Objective
To evaluate mastication in a group of patients with adolescent idiopathic scoliosis (AIS) with a control group, by means of the prevalence of reverse chewing cycles (RCCs).Material and Methods
This study included a group of patients (N = 32; F = 24; M = 8; mean age ± SD = 14 ± 3 years) with a confirmed diagnosis of AIS and a group of control subjects (N = 32; F = 24; M = 8; mean age ± SD = 13 ± 6 years) without spinal disorders. Mastication was recorded with both a hard and a soft bolus, following a standardized protocol, and the prevalence of RCCs was compared between the groups.Results
The prevalence of RCCs was significantly higher in the AIS group, with both a soft and a hard bolus, compared to the control group (P < .001).Conclusion
The results of this study indicate that the presence of AIS influences mastication, one of the main functions of the stomatognathic system. A multidisciplinary approach to these patients may be relevant in providing the best possible treatment outcomes. 相似文献3.

Background
Pathological deformities involving the sagittal alignment of the spine may lead to loss of spine stability and imbalance. The effect of different patterns of sagittal balance on the loads acting in the spine was only marginally investigated, although it would be of critical importance in the clinical management of spinal disorders.Methods
Optimization-based finite element models of the human spine in the standing position able to predict the loads acting in the lumbar spine and the activation of the spinal muscles were developed and used to explore a wide range of sagittal balance conditions, covering both inter-subject variability and pathological imbalance. 1000 two-dimensional randomized spine models with simplified geometry were generated by varying anatomical parameters such as lumbar lordosis, sacral slope, and C7 plumb line. Muscular loads were calculated by means of an optimization procedure aimed to minimize total muscular stress.Findings
The simulation of a physiological spine in the standing position predicted average disk stresses ranging from 0.38 to 0.5 MPa, in good agreement with in vivo measurements. The C7 plumb line and the parameters describing the lumbar spine were found to be the strongest determinants of the lumbar loads and muscle activity. Marginal relevance was found concerning the thoracic and cervical parameters.Interpretation
The present modeling approach was found to be able to capture correlations between sagittal parameters and the loads acting in the lumbar spine. The method represents a good platform for future improvements aimed at patient-specific modeling to support pre-operative surgical planning. 相似文献6.
Percutaneous approaches to lumbar discectomy were somewhat controversial, because of their limited indications. They have not proven to be as effective as standard open lumbar disc surgery, because of longer operating times and some technical problems in addressing all the different aspects of lumbar disc herniations. New percutaneous posterior procedures for lumbar disc disease have been described in the last few years, but the MicroEndoscopic Discectomy (MED) introduced by Foley and Smith in 1997 seems to be the most promising one. MED, and METR’x which evolved from it, . allow the surgeon to address not only contained lumbar disc herniations, but also free-fragment disc pathology and symptomatic lateral recess stenosis secondary to bony hypertrophy. The surgical technique is summarized and some preliminary clinical results of a prospective multicenter study with 13 months’ mean follow-up are presented. 相似文献

The surgical devices for the treatment of degenerative disc disease are based on different concepts (rods for spine fusion, ROM-restricting or load-bearing devices for dynamic stabilization). In the present work, the effects of some stabilization systems on the biomechanics of the lumbar spine were investigated by means of a finite element model of the L2-L5 spine segment. Pedicular screws and stabilization devices were added at L4-L5. Different rods were considered: stainless steel, titanium, PEEK and the composite ostaPek. Two pedicular devices aimed at motion preservation were also considered: the FlexPLUS and the DSS. All models were loaded by using the hybrid protocol in flexion, extension, lateral bending and axial rotation. The spine biomechanics after implantation resulted significantly sensitive to the design and the materials of the device. The impact of all rods in reducing the ROM was found to be critical (>70% in flexion and extension). The dynamic devices were able to preserve the motion of the segment, but with different performances (ROM reduction from 30% (DSS) to 50% (FlexPLUS)). The shared load was more sensitive to the elastic modulus of the device material than the calculated ROMs (from 7% (PEEK) to 48% (stainless steel)). Regarding devices aimed at motion preservation, the authors suggest to distinguish "flexible" devices, which are able to preserve only a minor fraction (e.g. at most 50%) of the physiological ROM, from "dynamic" devices, which induce a smaller ROM restriction. However, the optimal characteristics of a stabilization device for the treatment of degenerative disc disease still need to be determined by means of basic science and clinical studies. 相似文献

The implantation of lumbar disc prostheses based on different design concepts is widely accepted. This paper reviews currently
available literature studies on the biomechanics of TDA in the lumbar spine, and is targeted at the evaluation of possible
relationships between the aims of TDA and the geometrical, mechanical and material properties of the various available disc
prostheses. Both theoretical and experimental studies were analyzed, by a PUBMED search (performed in February 2007, revised
in January 2008), focusing on single level TDA. Both semi-constrained and unconstrained lumbar discs seem to be able to restore
nearly physiological IAR locations and ROM values. However, both increased and decreased ROM was stated in some papers, unrelated
to the clinical outcome. Segmental lordosis alterations after TDA were reported in most cases, for both constrained and unconstrained
disc prostheses. An increase in the load through the facet joints was documented, for both semi-constrained and unconstrained
artificial discs, but with some contrasting results. Semi-constrained devices may be able to share a greater part of the load,
thus protecting the surrounding biological structure from overloading and possible early degeneration, but may be more susceptible
to wear. The next level of development will be the biomechanical integration of compression across the motion segment. All
these findings need to be supported by long-term clinical outcome studies. 相似文献

Existing studies on micro-endoscopic lumbar discectomy report similar outcomes to those of open and microdiscectomy and conflicting
results on complications. We designed a randomised controlled trial to investigate the hypothesis of different outcomes and
complications obtainable with the three techniques. 240 patients aged 18–65 years affected by posterior lumbar disc herniation
and symptoms lasting over 6 weeks of conservative management were randomised to micro-endoscopic (group 1), micro (group 2)
or open (group 3) discectomy. Exclusion criteria were less than 6 weeks of pain duration, cauda equina compromise, foraminal
or extra-foraminal herniations, spinal stenosis, malignancy, previous spinal surgery, spinal deformity, concurrent infection
and rheumatic disease. Surgery and follow-up were made at a single Institution. A biomedical researcher independently collected
and reviewed the data. ODI, back and leg VAS and SF-36 were the outcome measures used preoperatively, postoperatively and
at 6-, 12- and 24-month follow-up. 212/240 (91%) patients completed the 24-month follow-up period. VAS back and leg, ODI and
SF36 scores showed clinically and statistically significant improvements within groups without significant difference among
groups throughout follow-up. Dural tears, root injuries and recurrent herniations were significantly more common in group
1. Wound infections were similar in group 2 and 3, but did not affect patients in group 1. Overall costs were significantly
higher in group 1 and lower in group 3. In conclusion, outcome measures are equivalent 2 years following lumbar discectomy
with micro-endoscopy, microscopy or open technique, but severe complications are more likely and costs higher with micro-endoscopy. 相似文献
