A Management of Early CRPS I Caused by Ankle Sprain: A Case Report

Abstract:   We present a case of a 13-year-old boy who developed signs and symptoms of neuropathic pain/early Complex Regional Pain Syndrome (CRPS) Type I, formerly known as Reflex Sympathetic Dystrophy (RSD), after spraining his ankle while wrestling. Aggressive pain control, using medications and sympatholytic blocks, with physical therapy and rehabilitation, led to the resolution of his painful condition. This prevented the disease from possibly progressing to a full-blown case of CRPS I (RSD) that is very challenging to treat.     

Update on transplant pharmacology: sirolimus.

Rapamune (Sirolimus), the latest immunosuppressant agent for solid organ transplants, is prescribed for induction therapy, refractory rejection, steroid withdrawal, and combination therapy. As the use of this agent increases among various transplant populations, it is essential for critical care nurses to be cognizant of the indications, pharmacodynamics, current research findings, side effects, and implications. This knowledge will ultimately improve patient education and outcomes in this ever-growing field of nursing.     

Der adipöse Patient im Rettungsdienst

The prevalence of obesity in Germany has risen steadily in the last years; about 50% of the German population are overweight. There are various methods for determining obesity, such as BMI, the pattern of fat distribution, and the waist to hip ratio. Overweight is associated with chronic illnesses such as arterial hypertension, pulmoarterial hypertension, cardiomyopathy with left and right heart insufficiency, diabetes mellits type 2, all of which lead to an increased risk of morbidity and mortality. In addition, psychiatric comorbidity is known to occur. The care of obese patients provides an increasing medical and technical challenge in emergency medicine. We discuss the particular difficulties of examining obese patients in the emergency room (problems examining certain areas and in diagnosis due to excessive fatty tissue), at monitoring, in determining venous or intraosseous entrances, in airway management, respiration and artificial respiration, and during rescue and transport, and solutions offered. The outcome is, as a matter of course, worse in obese patients than in those with normal weight.     

Beitrag zur Frage der pharmakologischen Wirkung einiger aliphatischer Alkyl- und Alkanolamine

Zusammenfassung In Blutdruckversuchen an der narkotisierten Katze sowie am isolierten Warmblüterherzen und am Gefäßpräparat wurden zahlreiche aliphatische Alkyl- und Alkanolamine geprüft.Es wurde gefunden, daß allen diesen Körpern gewisse Herz- und Kreislaufwirkungen zukommen, die Einführung von Alkanolresten von Bedeutung ist und besonders auch die Alkylsubstitution am N Wirkungsänderungen bedingt.Auf die Beziehungen zwischen chemischer Konstitution und pharmakologischer Wirkung in dieser Stoffklasse wurde hingewiesen, gewisse neue Gesetzmäßigkeiten gefunden und die praktische Bedeutung dieser Befunde für die Auswahl geeigneter Lösungsvermittler in Theophyllinpräparaten angedeutet.Mit 9 Textabbildungen (14 Einzelbildern).
