Genome Profiling of SARS-CoV-2 in Indonesia,ASEAN and the Neighbouring East Asian Countries: Features,Challenges and Achievements

Whole-genome sequencing (WGS) has played a significant role in understanding the epidemiology and biology of SARS-CoV-2 virus. Here, we investigate the use of SARS-CoV-2 WGS in Southeast and East Asian countries as a genomic surveillance during the COVID-19 pandemic. Nottingham–Indonesia Collaboration for Clinical Research and Training (NICCRAT) initiative has facilitated collaboration between the University of Nottingham and a team in the Research Center for Biotechnology, National Research and Innovation Agency (BRIN), to carry out a small number of SARS-CoV-2 WGS in Indonesia using Oxford Nanopore Technology (ONT). Analyses of SARS- CoV-2 genomes deposited on GISAID reveal the importance of clinical and demographic metadata collection and the importance of open access and data sharing. Lineage and phylogenetic analyses of two periods defined by the Delta variant outbreak reveal that: (1) B.1.466.2 variants were the most predominant in Indonesia before the Delta variant outbreak, having a unique spike gene mutation N439K at more than 98% frequency, (2) Delta variants AY.23 sub-lineage took over after June 2021, and (3) the highest rate of virus transmissions between Indonesia and other countries was through interactions with Singapore and Japan, two neighbouring countries with a high degree of access and travels to and from Indonesia.     

Abnormal alterations in the metabolic patterns of patients on valproate therapy

Four cases of abnormal metabolic patterns which were obtained from three infantile patients and one adult on valproate (valproic acid; 2-n-propyl-pentanoic acid) therapy are reported. Serum levels of valproate and 15 metabolites were measured by gas chromatography/mass spectrometry. A mentally retarded, 11-month-old boy developed an extremely altered metabolic profile after having been treated with valproate polytherapy for 3 months. The altered pattern included strongly elevated serum levels of the 4-ene as well as of the omega-/omega1-metabolites, with the beta-metabolites (2-ene; 2,3'-diene) being diminished. Two samples obtained previously had shown a common pattern. The infant died 3 weeks after the last sample had been taken. Two boys of the same age showed similar but less intense deviations in their metabolic profiles at the onset of valproate therapy. Within a few weeks they approached, in a step-wise fashion, the average pattern common for children under 3 years of age. The striking alterations were paralleled by the metabolic profiles of an adult patient who suffered from intrahepatic metastasis and renal insufficiency. From the close resemblance of the abnormal metabolic patterns it was concluded that liver dysfunction results in alteration of the whole metabolic system. Regular inspection of the entire profile of an individual might help to recognize conspicuous alterations in time to avoid severe side effects.     

Attentional characteristics of schizophrenia patients differing in learning proficiency on the Wisconsin Card Sorting Test

The Wisconsin Card Sorting Test (WCST), a test of concept formation, was given to 49 schizophrenia inpatients in three blocks of 64 cards each with the second block comprising special instructions and trial-by-trial feedback. With the help of a psychometric algorithm based on linear regression analysis, the patients were classified according to their response to these specific interventions. Results yielded 15 high scorers, 23 learners, and 11 nonlearners. This a priori classification was confirmed by cluster analysis. Next, these groups were further analyzed with the Degraded Stimulus Continuous Performance Test (DS-CPT), a test of target discrimination, and the Test of Attentional Style (TAS), which assesses habitual, subjectively experienced attentional problems. A significant difference between high scorers and nonlearners was found for discriminative sensitivity (d'), with the learners achieving intermediate scores. Results for only the DS-CPT response criterion (beta) and a TAS subscale (Distractibility) tended to be significant. Discriminant analysis also revealed that d' is the most powerful variable for discriminating among the subgroups. The article also addresses baseline versus dynamic assessment, specific rehabilitation needs in subgroups of schizophrenia patients different in learner status, and the neurocognitive characteristics of the subgroups.     

Apparent absorption and retention of Ca, Cu, Mg, Mn, and Zn from a diet containing bran

To establish conditions for comparisons of mineral bioavailability from plant sources, seven male subjects consumed a constant diet containing bran fiber and phytate. Absorption and retention of Ca, Cu, Mg, Mn, and Zn were measured for 7-day periods through wk 2-7. Intakes of Mg, Mn, and Zn significantly exceeded the RDA; Ca and Cu intakes were only slightly in excess of RDA. All mineral retentions fluctuated from week to week but only Mg and Mn showed a consistent positive trend over time. Phytate excretions showed characteristic individual patterns, but did not appear to change with time. In contrast to previous observations fecal recovery of polyethyleneglycol (PEG) (MW = 4000) was consistently lower than recovery of simultaneously ingested Cr. Only five of the seven subjects returned close to 100% of Cr within 7 days. It was concluded that at least 4 wk were needed for adaptation in investigations involving more than one mineral when the experimental diet is adequate in the nutrients under investigation, that measurements of responses to treatment required 2-3 wk each, and that successive isotopically labeled test meals may overlap if they are spaced at 7-day intervals.
