Inpatient depression in persons with burns.

In this investigation the authors collected data regarding trait anxiety, well-being, and depression from 209 men and women who had been screened for prior psychiatric diagnosis and treated in an acute-care setting for burn injuries. Well-being was measured in reference to the month before the burn injury, whereas level of depression was self-rated by patients within 2 days of hospitalization, 5 days later, and 5 days after that. Ratings of depression were also obtained 1 month after hospital discharge. Results indicated that few patients rated their depression as severe at any point in time. Depression scores decreased significantly across the hospitalization period and were correlated with burn size, trait anxiety, and well-being. Depression ratings after discharge were significantly related to depression scores obtained at the end of the inpatient phase of the study. Although most patients did not report experiencing severe levels of depression, the stability of scores across time suggests the usefulness of early screening procedures. Catching such problems early may head off longer-term difficulties.     

Use of class II tetramers for identification of CD4+ T cells

Multivalent MHC class II molecules containing peptide antigens are useful tools for the detection of antigen specific human CD4+ T cells. Tetramers produced by exogenous peptide loading onto empty class II molecules are comparable to tetramers with peptide tethered to the class II chain covalently, but have many practical advantages. Conditions for optimal peptide loading to generate tetramers are discussed and optimal conditions of using tetramers for staining T cells are examined. As the frequency of antigen specific CD4+ T cells in peripheral blood is low, we demonstrate that an in vitro expansion step is effective in detecting low frequency T cells. Two new applications with tetramers, their uses for mapping T cell epitopes and for the detection of low affinity T cells are described. In a clinical setting, potential applications include using these reagents for monitoring disease progression during clinical intervention.     

VirtoScan-on-Rails – an automated 3D imaging system for fast post-mortem whole-body surface documentation at autopsy tables

Two-dimensional photographic documentation is a substantial part of post-mortem examinations for legal investigations. Additional three-dimensional surface documentation has been shown to assist in the visualization of findings and contribute to the reconstruction of the sequence of events. However, 2D photo documentation and, especially, 3D surface documentation, are time-consuming procedures that require specially trained personnel. In this study a 3D imaging system, called VirtoScan-on-Rails, was developed to automate and facilitate 3D surface documentation for photo documentation in autopsy suites. The imaging system was built to quickly acquire photogrammetric image sets of whole bodies during different stages of external and internal examinations. VirtoScan-on-Rails was set up in the autopsy suite of the Zurich Institute of Forensic Medicine at the University of Zurich (Zurich, Switzerland). The imaging system is based on a movable frame that carries a multi-camera array. Data quality and the applicability of the system were analyzed and evaluated within two test series. Up to 200 overlapping photographic images were acquired at consecutive image-capturing positions over a distance of approximately 2000 mm. The image-capturing process took 1 min and 23 s to acquire a set of 200 images for one side of the body. During test series one and two, 53 photogrammetric image sets taken from 31 forensic cases were successfully reconstructed. VirtoScan-on-Rails is an automated, fast and easy-to-use 3D imaging setup for autopsy suits. It facilitates documenting bodies during different stages of forensic examinations and allows standardizing the procedure of photo documentation.

Respite care for families of children with severe handicaps: An evaluation study of parent satisfaction

Respite care services provided by foster parents to 68 children with severe handicaps between 1976 and 1979 were systematically studied to determine parent satisfaction. A variety of rest options were made available to the parents of handicapped children for purposes of revitalizing themselves to deal with their child's daily needs. The respite options included short periods of time varying from 2 to 15 consecutive days, i. e., taking a much needed vacation, a week-edn or two a month off, a few days off during the week, or time away from the child due to an emergency. Consumer satisfaction was measured by survey questionnaires developed specifically for respite care and mailed to parents of handicapped children who had used the service. Results indicated that consumer opinion acquired via the follow-up questionnaire method was extremely positive, A most important consideration was to collect data on scheduled intervals about children subsequently receiving care from the same program to determine whether ratings increased or decreased. By focusing on specific respite care activities over time, it was possible to align optimum versus actual satisfaction. Through an analysis of observer variance, the modification of documented weaknesses can decrease the difference between optimum and actual consumer satisfaction levels.     

Community integration after burn injuries

Evaluation of community integration is a meaningful outcome criterion after major burn injury. The Community Integration Questionnaire (CIQ) was administered to 463 individuals with major burn injuries. The CIQ results in Total, Home Integration, Social Integration, and Productivity scores. The purposes of this study were to determine change in CIQ scores over time and what burn injury and demographic factors predict CIQ scores. The CIQ scores did not change significantly from 6 to 12 to 24 months postburn injury. Home integration scores were best predicted by sex and living situation; Social Integration scores by marital status; and Productivity scores by functional outcome, burn severity, age, and preburn work factors. The data demonstrate that individuals with burn injuries have significant difficulties with community integration due to burn and nonburn related factors. CIQ scores did not improve over time but improvement may have occurred before the initial 6-month postburn injury follow-up in this study.     

Paroxysmal non-kinesigenic dyskinesia caused by the mutation of MR-1 in a large Polish kindred

Paroxysmal non-kinesigenic dyskinesia (PNKD) is a clinical syndrome of sudden involuntary movements, mostly of dystonic type, which may be triggered by alcohol or coffee intake, stress and fatigue. The attacks of PNKD may consist of various combinations of dystonia, chorea, athetosis and balism. They can be partial and unilateral, but mostly the hyperkinetic movements are bilateral and generalized. We present a large Polish family with 7 symptomatic members of the family in 6 generations. In all affected persons, the onset of clinical symptoms was in early childhood. All male cases showed an increase in severity and frequency of the attacks with ageing, while the only living female patient noticed an improvement of PNKD during both her pregnancies and also after menopause. In addition, at the age of 55 years, she developed symptoms of Parkinson's disease with good response to levodopa treatment.
