A case of fetal death due to an amniotic band and an umbilical cord true knot is presented. In recent years, the need for a national burn center based on ABA guidelines has emerged in Israel. The formation of such a center is now underway in the Chaim Sheba Medical Center. As a first step in the standardization of burn care in Israel, we have conducted a nation-wide survey among burn care personnel (physicians, nurses and other burn team members), regarding different aspects of the treatment of burn patients. METHODS: A questionnaire comprised of 30 questions regarding the severity of burns admitted, the site of initial management, wound care (both burn/skin-graft sites and donor sites), dressing changes protocols, sterility precautions, hydrotherapy, and pressure dressings was presented to 70 health-care professionals involved in the treatment of burns. RESULTS AND DISCUSSION: Seventy-seven percent of interviewed personnel participated in the survey. Consensus was found regarding most local (topical) wound care, (SSD for clean non-facial burns, Sulfamylon (mafenide-acetate) for contaminated non-facial burns, Threolone (chloramphenicol 3% and prednisolone 0.5%) or Bacitracin for facial burns, Paraffin gauzes with or without Sulfamylon for donor and graft sites). Dressing changes regimes were also agreed upon generally. However, there was no consensus regarding the ideal time for the removal of donor site dressings and this issue will need to be resolved. Other important findings are that both Edinborough University Solution of Lime (EUSOL), which has been deemed unsuitable for burn treatment due to toxic effects, and hydrotherapy, which has been proposed as a source of infection and contamination, are still widely used. We examined whether protein kinases have a role in the expression of anaphylactic shock (AS). Guinea-pigs sensitized to ovalbumin were administered i.p. saline (control) or 10 micrograms/kg K252a, a potent protein kinase inhibitor, 30 min before challenge. The development of AS and mortality was observed for the next 2 h. In the K252a-treated group the incidence of AS fell to 53% from 100%, the maximum intensity was 62% less than the control, and mortality dropped to 16% from 50% of the animals. We suggest that protein kinases are involved in the expression of AS, and that inhibitors of these enzymes may protect against the symptoms of AS and allergy. 相似文献

Blocking conformational changes in biologically active proteins holds therapeutic promise. Inspired by the susceptibility of viral entry to inhibition by synthetic peptides that block the formation of helix–helix interactions in viral envelope proteins, we developed a computational approach for predicting interacting helices. Using this approach, which combines correlated mutations analysis and Fourier transform, we designed peptides that target gp96 and clusterin, 2 secreted chaperones known to shift between inactive and active conformations. In human blood mononuclear cells, the gp96-derived peptide inhibited the production of TNFα, IL-1β, IL-6, and IL-8 induced by endotoxin by >80%. When injected into mice, the peptide reduced circulating levels of endotoxin-induced TNFα, IL-6, and IFNγ by >50%. The clusterin-derived peptide arrested proliferation of several neoplastic cell lines, and significantly enhanced the cytostatic activity of taxol in vitro and in a xenograft model of lung cancer. Also, the predicted mode of action of the active peptides was experimentally verified. Both peptides bound to their parent proteins, and their biological activity was abolished in the presence of the peptides corresponding to the counterpart helices. Recent findings demonstrate strong links between abnormalities in circadian rhythms and sleep and the etiology, pathophysiology and treatment of major affective disorders. Further exploration of these interactions requires the development, identification and utilization of good and predictive animal models. The biology and behavior related to circadian rhythms are significantly different in diurnal and nocturnal rodents. Accordingly, it is possible that exploring the interactions between these mechanisms and affective change in diurnal animals may be advantageous. Recent studies demonstrate that diurnal fat sand rats and Nile grass rats show depression-like behavior when maintained under short-photoperiod (SP) conditions compared with animals maintained under neutral photoperiod (NP) conditions. Moreover, these behaviors were ameliorated after treatment with bright light. The present study further explores the possible utility of sand rats as animal models by testing the effects of antidepressants on the SP-induced depression-like behaviors of sand rats. Sand rats maintained in SP or NP conditions for 3 weeks were treated subchronically (5 injections) with the clinically effective antidepressant bupropion, and their behavior was tested in a number of depression-related tests. Results show that antidepressant treatment reverses the effects of SP conditions in the forced swim test, but that neither SP conditions nor antidepressants influenced sweet solution preference. These results partly support the validity of the sand rat model, but suggest that not all tests that were validated in nocturnal laboratory rodents can be applied to other rodent species and that additional tests should be applied to further explore the validity of the model. 相似文献

Four patients with endometrial foam cells are described. In one the endometrium was obtained after parturition. It showed decidua and endometritis, whereas in the second patient, who had received birth control medication in the past, the endometrium was proliferative. In both cases there was a considerable amount of endometrial hemorrhage. The foam cells in these two patients had little granularity, and their cytoplasm contained hemosiderin. The other two patients received exogenous estrogen and presented with endometrial hyperplasia and adenocarcinoma. In these latter cases the foam cells were more granular, and contained no hemosiderin. Our findings confirm the presence of two types of histologically similar endometrial foam cell: a histiocytic, "non-estrogenic" reactive type capable of phagocytosis, and a stromal, "estrogenic" non-phagocytic type which is associated with hyperplasia or carcinoma and which may also be related to estrogen administration. Staining for hemosiderin may be particularly helpful in differentiating between the two types of foam cell. Such a differentiation is of practical importance because of the biologic significance attached to the "estrogenic" foam-cells which are usually indicative of endometrial hyperplasia or carcinoma. 相似文献

Five furocoumarins were isolated by HPLC from the petroleum ether extract of shoots of PITURANTHOS TRIRADIATUS (H OCHST. ex B OISS.) A SCHERS. et S CHWEIF. (Umbelliferae). This species is commonly used by Beduins still practicing folk medicine in the Negev and Sinai Deserts. The furocoumarins were found to be: isoimperatorin, 5-(3-methyl-2-butenyloxy)-psoralen; cnidilin, 8-methoxy-5-(3-methyl-2-butenyloxy)-psoralen; imperatorin, 8-(3-methyl-2-butenyloxy)-psoralen; bergapten, 5-methoxypsoralen; and isopimpinellin, 5,8-dimethoxypsoralen. Some chemical and physical characteristics of these compounds, including RT values, were determined. 相似文献
