Background

There is a lack of biomarkers to predict outcome with targeted therapy in metastatic clear cell renal cancer (mccRCC). This may be because dynamic molecular changes occur with therapy.

Objective

To explore if dynamic, targeted-therapy-driven molecular changes correlate with mccRCC outcome.

Design, setting, and participants

Multiple frozen samples from primary tumours were taken from sunitinib-naïve (n = 22) and sunitinib-treated mccRCC patients (n = 23) for protein analysis. A cohort (n = 86) of paired, untreated and sunitinib/pazopanib-treated mccRCC samples was used for validation. Array comparative genomic hybridisation (CGH) analysis and RNA interference (RNAi) was used to support the findings.

Intervention

Three cycles of sunitinib 50 mg (4 wk on, 2 wk off).

Outcome measurements and statistical analysis

Reverse phase protein arrays (training set) and immunofluorescence automated quantitative analysis (validation set) assessed protein expression.

Results and limitations

Differential expression between sunitinib-naïve and treated samples was seen in 30 of 55 proteins (p < 0.05 for each). The proteins B-cell CLL/lymphoma 2 (BCL2), mutL homolog 1 (MLH1), carbonic anhydrase 9 (CA9), and mechanistic target of rapamycin (mTOR) (serine/threonine kinase) had both increased intratumoural variance and significant differential expression with therapy. The validation cohort confirmed increased CA9 expression with therapy. Multivariate analysis showed high CA9 expression after treatment was associated with longer survival (hazard ratio: 0.48; 95% confidence interval, 0.26–0.87; p = 0.02). Array CGH profiles revealed sunitinib was associated with significant CA9 region loss. RNAi CA9 silencing in two cell lines inhibited the antiproliferative effects of sunitinib. Shortcomings of the study include selection of a specific protein for analysis, and the specific time points at which the treated tissue was analysed.

Conclusions

CA9 levels increase with targeted therapy in mccRCC. Lower CA9 levels are associated with a poor prognosis and possible resistance, as indicated by the validation cohort.

Patient summary

Drug treatment of advanced kidney cancer alters molecular markers of treatment resistance. Measuring carbonic anhydrase 9 levels may be helpful in determining which patients benefit from therapy.  相似文献

Patients with type IV Ehlers-Danlos syndrome (EDS) have defective collagen synthesis, which places them at risk for spontaneous vascular rupture and other clinical sequelae. After treatment with resection and colostomy, a hepatoportal fistula developed in a 16-year-old boy with type IV EDS. The fistula was embolized angiographically by transcatheter coil occlusion. A highly specific cellular recognition system, capable of distinguishing between syngeneic and allogeneic tissue, exists in Anthopleura elegantissima, a sea anemone that lives in clonal colonies and attacks foreign clones. During the attack, specialized surface protrusions (acrorhagi) are used for stinging. The recognition process was studied by presenting various tissues to the surface of inflated acrorhagi and observing whether nematocyst discharge occurred. Nematocyte excitation required direct contact of th acrorhagus with foreign tissue and is presumably mediated by cell surface receptors. Most foreign anthozoans were excitatory, but intact syngeneic individuals, organisms other than anthozoans, and inanimate objects consistently failed to elicit discharge. When the intact surface of an excised tentacle from one anemone was presented to the acrorhagus of another, discharge occurred in 101 of 102 allogeneic combinations; more than 50 tests with tentacles from clone mates (i.e., syngeneic combinations) were all negative. No evidence for specific immunological memory was found. It is suggested that clonal recognition depends upon genetically determined chemical markers in the surface membrane of the epithelial cells; these are assumed to differ between clones although, in rare cases, allogeneic clones may have similar markers.  相似文献

Low socioeconomic status (SES) is associated with poor health outcomes in patients who have coronary heart disease (CHD). Inflammation is a potential mechanism by which low SES may lead to adverse cardiovascular outcomes, but it is not known whether low SES is associated with inflammation in patients who have CHD. We measured high-sensitivity C-reactive protein (CRP) levels in a cross-sectional study of 985 adults who had CHD. Income and education were determined by self-report. We used ordinal logistic regression to examine the association of income and education with CRP. Of the 985 participants, 390 had high CRP levels (>3 mg/dl). The proportion of participants who had high CRP levels ranged from 30% (103 of 340) in those who had a college degree to 51% (65 of 127) in those who had less than a high school degree (p<0.0001). The proportion of subjects who had a high CRP level ranged from 28% (52 of 183) in those who had annual income>or=$50,000 to 42% (199 of 974) in those who had an annual income<$20,000 (p<0.001). After adjustment for traditional cardiovascular risk factors and other potential confounding variables, lower income and education remained associated with higher CRP levels. In conclusion, low SES is associated with high CRP levels in patients who have CHD. This observation raises the possibility that inflammation may contribute to the adverse cardiovascular outcomes associated with low SES.  相似文献

Mature nematocysts of the sea anemones Rhodactis rhodostoma and Anthopleura elegantissima contain a fluid that has a high concentration of solutes and is extraordinarily rich in calcium (ca. 500-600 mmol/kg wet weight); this contrasts with the surrounding cytoplasm which is rich in potassium but poor in calcium. The undischarged capsule is surrounded by a membrane that probably acts as a selective permeability barrier between the cytoplasm and the nematocyst fluid. During discharge the nematocyst moves to the surface of the nematocyte and comes into contact with the external sea water medium. Calcium, which may be bound to proteins in the undischarged state, is rapidly lost from the fluid; at the same time, sea water enters the capsule. In vitro experiments have already shown that calcium loss increases the osmotic pressure of the capsular fluid, causing an influx of water from the external medium; this influx appears to increase the hydrostatic pressure inside the capsule to the point that the thread everts explosively.  相似文献
