The jawbones can be sites of various neoplastic conditions. Given the variety of processes affecting this particular anatomical area, formulation of a precise diagnosis can be challenging to clinicians. Limited jaw movement, pain, and facial asymmetry are common signs among patients, especially those with temporomandibular disorder (TMD). This paper reports a case of primary fibrosarcoma affecting the mandible and surrounding structures in a 14-year-old girl presenting signs and symptoms similar to TMD. Her condition was misdiagnosed, and she was treated for TMD over an extended period before the correct diagnosis was made for fibrosarcoma. The patient underwent surgical resection with postsurgical radiotherapy and chemotherapy and now is being followed up. Although malignant lesions are rare in the temporomandibular joint (TMJ) region, dentists are advised to be aware of the condition and to keep in mind that patients who are admitting for TMD can also possibly be affected from neoplasms. To evaluate the effect of artery-only (AO) and artery–vein (AV) clamping during partial nephrectomy (PN) on short- and long-term renal function outcome.
Methods
Medical records of 154 patients in the AO group and 192 patients in the AV group who underwent open and minimally invasive (laparoscopic/robotic) PN between January 2011 and January 2018 were retrospectively assessed. Preoperative patient and tumor-specific characteristics in addition to perioperative factors and renal function outcomes were compared. The change in the estimated glomerular filtration rate (eGFR) from postoperative 1–3 days, 12 and 24 months after surgery was calculated. Acute kidney injury (AKI) was defined a as a?>?25% reduction in eGFR.
Results
There were no statistically significant differences between the clamping techniques in terms of postoperative 1–3 days, 12 and 24 months eGFR change percentage and risk of progression to chronic kidney disease (CKD). No significant difference in short- and long-term renal functions was found between the minimally invasive or open AO and AV clamping subgroups at any time point. In multivariate analysis, the R.E.N.A.L score (AO group p?=?0.026, AV group p?<?0.001) and preoperative eGFR (AO group p?<?0.001, AV group p?=?0.010) were strong predictors of the acute kidney injury in both groups. Older age (AO group p?=?0.045, AV group p?=?0.010) and preoperative eGFR (AO group p?=?0.008, AV group p?=?0.002) were significantly associated with CKD progression at 2-year follow-up in both groups.
Conclusion
AV clamping does not adversely affect postoperative renal function compared to AO clamping. Preoperative patient- and tumor-related factors are more important for renal function regardless of the clamping technique.
Three clusters of Chryseobacterium meningosepticum infections in a tertiary health center in July 2006 and January 2007 involving 8 newborns and 5 older children were investigated. The index patient was from the neonatal intensive care unit, and the older patients were from other pediatric wards. Cultures were obtained from the environment and from health care workers' hands as part of an outbreak investigation. C meningosepticum was isolated from hand cultures obtained from a senior resident and from environmental cultures obtained from powdered infant formula, an electrical button, a computer keyboard, phone, a doorknob, and an Ambu bag. Antibiogram typing and enterobacterial repetitive intergenic consensus sequence polymerase chain reaction indicated that all of the isolates were epidemiologically related. Nine patients improved on antimicrobial treatment, and 4 premature infants died after the infection. C meningosepticum is a well-known etiologic agent for nosocomial infections involving newborns and immunocompromised patients. Wet and dry environmental surfaces and equipment may act as a source or play a role in disseminating the microorganism. Outbreaks may be controlled with strong emphasis on infection control measures. 相似文献

Production of carbon fibers (CF) using renewable precursors has gained importance particularly in the last decade to reduce the dependency on conventional petroleum-based precursors. However, pre-treatment of these renewable precursors is still similar to that of conventional ones. Little work is put into greener pre-treatments and their effects on the end products. This work focuses on the use of bio-cleaned lignin as a green precursor to produce CF by electrospinning. Bio-cleaned kraft lignin A (Bio-KLA) and uncleaned kraft lignin A (KLA) were used to explore the effect of bio-cleaning on the diameter and mechanical properties of lignin fibers and CF. The effect of electric field, lignin-to-poly(ethylene oxide) (PEO) ratio and PEO molecular weight (MW) were evaluated by 33 factorial design using Design of Experiment (DOE). The electrospinning process parameters were optimized to obtain a balance between high elastic modulus and small fiber diameter. The model predicted optimized conditions were 50 kV m−1 electric field, 95/5 lignin-to-PEO ratio and 1000 kDa MW of PEO. When compared to KLA, Bio-KLA CFs showed a 2.7-fold increase in elastic modulus, 2-fold increase in tensile strength and 30% decrease in fiber diameter under the same optimum conditions. The results clearly show that bio-cleaning improved the mechanical properties of lignin derived CF.Waste lignin (KLA) and bio-cleaned lignin (Bio-KLA) precursors, used to produce parameter-optimized-electrospun carbon fibers showed improved mechanical properties for Bio-KLA.相似文献
