The overall 5-year relative survival rate for all cancers combined is now 68%, and there are over 16.9 million survivors in the United States. Evidence from laboratory and observational studies suggests that factors such as diet, physical activity, and obesity may affect risk for recurrence and overall survival after a cancer diagnosis. The purpose of this American Cancer Society guideline is to provide evidence-based, cancer-specific recommendations for anthropometric parameters, physical activity, diet, and alcohol intake for reducing recurrence and cancer-specific and overall mortality. The audiences for this guideline are health care providers caring for cancer survivors as well as cancer survivors and their families. The guideline is intended to serve as a resource for informing American Cancer Society programs, health policy, and the media. Sources of evidence that form the basis of this guideline are systematic literature reviews, meta-analyses, pooled analyses of cohort studies, and large randomized clinical trials published since 2012. Recommendations for nutrition and physical activity during cancer treatment, informed by current practice, large cancer care organizations, and reviews of other expert bodies, are also presented. To provide additional context for the guidelines, the authors also include information on the relationship between health-related behaviors and comorbidities, long-term sequelae and patient-reported outcomes, and health disparities, with attention to enabling survivors' ability to adhere to recommendations. Approaches to meet survivors' needs are addressed as well as clinical care coordination and resources for nutrition and physical activity counseling after a cancer diagnosis. 相似文献

Background: This retrospective study aimed to determine the prevalence of abnormal metabolic parameters in obese children and its correlation to the degree of obesity determined by body mass index (BMI). Methods: In total, 101 children seen at the Pediatric Gastroenterology Obesity Clinic at Stony Brook Children's University Hospital were enrolled in the study. The degree of obesity was characterized according to the following formula: (patient's BMI/BMI at 95th percentile) × 100%, with class I obesity >100%–120%, class II obesity >120%–140%, and class III obesity >140%. A set of metabolic parameters was evaluated in these patients. Frequency distributions of all study variables were examined using the χ2 test of independence. Mean differences among the obesity classes and continuous measures were examined using 1‐way analysis of variance. Results: Within our study population, we found that 80% of our obese children had a low high‐density lipoprotein (HDL) cholesterol level, 58% had elevated fasting insulin levels, and 32% had an elevated alanine aminotransferase (ALT) level. Class II obese children had a 2‐fold higher ALT value when compared with class I children (P = .036). Fasting insulin, ALT, HDL cholesterol, and triglyceride levels trended with class of obesity. Conclusion: Obese children in classes II and III are at higher risk for developing abnormal laboratory values. We recommend obese children be further classified to reflect the severity of the obesity since this has predictive significance for comorbidities. Obesity classes I, II, and III could help serve as a screening tool to help communicate risk assessment. 相似文献

Background
Teduglutide is a glucagon‐like peptide 2 (GLP‐2) analog that has been approved for the treatment of adult short bowel syndrome (SBS)–associated intestinal failure (IF; SBS‐IF). Teduglutide increases villus height and crypt depth in the small bowel mucosa, promoting nutrition absorption and enteral independence from parenteral nutrition (PN). We aim to report our single‐center experience with teduglutide in adult patients with SBS to provide real‐world context to its use.Method
We conducted a retrospective analysis on patients managed within our tertiary‐level intestinal rehabilitation program to identify patients with SBS‐IF treated with teduglutide from 2009–2015. The current report includes all patients at our center who had any exposure to teduglutide, including those who received commercial drug after approval by the Food and Drug Administration (FDA) and outside the scope of clinical trials.Results
A total of 18 patients were treated with teduglutide. Eleven patients (61%) achieved complete enteral independence from PN and/or intravenous fluids (IV) at a median time of 10 months (range: 3–36 months). PN/IV volume requirement was reduced in all patients except two. Ten of the 11 patients (91%) who achieved enteral autonomy had colon. All patients off PN/IV required additional oral vitamins and electrolyte supplementations.Conclusion
Our preliminary experience is consistent with prior reports of successful partial or complete weaning from PN/IV with teduglutide treatment in adult patients with SBS. The presence of colon appears to be favorable in obtaining enteral independence from PN/IV, regardless of residual small bowel length. Patients on teduglutide may remain at high risk of micronutrient deficiencies. 相似文献8.

Background: Earlier literature showed reduced efficacy of warfarin when co‐administered with enteral nutrition formulas through feeding tubes. This study used an in vitro model for gastric administration of warfarin through a feeding tube to evaluate potential causes for reduced warfarin absorption when administered through feeding tubes. Methods: There were 2 phases of the study. The first phase used an artificial stomach model with or without the infusion of enteral nutrition formula. Warfarin was added to the contents either directly into the vessel or passed through a feeding tube. Warfarin tablet dissolution was compared to the injectable formulation, which served as a control. The second phase used chopped feeding tube material added to beakers containing warfarin in increasing amounts. Results: Warfarin injection and tablet formulations showed decreased solubility when combined with acid. The warfarin solubility was higher when enteral formula was added. Warfarin concentration dropped by 35% when the drug was passed through a feeding tube, as opposed to added directly to the flask. In the second study, the warfarin levels were lower in the beakers containing feeding tubes. Doubling the amount of warfarin added did not raise levels to that of the initial dissolved. Doubling the amount of feeding tube material further reduced the concentration dissolved. Conclusions: Feeding‐tube administration compromises the total amount of warfarin reaching patients. It appears, from this in vitro study, that the mechanism of the interaction of warfarin may be a result of direct binding to the feeding tube. 相似文献
