Two cases of vasovagal syncope (VVS) during venous access are reported. Both patients had a history of fainting episodes and experienced bradycardia with asystole, hypotension, and fainting. Pain and phobic stress during venous access triggered an increase in parasympathetic tone, resulting in bradycardia with asystole and hypotension in both cases. Hypotension and bradycardia likely caused cerebral hypoperfusion, leading to fainting. The intense parasympathetic tone triggered by somatic or emotional stress was likely responsible for directly depressing the sinus node, leading to asystole and bradycardia. Bradycardia with asystole progressing to syncope is a potentially fatal dysrhythmia in patients with cardiovascular disease or older patients with decreased cardiac function. Appropriate treatment for VVS includes the administration of intravenous fluids, vagolytics, ephedrine, and the rapid use of the Trendelenburg position. Pigeons were trained to perform simultaneous pattern and color discrimination tasks. After their training was completed, bilateral electrolytic lesions were made in the ventral lateral geniculate nucleus (GLv). Following the surgery, they were retrained to their preoperative performance levels. Lesions of GLv caused no deficits in pattern discrimination performance. The birds which had been trained for discrimination of red vs. magenta showed a slight decline in their performance. This impaired performance on color discrimination was not, however, as severe as that of a bird with lesions in the nucleus rotundus. The cell bodies of the sensory and sympathetic pre- and postganglionic neurons projecting into the cervical sympathetic trunk were retrogradely labeled with horseradish peroxidase in the chicken. Preganglionic neurons were located in the spinal segments T1-T6 (maximum T2), postganglionic neurons in the paravertebral ganglia T1-T3 (maximum T1) and sensory neurons in the dorsal root ganglia T1-T4 (maximum T1). Labeled preganglionic neurons were widely distributed across the intermediate gray matter and lateral funiculus, but the majority of them were located in the intermediomedial area dorsolateral to the central canal. The short and long axis diameters of labeled preganglionic neurons in this area decreased caudally. From the data of the present study, it is estimated that about 4190 preganglionic, about 450 postganglionic and about 390 sensory neurons project into the cervical sympathetic trunk cranial to the paravertebral ganglion T1 in the chicken. 相似文献

Binswanger’s disease is pathologically characterized by a combination of diffuse cerebrovascular white matter lesions and
lacunar infarcts in the basal ganglia and white matter. Although a blood-brain barrier (BBB) dysfunction has been implicated
in the pathogenesis of these white matter (WM) lesions, few authors have addressed this problem. In the present study, we
describe BBB dysfunction and its regional differences in the brains of Binswanger’s disease patients. Twelve brains from Binswanger’s
disease patients (group III) were examined and compared with those from five patients with non-neurological disease (group
I) and five cortical infarct patients without significant WM lesions (group II). Immunohistochemistry was performed for glial
fibrillary acidic protein and vimentin as astroglial cell markers, and for immunoglobulins, complements and fibrinogen as
extravasated serum protein markers. The grading scores for IgG extravasation were significantly higher in group III as compared
to group I, in both the periventricular WM and the subcortical WM (P < 0.01). In group III, the scores in the periventricular WM and subcortical WM were significantly higher than in the subcortical
U fibers and cerebral cortex (P < 0.01 for the periventricular WM; P < 0.001 for the subcortical WM), respectively. Clasmatodendritic astroglia, which had swollen cell bodies and large cytoplasmic
vacuoles with disintegrated processes, incorporated the serum components IgG, IgM, C3d, C1q and fibrinogen, both in the periventricular
WM and subcortical WM in 5 out of 12 (42%) Binswanger’s disease brains. These results indicate that WM lesions in Binswanger’s
disease are accompanied by BBB dysfunction, although it remains uncertain whether BBB dysfunction is secondary to either chronic
cerebral ischemia or arterial hypertension.

The characteristics of acetate uptake by colonic epithelial cells of the rat were studied. Clear saturation kinetics of acetate uptake were not observed in these experiments at either 0° C or 30° C. A decrease in the pH of the medium markedly increased the acetate uptake. The activation energy for acetete uptake derived from an Arrhenius plot was about 6.1 kcal/mole. Among the inhibitors tested, no effective inhibition of acetate uptake at 0° C was observer. Metabolic inhibitors severely inhibited transport at 30° C. Inhibition of acetate uptake by other short chain fatty acids, which was non-competitive, was observed. The finding that efflux from the cells was stimulated in the presence of compounds such as pyruvate and bicarbonate supported the notion of a close interrelationship between weak electrolyte transports in vivo. Although the H+ gradient across the cell membrane is suggested to be one of the factors determining the uptake rate, it seems difficult to explain all the results in this way. 相似文献

Since patients with primary biliary cirrhosis (PBC) have evidence of abnormal function of the immune system, we evaluated production of various cytokines by peripheral blood mononuclear cells (PBMCs) and monocytes from patients with this disease, using an enzyme-linked immunosorbent assay. The mean amounts of production of tumor necrosis factor alpha(TNF alpha), interleukin 1 beta (IL1 beta), and interferon-gamma (IFN-gamma) by PBMCs from patients with PBC tended to be increased in cultures in the presence of stimulating agents in comparison with controls, but there was no significant difference because of a wide scatter of results. Monocytes from PBC patients also tended to produce higher amounts of TNF alpha and IL1 beta than control monocytes did, although the percentage of monocytes in PBMCs was similar in PBC and controls. A significant correlation was found between TNF alpha production and IL1 beta production in PBC patients. The number of TNF alpha or IFN-gamma positive infiltrating mononuclear cells detected by immunohistochemical staining in liver biopsy sections correlated with the production of these cytokines by PBMCs in vitro. However, cytokine production did not correlate with serum biochemical or hepatic histologic findings, except for serum alkaline phosphatase values. In patients with type B chronic active hepatitis, IL1 beta and IFN-gamma production was similar to controls, while TNF alpha production tended to be enhanced. A new method for preventing pulse pileup in scintillation detectors is proposed. In the new method (G-INT), the energy of an event is calculated from the 'gated integral' of the pulse signal and the period of integration. The period of integration is not fixed but is shortened by the arrival of the succeeding pulse so as to avoid post-pulse pileup. The effect of pre-pulse pileup is corrected by subtracting the remnant energy of the preceding pulses, which is calculated from the gated integral of the preceding pulse. To avoid error due to short pulse intervals, pre- and post-pulse deadtimes are imposed. The method is similar to Wong's method (W-SUM) that depicts the energy by the 'weighted sum' of the current signal and the integrated signal. The performance of G-INT has been studied by Monte Carlo simulation in comparison with W-SUM, the variable sampling-time technique and simple delay-line clipping. It is shown that G-INT provides the smallest degradation in pulse height resolution for a given count rate capability. The difference between G-INT and W-SUM is explained by the difference in the amount of statistical noise involved in the gated integral and in the weighted sum. 相似文献

Some osteocytes released from their lacunae are embedded again in the bone and not engulfed by osteoclasts during bone remodeling 总被引:1,自引:0,他引:1
It is generally accepted that osteocytes derive from osteoblasts that have secreted the bone around themselves. Osteocytes are cells embedded in the lacunae in the bone, and they are characteristically in contact with other cells by many slender cytoplasmic processes in canaliculi. During bone remodeling, many osteocytes in the bone are released from their lacunae by osteoclasts; however it remains unclear what happens to these released osteocytes. The cortical bone of the rat mandibular body was used in this study. Mandibles were fixed, decalcified, and then embedded in Epon 812. Specimens were sectioned in the frontal direction into serial 0.5 microm-thick semithin or 0.1 microm-thick ultrathin sections, and then examined by light or transmission electron microscopy. Cells that fitted in the osteocytic lacunae with canaliculi extending to the bone were identified as osteocytes in this study. Among many osteocytes released by osteoclasts in cutting cones, there were osteocytes half-released from their lacunae. These cells fitted in their lacunae with canaliculi extending to the bone and showed developed cell organelles in the cytoplasm. In closing cones, many osteocytes were situated in the bone away from cement lines; however, there were half-embedded osteocytes in the bone formed on cement lines. These cells fitted in their lacunae with canaliculi extending to the bone formed below cement lines and showed developed cell organelles in the cytoplasm. These results show that half-embedded osteocytes in closing cones derive from half-released osteocytes in cutting cones. Osteocytes encircled by osteoclasts were sometimes observed on one section, but serial sections showed that these osteocytes fitted in their remaining lacunae in the bone on other sections. This shows that not all osteocytes released from their lacunae are engulfed by osteoclasts. Consequently, the present results suggests that some osteocytes released from their lacunae are embedded again in the bone and not engulfed by osteoclasts during bone remodeling. 相似文献

TT virus (TTV) is a newly discovered virus from a patient with post-transfusion hepatitis. We investigated the frequency
and pathogenesis of TTV infection in children. A semi-nested PCR assay was used to amplify TTV-DNA in serum samples from 254
ambulatory children without liver disease, 20 with hepatitis of unknown etiology, and 18 transfusion recipients or hemophiliacs.
In positive samples, TTV-DNA was quantified by real-time quantitative PCR using a fluorescent probe. We detected TTV-DNA in
20% of children with hepatitis of unknown etiology, which was not statistically different from the 23% prevalence in ambulatory
children. In transfusion recipients or hemophiliacs, the frequency was higher (50%) than that in ambulatory children (P = 0.01). Among ambulatory children, TTV-DNA was frequently detected in children with acute gastroenteritis (36%). TTV-DNA
was detected in 10% of the infants under 6 months old, and 20% of the children from 7 to 12 months old. The prevalence was
constant after the age of 1 year; however, the copy number of TTV-DNA was significantly higher in children under 1 year of
age (mean: 105.4 versus 103.8 copies/ml, P= 0.008). Finally, TTV-DNA was quantified serially in three children with chronic hepatitis who were positive for TTV-DNA.
The presence or amount of TTV-DNA was unrelated to the serum alanine aminotransferase level. These results indicate that TTV
infection is common in children. The larger quantity of TTV-DNA in infants and the high prevalence of TTV in children of all
ages suggest that TTV may be transmitted in early childhood. Its relationship to hepatitis is doubtful in children.
