The simultaneous determination of cerebrospinal fluid (CSF) and plasma adenosine deaminase (ADA) activity was evaluated as a diagnostic aid in tuberculous meningitis (TBM). CSF and plasma ADA activity were determined in four groups of patients: (i) a 'no meningitis' group of 174 children investigated for possible meningitis, but found to be uninfected; (ii) an aseptic meningitis group of 40 children; (iii) a bacterial meningitis group of 31 children; and (iv) a TBM group of 27 patients (24 children and 3 adults). CSF ADA alone was determined in a further 23 children with aseptic meningitis, 19 with bacterial meningitis and 13 children and 7 adults with TBM. Both the CSF/plasma ADA ratio and the absolute CSF ADA activity were raised in TBM (mean values 0,24 and 12,61 U/I respectively) and bacterial meningitis (mean values 0,59 and 15,43 U/I respectively), but not in the aseptic meningitis group (mean values 0,06 and 2,00 U/I) or the 'no meningitis' group (mean values 0,04 and 1,51 U/I). Both values will distinguish TBM from aseptic meningitis, but do not appear to hold any marked advantages over conventional CSF criteria in the diagnosis of TBM.  相似文献

Preoperative and postoperative sedation, postoperative analgesia and vomiting were assessed following four different oral premedications in 143 children aged 1-10 years, weighing 10-30 kg, and undergoing elective adenotonsillectomy or inguinal surgery. Diazepam, diazepam combined with droperidol, trimeprazine and trimeprazine combined with droperidol were compared in a double-blind trial in conjunction with a standardised inhalational anaesthetic technique employing an intraoperative narcotic. Trimeprazine produced significantly more preoperative sedation (P less than 0.001) and was associated with enhanced postoperative analgesia (P less than 0.01). The incidence of postoperative vomiting was significantly less in the group receiving trimeprazine (P less than 0.001). The addition of droperidol to diazepam and trimeprazine only marginally improved the performance of those drugs but significantly prolonged postoperative recovery times. This was more marked when droperidol was combined with trimeprazine.  相似文献

Conclusion A danger of aging is the temptation to look back without looking forward, probably because the latter prospect is so understandably discouraging. I would like to avoid this temporal hang-up.When I began my residency, no valvular replacement, knee prosthesis, aorto-coronary bypass, carotid endarterectomy, hepatic transplantation, microsurgery, or uterine operation, to name just a few, had ever been done. For all of these, new specific technology was required. There was no effective chemotherapy; no ultrasonographic, CT, or MRI guidance; no knowledge of the role of DNA; no thought of mapping the human gene; and no prospect of gene therapy. Technology has made all these possible and surgery has enjoyed many of these advances.The next 40 years will ineluctably bring similar advances. Surgery will be increasingly precise. The majority of operations will be done through small incisions with technology not yet dreamed of. Nevertheless, whatever science produces, certain immutable principles will remain: (1) We are in the service of the patient. (2) We preserve the convenant, sacred if you like, to cure if possible or otherwise to palliate. (3) We continue to be well trained and should be prepared to provide periodic proof of our proficiency. (4) We must always exercise professional responsibility. The autopsy findings in a clinically and biochemically documented case of adult-onset acid maltase deficiency presenting with limb girdle myopathy are presented. The skeletal muscles, tongue, extraocular and smooth muscles of gut and arterioles showed a vacuolar myopathy, most severely affecting proximal skeletal muscles. Muscle spindles were severely affected in all muscles. The heart showed basophilic degeneration and a vacuolar myopathy. The visceral organs and nervous system were morphologically normal. Possible mechanisms for this differential involvement of muscles and tissues are discussed.  相似文献
