and treatment. Objective radiographic criteria, including femoral length index, acetabular depth index, acetabular angle index, and shape of the proximal femur were determined in 49 patients before cartilaginous ossification of the femoral capital epiphysis; final classification was based on follow-up radiographs or findings at arthrography or surgery. These parameters were analyzed to determine the accuracy and contributions of each in classification. Correct classification into one of three groups was possible in 86% of cases with use of three of the parameters: femoral length index, acetabular depth index, and shape of the proximal femur. The acetabular angle was found to contribute insignificantly to classification. Magnetic resonance imaging, used in only one case, depicted the nonossified cartilaginous femoral capital epiphysis, thus obviating the need for invasive diagnostic procedures and facilitating early classification.  相似文献

The current technique was developed to characterize the morphologic changes in the retinas of oxygen-reared rats, as an animal model of retinopathy of prematurity. Past studies have used ink perfusion to observe the retinal vasculature, but this method is static and requires the sacrifice of the subject. Fluorescein angiography, however, is dynamic and relatively noninvasive, and allows the survival of the animal for further study. The fundus camera cannot be used because the source of light that is focused in an annulus is too large for the pupil size of a young (approximately 14-day-old) rat. To overcome this, a Nikon inverted microscope (Diaphot-TMD) was used. Using the proper exciting and barrier filters for fluorescene, a photographic sequence was made by rapidly focusing to the plane of the retinal vessels. To our knowledge, similar photographs have not been previously published. This technique was used in newborn pigmented ratlings that were 1) exposed to 80% oxygen for the first 14 days of life; 2) exposed to 80% oxygen for the first 21 days of life; or 3) exposed for the first 14 days followed by 7 days in room air. Age-matched controls were raised simultaneously in room air and evaluated with the same technique. Differences were observed between treatments in the amount of retinal capillary loss, and in the tortuosity and diameter of the major retinal vessels. The hyaloid system also varied between treatment groups. Oxygen-exposed rats showed a persistence of the hyaloid vessels that was particularly prominent in the group returned to room air before analysis. Comparisons are made to past results obtained with other histologic techniques.  相似文献
