Background: Recent development of extracorporeal magnetic stimulation (ECMS) which uses current‐changing magnetic fields allows the induction of electrical stimulation in the desired deep tissue. Recent study showed the sacral nerve stimulation reduces corticoanal excitability that may play a functional role in anal continence mechanisms. Preliminary study shows that ECMS of sacral nerve can modify pelvic floor function and expel rectal balloon in patients with pelvic floor dyssynergia (PFD). Aims: To evaluate the effect of ECMS compared with biofeedback therapy (BF) in patients with PFD. Methods and Materials: Thirty‐eight patients who fulfilled Rome II criteria for PFD by colon transit time and anorectal function tests, were randomly treated with 8 sessions of ECMS (2/weeks; n = 19) at prone position or BF (2/weeks; n = 19) at sitting position. Stimulation parameters were set at 50–80% of maximum intensity, 10 and 50 Hz frequency, 3 s burst length with 3 and 6 s off using arm‐typed stimulator (BioCom‐1000, Mcube Co., Korea). Symptom scores for constipation with/without anorectal function test were repeatedly measured after each treatment. Response was defined as 50% or more decreased symptom score after treatment (partial response: 30–50%, poor: <30%). Results: Fifteen patients (age 49.1 ± 13.4 years, mean ± SD; 4 men) completed 8 session of BF and 14 patients (54.5 ± 17.6 years, 3 men) completed 8 session of ECMS. Four patients of BF group discontinued treatment due to unsatisfactory therapeutic effect (n = 1) and withdrew consent (n = 3) and 5 patients of ECMS group discontinued treatment because of same reasons (n = 1, 4). Total symptom scores were significantly decreased after treatment of 8 session in both treatment groups (13.4 ± 6.6 vs. 4.3 ± 4.0 for BF, p = 0.009; 14.9 ± 5.6 vs. 3.4 ± 4.0 for ECMS, p < 0.001). Bowel movements per week were also significantly increased after treatment in both groups (median 2 vs. 7 for BF, p = 0.035; median 2 vs. 7 for ECMS, p = 0.008). Thirteen out of 15 patients showed response in BF group and 12 out of 14 showed good response in ECMS group. No adverse effects in both groups. Conclusions: ECMS is as effective as BF for the treatment of PFD. Long‐term effect of ECMS for the patients with pelvic floor dyssynergia need to be evaluated in the near future. 相似文献

Previous study has shown that α2D-adrenoceptors are involved in modulation of peristalsis in the rat ileum. The aim of the present study was to determine the
tissue location of α-adrenoceptors in the rat ileum by using a recently devised method. The pre-synaptic α-adrenoceptors were
characterised by measuring the potencies of agonists to inhibit transmurally-evoked (1 ms pulses, 10 Hz, 8-10 s trains) contractions
of the longitudinal and circular muscles and the affinities of antagonists. Post synaptic α-adrenoceptors were identified
by screening agonists and antagonists in carbachol-contracted tissues. In the circular muscle the order of potencies for inhibiting
transmurally-induced contraction was: clonidine ≥ oxymetazoline ≥ UK 14,304 ≥ guanfacine > talipexole > phenylephrine > azepexole.
The potency ratios relative to clonidine correlated to those previously derived using the rat ileum peristaltic reflex preparation.
Most of the α-adrenoceptor agonists, however, caused only small inhibitions of the longitudinal muscle contraction in response
to transmural stimulation, except phenylephrine and azepexole. RX 821002, yohimbine, rauwolscine, BRL 44408, phentolamine,
idazoxan, ARC 239, and prazosin inhibited the effect of clonidine on the circular muscle response with apparent pKB values best correlated with pKB or pKi values derived from the rat ileum peristaltic reflex preparation and other tissues known to have the α2D-subtype. The rank order of potencies at inhibiting carbachol-induced responses of both muscle layers was: phenylephrine ≥
oxymetazoline > clonidine ≥ talipexole > azepexole >> guanfacine. UK 14,304 was inactive up to 10 μM. The EC50 value of each agonist on the longitudinal muscle was not significantly different to the corresponding value on the circular
muscle. Prazosin was more potent than yohimbine at inhibiting the relaxant effect of phenylephrine in both muscle layers of
carbachol-contracted tissues. It is concluded that the recently identified α2D-adrenoceptors of the rat ileum are located on cholinergic neurons controlling circular muscle contraction. The study also
demonstrated the presence of postsynaptic α1-adrenoceptors involved in mediating relaxation in both muscle layers.

The vascularity of 49 renal masses (26 malignant and 23 benign lesions) was investigated with duplex Doppler ultrasound. Doppler signals obtained at the margins of renal masses were defined as "tumor signals" when the Doppler-shifted frequency of the lesion exceeded the frequency shift in the ipsilateral main renal artery. These exceeded 2.5 kHz with a 3-MHz insonating frequency. Among the 26 renal masses that subsequently proved to be malignant, tumor signals were obtained in 15 of 18 (83%) untreated renal cell carcinomas, in three of four Wilms tumors, and in two patients with metastases to the kidney, but not in the one patient with lymphoma. None of the 23 benign renal masses demonstrated tumor signals. Tumor vascularity in malignant lesions gives rise to abnormal, high-velocity, Doppler-shifted signals that can help in the differential diagnosis of renal masses. 相似文献

The clinical history and histological features of seven cases of granulomatous mastitis are presented. The lesion occurs in young parous women as a tender extra-areolar breast lump. Histologically, non-caseating discrete granulomas are present, confined to breast lobules with, in three cases, coalescence of the granulomas and microabscess formation. Pathogenesis of the changes is discussed. It is thought that granulomatous mastitis is an entity morphologically distinct from duct ectasia/plasma cell mastitis and the commoner forms of granulomatous breast diseases. 相似文献

Summary. The mild fowlpox vaccine, FPV M, widely used in Australia is composed of two predominant genotypes based upon differences
identifiable in restriction enzyme analyses of plaque purified derivatives of this vaccine. The differences, where identifiable,
were in the end fragments of the genomes. Five field isolates of FPV from chickens in New South Wales showed restriction enzyme
profiles closely related to the more virulent (standard) vaccine strain, FPV S. The FPV S strain differs from FPV M in both
terminal genome fragments and in the presence of a PstI fragment of approximately 10kb (this fragment was also present in
PstI digests of all of the field isolates). Plaque purified derivatives of FPV M showed similar lesion development upon inoculation
into the wing web of chickens. The field isolates showed significantly higher virulence in day-old and three-week-old chickens
in comparison with FPV M. One field isolate was similar to the FPV S vaccine. Two isolates had slowly developing wing web
lesions, caused significant secondary lesions in three-week-old chickens and generalised poxvirus infection when inoculated
into day-old chickens. For two isolates, the primary wing web lesion took even longer to develop and resolve although these
isolates did not cause generalised poxvirus infection. It was possible to identify four virulence/pathogenicity types amongst
these vaccine and field isolates of FPV. These strains may allow the characterisation of FPV encoded virulence factors. The
field strains with higher virulence may be suitable as parent strains for the construction of FPV recombinants with enhanced
immune responses to co-expressed vaccine antigens when compared with current FPV M strain based recombinants.
