We report the management of two children and 11 adults with paraplegia secondary to vertebral hydatidosis. Destruction of pedicles, posterior vertebral elements and discs as well as the vertebral bodies was common and all six patients with thoracic disease had involvement of adjacent ribs. The 13 patients had a total of 42 major surgical procedures; two patients died from postoperative complications and four from complications of the disease and paraplegia. All eight patients initially treated by laminectomy or anterior decompression alone relapsed within two years and seven required further surgery. Circumferential decompression and grafting gave the best results, six of nine patients being in remission an average of three years and six months later. The prognosis for such patients is poor; remission is the aim, rather than cure. Anthelminthic drugs may improve the prognosis, but radical surgery is likely to remain the keystone of treatment in the foreseeable future. 相似文献
Purpose: The purpose of this study was to evaluate the effects of 6 months of specific balance training included in endurance-resistance program on postural balance in haemodialysis (HD) patients.Methods: Forty-nine male patients undergoing HD were randomly assigned to an intervention group (balance training included in an endurance-resistance training, n?=?26) or a control group (resistance-endurance training only, n?=?23). Postural control was assessed using six clinical tests; Timed Up and Go test, Tinetti Mobility Test, Berg Balance Scale, Unipodal Stance test, Mini-Balance Evaluation Systems Test and Activities Balance Confidence scale.Results: All balance measures increased significantly after the period of rehabilitation training in the intervention group. Only the Timed Up and Go, Berg Balance Scale, Mini-Balance Evaluation Systems Test and Activities Balance Confidence scores were improved in the control group. The ranges of change in these tests were greater in the balance training group.Conclusions: In HD patients, specific balance training included in a usual endurance-resistance training program improves static and dynamic balance better than endurance-resistance training only.

Our objective was to investigate the prevalence and the environmental determinants of alcohol use among students in the region of Sfax in Tunisia. We carried out a cross-sectional study among 315 middle and high school students. We used the Alcohol Use Disorders Identification Test (AUDIT) to identify risky alcohol consumption, and we used the Parenting Styles and Dimensions Questionnaire (PSDQ) to assess the students’ perceptions of their parents’ parenting styles. The results show that 19.7% reported drinking alcohol at least once in their lifetime. Among them, 21% scored as dependent alcohol users, according to the AUDIT. Those who drank alcohol at least once were more likely to have parents with a permissive parenting style (p < 0.001; Cramer’s V = 0.287), and a father (p < 0.001; Cramer’s V = 0.258), a mother (p = 0.025; Cramer’s V = 0.158), or a friend (p < 0.001; Cramer’s V = 0.341) who drinks. Students perceiving their parents’ parenting style as permissive had the highest AUDIT score (p = 0.005; partial η2 = 0.132). The authoritarian style score was significantly higher for students who were current alcohol users (p = 0.028; Cohen’s d = 0.57). Our study highlights the influence of peers, family drinking, and parenting styles on alcohol use among middle and high school students. Therefore, particular attention should be given to students that are at risk of having the abovementioned environmental determinants of alcohol use. And, prevention strategies should involve parents, as well as enhanced guidance and counseling for these students.
BackgroundAngiotensin‐converting enzyme (ACE) plays a pivotal role in several pathologies including cancers. The association of insertion/deletion (I/D) polymorphism of the ACE gene with prostate cancer (PC) risk remains controversial. We aimed to investigate for the first time, to our Knowledge, in North Africa the potential relationship between ACE I/D polymorphism with PC susceptibility and clinical outcomes of PC patients.MethodsThis case‐control study included 143 healthy individuals and 124 patients diagnosed with PC. Using genomic DNA, the samples were genotyped for ACE I/D polymorphism by polymerase chain reaction (PCR).ResultsWe found that The D allele is significantly associated with an increased risk of PC and D/D + D/I genotypes were at 3 times increased risk of PC ([p = 0.005], OR = 2.95, IC 95% = 1.26–7.09) compared with I/I genotype (p = 0.003, OR = 0.3, IC 95% = 0.12–0.74). We observed an association between D/D and D/I genotypes with advanced age (≥70 years) (p = 0.014; r2 = 0.22). Furthermore, there is a significant prediction of advanced Gleason score ≥8 based on epidemiological parameters and ACE genotype (p = 0.000; R2 = 0.349), although no significant association was observed with stage and metastasis.ConclusionThe ACE I/D polymorphism is likely to predispose to PC and could play a role in PC progression and aggressiveness. 相似文献
Dans le Sahel, entre la Mauritanie et la Somalie incluant le Nord Kenya, environ 20 à 30 millions de personnes vivent en transhumance. Le rythme de leur migration suit l’évolution saisonnière du climat et la disponibilité des ressources, telle que l'eau, le pâturage et le sel. Malgré une exposition élevée à certaines maladies comme les zoonoses et les problèmes conditionnés liés au climat, les pasteurs mobiles sont parmi les populations quasiment exclues du système de santé, car la mise à disposition des services sociaux adaptés à un mode de vie mobile est difficile. Suivant l'objectif de recherche d'un meilleur accès aux soins des pasteurs mobiles, l'Institut Tropical et de Santé Publique Suisse, en partenariat avec plusieurs institutions dans la région, est actif au Sahel depuis 15 ans, aussi bien dans le domaine de la recherche, que celui des actions de développement. Basées sur une approche orientée vers les besoins des pasteurs mobiles pour leur développement, des recherches interdisciplinaires ont contribué à mieux comprendre la situation et les problèmes des éleveurs. En relation de la proximité entre l'homme et son bétail, une approche unissant la santé humaine et animale s'est avérée bonne et la valeur ajoutée d'une meilleure collaboration entre médecine humaine, animale et l'environnement a été démontrée. The present study investigated the effect of biochar (BC) addition on mechanical, thermal, and water resistance properties of PLA and hemp-PLA-based composites. BC was combined with variable concentration to PLA (5 wt%, 10 wt%, and 20 wt%) and hemp (30 wt%)-PLA (5 wt% and 10 wt%); then, composites were blended and injection molded. Samples were characterized by color measurements, tensile tests, thermogravimetric analysis (TGA), differential scanning calorimetry (DSC), and water contact angle analysis. Experimental results showed that adding 5 wt% of BC enhanced the composite’s tensile modulus of elasticity and strength. Hence, the use of optimized loading of BC improved the mechanical strength of the composites. However, after BC addition, thermal stability slightly decreased compared with that of neat PLA due to the catalytic effect of BC particles. Moreover, the water-repelling ability decreased as BC content increased due to the specific hydrophilic characteristics of the BC used and its great porosity. 相似文献
