Because of evidence that topical phenytoin (PHT) can promote wound healing, we undertook pilot clinical trials to evaluate its effectiveness in treating decubitus ulcers that resulted from war-related wounds at Basrah Teaching Hospital, Basrah, Iraq. Prompt pain relief, decreased wound exudate and bacterial contamination, enhanced granulation tissue formation, and more rapid healing characterized the PHT group. Fifteen PHT-treated decubitus ulcer patients healed in 1-3 weeks (one graft) versus 6-8 weeks (five grafts) in the control group. In addition, 20 patients with missile wounds were treated with PHT in an open trial. Further trials of PHT in war-related wounds are indicated.  相似文献

BACKGROUND: Assessment of quality of life is vital inmonitoring response to various treatment measures. Various instruments, which include both generic and disease-specific instruments, are used in the assessment of health-related quality of life (HRQOL). In this study, we compare two commonly used generic instruments. OBJECTIVES: The objective of this study was to compare two generic instruments, the Karnofsky Performance Status Scale and the SF-36 Health Survey in hemodialysis (HD) patients. The study also aims to find out the association (if any) between HRQOL scores using these two scales and various clinical and biochemical parameters. MATERIALS AND METHODS: Sixty-two maintenance HD patients were recruited after informed consents were obtained. Detailed sociodemographic data was obtained. They were assessed during their regular HD sessions. Serum chemistry (which included serum urea, creatinine, Na+, K+, HCO3-, Ca2+, Po4(2-)), albumin, globulin, total protein and hemoglobin (g/dl) were assessed in all the patients. Adequacy of HD was assessed using second-generation Daugirdais formula. HRQOL was assessed using the Karnofsky and SSF-36 instruments and the scores collated and compared. Data was analyzed using SPSS version 10. RESULTS: Fifty-five patients completed the study (27 males and 28 females, mean age 40.76 +/- 11.05 years and age range of 20-65 years). There was a significant positive correlation between Karnofsky scores and all eight SF-36 domains, but only physical functioning, social functioning and role limitation due to emotional problems maintained the significance on multiple regression analysis. The serum creatinine and hemoglobin postively correlated with physical function, bodily pain, social functioning and Karnofsky scores. Age of the patients correlated negatively with two SF-36 dimensions (physical functioning and role limitation due to physical fitness) and Karnofsky scores. CONCLUSION: This study revealed a good correlation between Karnofsky performance status scale and the short-form (SF36) health survey in this Egyptian population. Age, serum creatinine and hemoglobin significantly influence quality of life in this HD patient population.  相似文献

Differences in age and number of children tested per family between two ethnic groups (Americans of Japanese ancestry and Americans of European ancestry) have a trivial effect on differences in regressions of midchild on midparent for tests of specific cognitive abilities. Although the regression of offspring on midparent value is not a mathematical function of the spouse correlation, an empirical association between these two measures of familial resemblance is expected for characters with nonzero heritability. Such an association is reported for anthropometric characters. Since age effects contribute to the covariance of family members, age correction of family cognitive data is essential.The results reported here are made possible by collaboration of a group of investigators (G. Laser in situ keratomileusis for myopic anisometropia in children.   总被引:7,自引:0,他引:7  
PURPOSE: To evaluate the visual and refractive results of laser in situ keratomileusis (LASIK) in pediatric patients with myopic anisometropia and amblyopia, and to assess the predictability, safety, and efficacy of the procedure in children. METHODS: Fourteen patients aged 7 to 12 years with myopic anisometropia and amblyopia had LASIK in the more myopic eye (14 eyes) using the Chiron Automatic Corneal Shaper and the Chiron-Technolas Keracor 116 excimer laser. Preoperative spherical equivalent manifest refraction ranged from -4.62 to -12.50 D (mean, -7.87 D) and spectacle-corrected visual acuity ranged from 20/40 to 20/100 (median, 20/50). All patients completed a minimum follow-up of 12 months. RESULTS: One year after LASIK, spherical equivalent manifest refraction ranged from 0 to -1.50 D (mean, -0.55 D). Spectacle-corrected visual acuity improved in all eyes (range, 20/20 to 20/40; median, 20/25). Six eyes (42.9%) had a postoperative spectacle-corrected visual acuity of 20/20. Uncorrected visual acuity was 20/40 or better in 10 eyes (71.4%) (range, 20/20 to 20/70; median, 20/30). Uncorrected visual acuity exceeded preoperative spectacle-corrected visual acuity in all eyes by an average of 2 lines. There were no significant complications. CONCLUSION: LASIK was effective for correction of myopic anisometropia in this small group of children and reversed refractive amblyopia. LASIK in these children was safe, predictable, and provided good visual results.  相似文献

Solid-state dye-sensitized solar cells (ss-DSSCs) comprising Sn2+-substituted ZnO nanopowder were purposefully tailored via a co-precipitation method. The solar cells assembled in this work were sensitized with N719 ruthenium dye and insinuated with spiro-OMeTAD as a solid hole transport layer (HTL). Evidently, significant enhancement in cell efficiency was accomplished with Sn2+ ions-substituted ZnO photoelectrodes by maintaining the weight ratio of SnO at 5%. The overall power conversion efficiency was improved from 3.0% for the cell with pure ZnO to 4.3% for the cell with 5% SnO substitution. The improvement in the cell efficiency with Sn2+-substituted ZnO photoelectrodes is attributed to the considerably large surface area of the nanopowders for dye adsorption, efficient charge separation and the suppression of charge recombination provided by SnO. Furthermore, the energy distinction between the conduction band edges of SnO and ZnO implied a type II band alignment. Moreover, the durability as well as the stability of 15 assembled cells were studied to show the outstanding long-term stability of the devices made of Sn2+ ion substituted ZnO, and the PCE of each cell remained stable and ∼96% of its primary value was retained for up to 100 h. Subsequently, the efficacy was drastically reduced to ∼35% after 250 h of storage.

Increased gametocytemia in infections with resistant strains of Plasmodium species and their enhanced transmissibility are a matter of concern in planning and evaluating the impact of malaria control strategies. Various studies have determined weekly gametocyte carriage in response to antimalarial drugs in clinical trials. The advent of molecular biology techniques makes it easy to detect and quantify gametocytes, the stages responsible for transmission, and to detect resistant genotypes of the parasite. With the validation of molecular markers of resistance to certain antimalarial drugs, there is a need to devise a simpler formula that could be used with these epidemiological antimalarial resistance tools. Theoretical models for transmission of resistant malaria parasites are difficult to deploy in epidemiological studies. Therefore, devising a simple formula that determines the potential resistant-genotype transmission of malaria parasites should provide further insights into understanding the spread of drug resistance. The present perspective discusses gametocytogenesis in the context of antimalarial treatment and drug resistance. It also highlights the difficulties in applying the available theoretical models of drug resistance transmission and suggests Rashad’s devised formula that could perhaps be used in determining potentially transmissible resistant genotypes as well as in mapping areas with high potential risk for the transmission of drug-resistant malaria. The suggested formula makes use of the data on gametocytes and resistant genotypes of malaria parasites, detected by molecular techniques in a certain geographical area within a particular point in time, to calculate the potential risk of resistant genotype transmission.  相似文献

The ultimate aim of malaria chemotherapy is not only to treat symptomatic infection but also to reduce transmission potential. With the absence of clinically proven vaccines, drug-mediated blocking of malaria transmission gains growing interest in the research agenda for malaria control and elimination. In addition to the limited arsenal of antimalarials available, the situation is further complicated by the fact that most commonly used antimalarials are being extensively resisted by the parasite and do not assist in blocking its transmission to vectors. Most antimalarials do not exhibit gametocytocidal and/ or sporontocidal activity against the sexual stages of Plasmodium falciparum but may even enhance gametocytogenesis and gametocyte transmissibility. Artemisinin derivatives and 8-aminoquinolines are useful transmission-blocking antimalarials whose optimal actions are on different stages of gametocytes. Transmission control interventions that include gametocytocides covering the spectrum of gametocyte development should be used to reduce and, if possible, stop transmission and infectivity of gametocytes to mosquitoes. Potent gametocytocidal drugs could also help deter the spread of antimalarial drug resistance. Novel proof-of-concept compounds with gametocytocidal activity, such as trioxaquines, synthetic endoperoxides, and spiroindolone, should be further tested for possible clinical utility before investigating the possibility of integrating them in transmission-reducing interventions. Strategic use of potent gametocytocides at appropriate timing with artemisinin-based combination therapies should be given attention, at least, in the short run. This review highlights the role that antimalarials could play in blocking gametocyte transmission and infectivity to mosquitoes and, hence, in reducing the potential of falciparum malaria transmissibility and drug resistance spread.  相似文献
