BackgroundThe insertion of peroneus longus is traditionally described to the plantar surface of the 1st cuneiform and 1st metatarsal. It is thought to be the main contributor to the plantarflexed first ray seen in cavus feet.MethodsWe studied the insertion of peroneus longus in 26 feet from 14 adult cadavers. The insertional points, presence of sesamoid bone and variations in insertion were noted.ResultsThe main insertion was to the base of the 1st metatarsal and the medial cuneiform in the majority of feet but variations were observed. A sesamoid bone was present within the tendon under the cuboid in 16 feet, 12 of which had additional lateral insertion bands.ConclusionsVariations in the insertion of peroneus longus were found and we have described two new lateral bands.  相似文献

Primary squamous cell carcinoma of seminal vesicle is extremely rare, and most cases regarding seminal vesicle tumors failed to address this kind of tumor. A 54-year-old male patient presented with intermittent painless visual hematuria for 6 months was hospitalized. Ultrasonography, computerized tomography and magnetic resonance imaging demonstrated a 4.4 cm × 3.6 cm × 3.0 cm mixed tumorous lesion in the left seminal vesicle. A transrectal needle biopsy revealed severe chronic inflammation. The mass was completely resected in a laparoscopic approach and was verified as a moderately differentiated squamous cell carcinoma in the seminal vesicle by post-surgical histopathological examination. The patient received totally 5 cycles of chemotherapy. A rectal metastasis was detected 7 months after the surgery.  相似文献

Objective

To assess the current evidence regarding the efficiency, safety, and potential advantages of right-laparoscopic live donor nephrectomy (LLDN) compared with left-LLDN.

Materials and methods

We comprehensively searched PubMed, Embase, the Cochrane Library, and Web of Science to perform a systematic review and cumulative meta-analysis of a randomized controlled trial (RCT) and prospective and retrospective comparative studies assessing the two techniques.

Results

A single RCT and 28 comparative studies including a total of 32,426 cases were identified. Although left-LLDN was associated with lower delayed renal function [odds ratio (OR) 0.73; 95 % confidence interval (CI) 0.62, 0.87, p = 0.0003] and less thrombosis (OR 0.35; 95 % CI 0.13, 0.96, p = 0.04) in our meta-analysis, sensitivity analysis did not show any significant difference between the two groups. There were no other considerable differences between the two groups.

Conclusions

With enough surgical experience, right-LLDN can be performed with equivalent safety and efficacy. Although large-volume centers are routinely performing right-LLDN, there is under appreciation of the right side in smaller centers given the associated fear of poor outcomes and technically demanding procedure. Due to the inherent limitations of the included studies, future well-designed RCTs are awaited to confirm and update the findings of this analysis.  相似文献

Introduction

Decision-making regarding adjuvant chemotherapy has been based on clinical and pathological features. However, such decisions are seldom consistent. Web-based predictive models have been developed using data from cancer registries to help determine the need for adjuvant therapy. More recently, with the recognition of the heterogenous nature of breast cancer, genomic assays have been developed to aid in the therapeutic decision-making.

Methods

We have carried out a comprehensive literature review regarding online prognostication tools and genomic assays to assess whether online tools could be used as valid alternatives to genomic profiling in decision-making regarding adjuvant therapy in early breast cancer.

Results and conclusions

Breast cancer has been recently recognized as a heterogenous disease based on variations in molecular characteristics. Online tools are valuable in guiding adjuvant treatment, especially in resource constrained countries. However, in the era of personalized therapy, molecular profiling appears to be superior in predicting clinical outcome and guiding therapy.  相似文献

The designing of polypeptides with novel conduction properties is important because of the role of proteins in molecular electronics. Ant algorithm which is based on the cooperative interaction between artificial ants has proved to be an effective tool for such designing. It has been used in the present work in combination with simple negative factor counting and inverse iteration methods to study the effect of basis set, electron correlation and hydration on the electronic properties and hence the optimum compositions of polyglycine, polyalanine and polyserine in the most conducting protein chain. The results show clearly that a better basis set and the consideration of correlation decreases the band gap by about 1–3 eV. The optimum solution however remains unaffected. Further, using minimal basis set in the presence of water decreases the band gap by about 2.64 eV and changes the major component of the conducting protein from polyserine to polyalanine.  相似文献
