House staff manuals: a survey of pediatric residency programs

To determine the role of house staff manuals in residency training, we did a survey of the pediatric residency directors in the United States. Questionnaires were returned from 77% of the programs (179/232). Sixty percent of respondents provided written administrative and medical guidelines to their house staff. Neonatal or pediatric intensive care and emergency protocol information were presented most frequently. Consistent patterns of content and organization were apparent in the 77 manuals reviewed, though style and size varied widely. We conclude that written departmental guidelines are a common and potentially useful educational resource. Evaluation of the effectiveness of manuals to solve patient care or administrative problems is necessary for future refinements in content and format.     

Inherited deficiency of the third component of human complement (C'3)

A kindred has been investigated in which seven individuals were found to have half-normal serum concentrations of the third component of complement (C'3). This partial deficiency was transmitted as an autosomal dominant trait. Affected individuals were entirely healthy. Hemolytic complement titers were slightly reduced but immune adherence titers and reagent titrations of the classical complement components were normal.Examination for C'3 allotypes revealed that all affected individuals had patterns resembling those of homozygotes. Analysis of the inheritance of C'3 structural genes disclosed that the most likely mechanism for partial C'3 deficiency in this family was nonexpression of one allele.     

Risikokommunikation

Menopausal hormone therapy (HT) is one of the most frequently prescribed pharmacotherapies. The 2009 published interdisciplinary guidelines “Hormone therapy in peri- and postmenopause (HT)” summarize the best evidence available and consider all indications and contraindications. The undue fear and confusion generated in recent years by overextrapolation or misinterpretation of clinical studies mandate a clearer explanation of the therapeutic benefit:risk ratio of HT for women at various times through and beyond menopause. Appropriate risk communication needs to be considered in the shared decision-making process of doctor and patient.
