慢性充血性心力衰竭时硝酸盐对心排出量和左心室流出阻力的影响

本文旨在进一步评价硝酸盐对慢性心力衰竭患者的血液动力学效应,并与纯粹减低前负荷的止血带法以及同时扩张动脉与静脉血管的静脉滴注硝普钠法相比较,以探讨流出阻力减低与前负荷减低在硝酸盐血液动力学反应中的相对作用。方法:28例Ⅲ或Ⅳ级充血性心力衰竭的心肌病患者,用Swan-Ganz导管作右心插管和血液动力学检查。自肱动脉直接测量血压。用染料稀释法测定心排出量。以肺动脉楔嵌压或肺动脉舒张压作为左心室充盈

ILEOCECAL POINT TENDERNESS IN DIAGNOSIS OF TUBERCULOUS PERITONITIS

A clinical study was undertaken to ascertain the significance of a probably newly discovered physical sign of tenderness at ileocecal point in diagnosis of tubercujous peritonitis (TP). It is a point at the junction of the inner and middle thirds of theiright umbilico-iliac line joining the umbilicus and the anterior superior iliac spine. Based on the study of 41 patients and an equal number of nontuberculous disorder served as controls, the rate of tendernesis at this point in TP is 85.4%, statistically significant in compari- son with the controls or any other 5 points in- cluding McBurney's point in the TP group. The author suggests that the tenderness at this point may be valuable as a diagnoistic clue for the de- tection of both early and atypical TP.
