Abstract

Purpose: Artemisinin (ART) has anti-inflammatory, antimicrobial, antioxidant, anti-amyloid, and anti-malarial effects, but its application is limited due to its low water solubility and poor oral bioavailability. In this study, the bioavailability, water solubility, and anti-plasmodial property of ART were improved by PCL–PEG–PCL tri-block copolymers.

Methods: The structure of the copolymers was characterized by 1H NMR, FT-IR, DSC, and GPC techniques. ART was encapsulated within micelles by a single-step nano-precipitation method, leading to the formation of ART-loaded PCL–PEG–PCL micelles. The obtained micelles were characterized by dynamic light scattering (DLS) and atomic force microscopy (AFM). The in vivo anti-plasmodial activity of ART-loaded micelles was measured against Plasmodium berghei infected Swiss albino mice.

Results: The results showed that the zeta potential of ART-loaded micelles was about ?8.37?mV and the average size was 91.87?nm. ART was encapsulated into PCL–PEG–PCL micelles with a loading capacity of 19.33?±?0.015% and encapsulation efficacy of 87.21?±?3.32%. In vivo anti-plasmodial results against P. berghei showed that multiple injections of ART-loaded micelles could prolong the circulation time and increase the therapeutic efficacy of ART.

Conclusion: These results suggested that PCL–PEG–PCL micelles would be a potential carrier for ART for the treatment of malaria.  相似文献

Objective

Ovarian hyperstimulation syndrome (OHSS) is a major complication of assisted reproductive technologies (ART). Polycystic ovary syndrome (PCOS) is a risk factor for OHSS. The aim of this randomized clinical trial (RCT) was to study the effect of low-dose aspirin (LDA) on the development of OHSS and ART outcomes in PCOS during ART.

Materials and methods

This double-blinded placebo controlled RCT was performed on 232 PCOS infertile women in their first ART cycles during 2010–2016. LDA and placebo capsules were prepared, packed and specified by code numbers in similar shapes. One package was given to every woman and asked to take one capsule/day since the 21st day of her cycle prior to the gonadotropin stimulation. Gonadotropin releasing hormone agonist long protocol and triggering by human chorionic gonadotropin were used. Development of moderate to severe OHSS and their ART outcomes were documented then the codes were broken and data analyzed. Chi-square and Mann–Whitney U tests were used for the statistical analyses.

Results

Eighteen cases that did not follow the study design were excluded. 214 cycles remained for the final analyses with 109 cases in LDA and 105 in the placebo group. Rate of the moderate to severe OHSS in LDA group was 34.9% compared to 30.5% in placebo group (P = 0.494). Fertilization rate was 71.8% vs 65.1% (P = <0.001) and the mean number of grade III embryos were 3.28 ± 3.53 vs 1.46 ± 1.42 (P = 0.014) in LDA and placebo groups, respectively. The mean number of the oocytes in different grades, total and frozen embryos also implantation and clinical pregnancy rates were not different between the groups.

Conclusion

Moderate to Severe OHSS was not decreased but fertilization rate and the mean number of poor quality embryos were increased in LDA arm.

We present four cases of cystosarcoma phyllodes in which mammograms revealed a small or large lobulated mass occupying the entire breast. Preoperative diagnosis was a benign lesion with a low probability of nodular carcinoma. Histologically, all tumors were classified as malignant. Although uncommon, cystosarcoma should be considered in the differential diagnosis of these masses, since local recurrence is frequent unless wide wedge resection or simple mastectomy is done.  相似文献

Hepatic dearterialization by ligation of the main hepatic artery followed by local intraarterial chemotherapy was performed in 24 patients with clinically isolated but unresectable hepatic metastasis after local resection of carcinoma of the colon. Computed tomography of the abdomen and laboratory parameters were used both to stage and follow-up these patients. Computed tomography changes following dearterialization and chemotherapy of the liver are described. Computed tomography is a valuable technique for both staging and follow-up this group of patients.  相似文献

Background

View into Ventricular Onset (VIVO) is a novel ECGI system that uses 3D body surface imaging, myocardial CT/MRI, and 12?lead ECG to localize earliest ventricular activation through analysis of simulated and clinical vector cardiograms.

Objective

To evaluate the accuracy of VIVO for the localization of ventricular arrhythmias (VA).

Methods

In twenty patients presenting for catheter ablation of VT [8] or PVC [12], VIVO was used to predict the site earliest activation using 12?lead ECG of the VA. Results were compared to invasive electroanatomic mapping (EAM).

Results

A total of 22 PVC/VT morphologies were analyzed using VIVO. VIVO accurately predicted the location of the VA in 11/13 PVC cases and 8/9 VT cases. VIVO correctly predicted right vs left ventricular foci in 20/22 cases.

Conclusion

View into Ventricular Onset (VIVO) can accurately predict earliest activation of VA, which could aid in catheter ablation, and should be studied further.  相似文献
