This paper is a comprehensive review on the effect of bandaging, bracing, and neoprene sleeves on knee proprioception following anterior cruciate ligament (ACL) injury and reconstruction with a focus on studies that have measured joint position sense and threshold to detection of passive knee motion. Disruption of the ACL does not appear to alter joint position sense soon after injury, although there is evidence that in some subjects deterioration may occur over time. An ACL tear creates a deficit in the threshold to detection of passive knee motion soon after injury and in those with chronic tears. The magnitude of worsening is less then 1.0 degree of movement in flexion-extension and of questionable concern from a clinical and functional perspective. Application of a functional brace or neoprene sleeve to the ACL-deficient limb does not improve the threshold to detection of passive knee motion; however, application of an elastic bandage to a knee with an ACL tear improves joint position sense. Reconstruction of a torn ACL is associated with a deficit in the threshold to detection of passive knee motion, and during the first year of healing the use of a neoprene sleeve provides improvement. Two years following ACL reconstruction there is no deficit in the threshold to detection of passive knee motion and the use of a brace has no effect on this outcome. 相似文献

Placement of transpedicular vertebral screws close to anterior vertebral cortex. Description of methods 总被引:4,自引:0,他引:4
Strengthening of the screw-vertebra interface has been shown to occur with implantation of longer transpedicular screws, the tips of which are placed closer to the anterior cortex of the vertebral body. Such implantation probably results in increased risk for anterior cortex penetration and associated vascular or pulmonary damage. Typically recommended lateral and posterior-anterior radiography is shown here to provide potentially misleading visualization during implantation. To reduce the risk of anterior cortical penetration, presented here are 1) a radiographic method ("near approach view") that avoids this problem and allows direct visualization of the relationship between drill bit or screw tip and anterior vertebral cortex, and 2) a surgical technique ("mallet method") that provides both an audible and a palpable change when the drill bit contacts the anterior cortex. 相似文献

We studied the effect that chronic anterior cruciate ligament disruption, functional bracing, and a neoprene sleeve have on knee proprioception by measuring the threshold to detection of passive knee motion in all three conditions. The threshold to detection of passive knee motion was worse in knees with chronic anterior cruciate ligament insufficiency when compared with uninjured knees. This difference was small, on average an additional 0.28 degree of flexion-extension rotation was required for the anterior cruciate ligament-deficient knee before the subject detected motion, and of questionable significance from a clinical and functional perspective. Wearing a functional brace or neoprene sleeve on the anterior cruciate ligament-deficient knee did not significantly change the threshold to detection of passive motion in comparison with the same knee without a brace, although improvements were observed. There was no relationship between the most common clinical means of characterizing altered biomechanics of the anterior cruciate ligament-deficient knee (that is, the magnitude of anterior-posterior knee laxity and the grade of pivot shift) and the threshold to detection of passive knee motion. 相似文献

Tracheobronchopathia osteochondroplastica (TO) is a rare benign disease characterized by the presence of osseous and cartilaginous submucosal nodules projecting into the tracheobronchial tree. Most cases are asymptomatic and discovered incidentally at post‐mortem. We identified a case of TO on thoracic spiral CT and confirmed the diagnosis on bronchoscopy. This article reviews the imaging characteristics of TO, and shows the 3‐D virtual bronchoscopic and multiplanar reconstruction appearances of TO. 相似文献

The present study compared the accuracy of ventilation perfusion scintigraphy (VQS) and CT pulmonary angiography (CTPA) for the diagnosis of pulmonary embolism. This was a prospective observational study of 112 patients with suspected pulmonary embolism (PE) who could be studied with both investigations within 24 h. Results were compared to final diagnosis at completion of 6-month follow up, using receiver operating characteristic (ROC) analysis. Pulmonary embolism was diagnosed in 27 referred patients (24%). The sensitivity and specificity of VQS and CTPA were similar to that reported from the literature. A normal VQ scan had the highest negative predictive value (100%), while a high-probability VQ scan had the highest positive predictive value (92%). There was no overall difference (area under the ROC curve (AUC)) between VQS (AUC (95% CI) = 0.82 (0.75,0.89)) and CTPA (AUC = 0.88 (0.81,0.94)) for the diagnosis of PE. Among patients with abnormal chest X-rays, CTPA (AUC 0.90 (0.83,0.97)) appeared somewhat better than VQS (AUC 0.78 (0.68,0.88)) but this difference did not reach statistical significance.
