This study addresses an advantageous application of a urinary zearalenone (ZEN) monitoring system not only for surveillance of ZEN exposure at the production site of breeding cows but also for follow-up monitoring after improvement of feeds provided to the herd. As biomarkers of effect, serum levels of the anti-Müllerian hormone (AMH) and serum amyloid A (SAA) concentrations were used. Based on the results of urinary ZEN measurement, two cows from one herd had urinary ZEN concentrations which were two orders of magnitude higher (ZEN: 1.34 mg/kg, sterigmatocystin (STC): 0.08 mg/kg in roughages) than the levels of all cows from three other herds (ZEN: not detected, STC: not detected in roughages). For the follow-up monitoring of the herd with positive ZEN and STC exposure, urine, blood, and roughage samples were collected from five cows monthly for one year. A monitoring series in the breeding cattle herd indicated that feed concentrations were not necessarily reflected in urinary concentrations; urinary monitoring assay by ELISA may be a simple and accurate method that reflects the exposure/absorption of ZEN. Additionally, although the ZEN exposure level appeared not to be critical compared with the Japanese ZEN limitation in dietary feeds, a negative regression trend between the ZEN and AMH concentrations was observed, indicating that only at extremely universal mycotoxin exposure levels, ZEN exposure may affect the number of antral follicles in cattle. A negative regression trend between the ZEN and SAA concentrations could also be demonstrated, possibly indicating the innate immune suppression caused by low-level chronic ZEN exposure. Finally, significant differences (p = 0.0487) in calving intervals between pre-ZEN monitoring (mean ± SEM: 439.0 ± 41.2) and post-ZEN monitoring (349.9 ± 6.9) periods were observed in the monitored five cows. These preliminary results indicate that the urinary ZEN monitoring system may be a useful practical tool not only for detecting contaminated herds under field conditions but also provides an initial look at the effects of long-term chronic ZEN/STC (or other co-existing mycotoxins) exposure on herd productivity and fertility.  相似文献

Abstract

An in vitro study was performed investigating the bactericidal ability of five common cleaning agents for leg bags against organisms frequently causing urinary tract infection in spinal cord injured (SCI) patients. Individual disposable Uridrain leg bags were inoculated with urine containing > 105 colonies/ml of the selected organism. The organisms used were: Pseudomonas aeruginosa, Proteus mirabilis, Providencia stuartii, Klebsiella pneumoniae, Serratia marcescens, Herellea, and Enterococcus. Each pair of inoculated bags (one bag for immediate or 0 hour culture and one bag for 24 hour culture) was filled with 30 ml of the test solution, cleaned, drained, and then cultured. The tested solutions included: plain tap water, 1/4% acetic acid, 3% hydrogen peroxide, Sporicidin 1:16 dilution, and 0.06% sodium hypochlorite. The results demonstrated that plain tap water had no effect on reducing the bacterial counts and that 1/4% acetic acid and 3% hydrogen peroxide had only a marginal effect across the spectrum of organisms. Sporicidin and 0.06% sodium hypochlorite exhibited complete bactericidal ability. In the final analysis, 0.06% sodium hypochlorite is the most cost efficient and readily accessible to patients. In practical application, disinfecting daily with 0.06% sodium hypochlorite can decrease external contamination of urinary drainage appliances in SCI patients.  相似文献

Few reports on nutritional management of patients with both brain damage and spinal-cord-injury appear in the literature. We present a case of a 20-year-old male quadriplegic, C4 complete, who also sustained brain damage secondary to cerebral anoxia. When the patient was transferred to our rehabilitation unit, deterioration in nutritional status was noted, as evidenced by weight loss and depressed serum albumin and hemoglobin. Nutritional rehabilitation consisted of weaning from nasogastric tube feedings to an oral diet providing snacks and commercial supplements. This resulted in a positive nitrogen balance. Other factors, such as mobilization, exercises, and closure of a pressure sore, contributed favorably to improvement of nutritional status.  相似文献

Chronic pain is a common clinical finding in spinal cord injury (SCI), with a reported incidence of between 45% and 90%. This figure was obtained by using nonstandardized pain evaluation and for mostly inpatient populations. Because of the shortcomings of previous investigations and the wide range of reported incidence, a study was conducted using self-rating pain measurement, an activity check list, and a drug-use rating scale. Pain in 40 hospitalized SCI patients (19 quadriplegics and 21 paraplegics) was evaluated according to the self-rating pain scale and for physical activities. The medical record of each of these patients was then reviewed to evaluate use of pain medications. Using the same method, 24 outpatients (12 with quadriplegia and 12 with paraplegia) in the Hospital Based Home Care Program were studied. Statistical analysis showed an incidence of chronic pain and decreased activity of 60% among inpatients and of 16.6% in outpatients. The drug-use rating scale was also significantly higher among inpatients while outpatients had a higher level of physical activity than inpatients. Whether these differences are causally related to the patient's hospitalization is difficult to determine.  相似文献
