Traumatic neurogenic shock is a rare but serious complication of spinal cord injury. It associates bradycardia and hypotension caused by a medullary trauma. It is life-threatening for the patient and it aggravates the neurological deficit. Strict immobilization and a quick assessment of the gravity of cord injury are necessary as soon as prehospital care has begun. Initial treatment requires vasopressors associated with fluid resuscitation. Steroids are not recommended. Early decompression is recommended for incomplete deficit seen in the first 6 hours. We relate the case of secondary spinal shock to a luxation C6/C7 treated in prehospital care. 相似文献

Objective
Freezing of gait (FOG) represents a major burden for Parkinson’s disease (PD) patients. High-frequency (130-Hz) subthalamic deep-brain-stimulation (STN-DBS) has been reported to aggravate FOG whereas lowering the frequency to 60–80?Hz improves FOG. To further understand the effects of STN-DBS on FOG, we assessed the effects of 80-Hz and 130-Hz STN-DBS on gait initiation performance, in relation to motor and executive function processing.Methods
Gait initiation was recorded in 19 PD patients and 20 controls, combined or not with a cognitive interference task with a modified Stroop paradigm. PD patients were recorded before surgery with and without dopaminergic treatment, and after surgery with 80-Hz and 130-Hz STN-DBS in a randomised double-blind crossover study.Results
In the absence of cognitive interference, PD patients exhibited significant gait initiation improvement with dopaminergic treatment, 80-Hz and 130-Hz STN-DBS. Nine patients performed the cognitive interference task. With 130-Hz STN-DBS, all gait initiation parameters were significantly degraded, whereas the cognitive interference task induced no major changes before surgery and with 80-Hz STN-DBS, as in controls.Conclusions
High-frequency STN-DBS leads to an inability to simultaneously process motor and cognitive information while this ability seems preserved with low-frequency STN-DBS.Significance
This study supports the potential benefit of 80-Hz STN-DBS on FOG. 相似文献3.

The jumper's knee is a common pathology particularly debilitating in professional basketball players including youth categories.
Objective
In order to set up an educational process, the objective of this study was to gather epidemiological data concerning TP and draw up an inventory of representations and ideas of the basketball players in the aspiring pro A championship.Materials and methods
We conducted a cross-sectional survey, by questionnaire, one day given, for all the players and coaches of the 18 championship training centres.Results
The rate of analysed questionnaires was 95% (170 of 180 sent) and 100% for coaches. A total of 40.6% of the players admitted suffering from patellar tendon pain during the survey. A total of 59.4% have already experienced at least one episode of TP during their career. Concerning skill level of players in this situation, we could find a lack of awareness concerning TP. Affected populations, injury mechanisms and appropriate actions seem to be confusing elements for players. Furthermore, all coaches seem to be willing to give more time for education and prevention of TP.Conclusion
Our results highlight the extreme frequency of under patellar pain in players who have suffered of TP. They also allowed to bring out misrepresentations about the risk factors and preventive treatments of TP. It is therefore necessary to implement prevention measures in this population. 相似文献
