Simultaneous Determination of Six Compounds in Rat Plasma by Ultra‑Performance Liquid Chromatography with Tandem Mass Spectrometry: Application in the Pharmacokinetic Study of Qing Gan‑Shu Yu‑Fang

A rapid and high selective ultra?performance liquid chromatography (UPLC) with tandem mass spectrometry method for simultaneous determination of six compounds including albiflorin, paeoniflorin, picroside I, picroside II, saikosaponin A, and saikosaponin D in rat plasma was developed and validated using butyl p-hydroxybenzoate as an internal standard. One-step direct protein precipitation with acetonitrile was used to extract the compounds from the rat plasma samples. Chromatographic separation was achieved using an ACQUITY UPLC BEH C18 column (100 mm × 2.1 mm, 1.7 µm) at a flow rate of 0.4 mL/min, using gradient mode containing 0.1% formic acid in water and acetonitrile were used as the Mobile phase A and B. Electrospray ionization in negative ion mode and multiple reaction monitoring were used to identify and quantify active components. Calibration curves showed good linearity (R2 > 0.9908) over a wide concentration range for all compounds. The intra- and interday precision (relative standard deviation) ranged 2.4%–7.0% and 2.6%–8.0%, respectively. The accuracy (relative error) was from ?13.0% to 13.2% at all quality control levels. The recovery ranged from 81.1% to 92.5%. The validated method was successfully applied to pharmacokinetic study in rats after oral administration of Qing Gan?Shu Yu?Fang. The results show that one can draw a conclusion that these six active ingredients can be quickly absorbed and play a pharmacodynamic role rapidly in vivo.     

穴位贴敷诱发的急、慢性接触性皮炎皮损特点及胸腺基质淋巴细胞生成素的表达差异性

目的观察穴位贴敷诱发的急性接触性皮炎与慢性接触性皮炎皮损特点以及皮损内胸腺基质淋巴细胞生成素(thymal stromal,tymphopoietin,TSLP)表达的差异性。方法选取三伏天接受穴位贴敷治疗哮喘且诱发急性或慢性接触性皮炎的患者20例,取皮分为急性接触性皮炎组(简称急性组)和慢性接触性皮炎组(简称慢性组),观察两组患者皮损特点及皮损组织中TSLP表达的差异性。结果急性组有大量的TSLP分布,弥漫性分布在表皮层(角质层、颗粒层、棘层和基底层)以及真皮层血管周围;慢性组TSLP数量较急性组少,集中分布在表皮层的角质层、棘层以及真皮层血管周围。结论 TSLP在穴位贴敷诱发的急性接触性皮炎与慢性接触性皮炎中的分布具有明显的数量差异性,而分布部位差异性不明显。推测皮炎性质(急性或慢性)会影响穴位贴敷疗效,而TSLP可以作为一个监测指标。

Abstract Objective To investigate how experienced surgeons in open surgery can quickly learn laparscopic resection for rectal cancer. Methods The clinical data of 120 cases of laparscopic resection for rectal cancer performed by the same team of surgeons from August 2009 to April 2015 in Changhai Hospital of the Second Military Medical University were analyzed retrospectively. The cases were divided into three groups (40 cases in each group) by operative sequence. The time span was 36, 19, 11 months respectively. The operating time??blood loss??the rate of conversion to open operation??number of lymph nodes??the integrityof mesorectum??postoperative aerofluxus time??postoperative hospital stay and the rate of postoperative complications were compared among the 3 groups. Results The operating time in group A was ??162.9±36.7??min??which as much higher than that in group B ??147.0±36.8??min and group C (132.7±31.9)min (P<0.05). However??there was no significant difference between group B and C (P>0.05). Intraoperative blood loss??the rate of conversion to open operation and postoperative hospital stay in group A had no significant difference compared with those in group B and C (P??0.05). Also??there was no significant difference in the integrity of mesorectum??number of lymph nodes??postoperative aerofluxus time and the rate of postoperative complications among 3 gourps ??P>0.05??. Conclusion From the learning curve??surgeons with rich experience in open surgery can learn laparscopic skills for rectal cancer after performing 40 cases.

护理干预对改善心内科住院患者睡眠质量的作用分析

目的：探讨护理干预在改善心内科住院患者睡眠质量方面的应用效果。方法：选取在我院接诊的65例心内科住院患者，其接受整体护理，设为观察组，回顾性分析早期在我院接诊的64例心内科住院患者的临床资料，其接受常规护理，设为对照组。通过调查问卷的方法了解两组心内科住院患者的护理满意度。结果：在接受不同护理情况下，观察组、对照组心内科住院患者的护理满意度分别为93．84％、78．12％，差异较大，有统计学意义。结论：护理干预能够有效改善心内科住院患者的睡眠质量，值得临床推广和应用。     

品质链服务在食管癌手术患者围手术期中的应用效果

目的探讨品质链服务在食管癌手术患者围手术期中的应用效果。方法选取本院收治的176例食管癌患者作为观察对象,按随机数字表法分为观察组(88例)和对照组(88例)。对照组患者给予常规方法,观察组行品质链服务治疗,对两组患者术后恢复情况、服务质量评分及并发症发生率进行比较。结果观察组的排气时间、住院时间、拔出引流管时间均短于对照组,胃肠减压引流量少于对照组,差异显著(P0.05);观察组对于康复指导、健康教育、入院接待等服务质量评分明显高于对照组,有统计学差异(P0.05);观察组并发症总发生率明显低于对照组(2.27%vs 6.82%),差异比较有统计学意义(P0.05)。结论对于食管癌手术患者围手术期中的患者采取品质链服务模式,能提升医疗服务满意度,促进患者术后恢复,临床疗效显著。
