The clinical syndrome of tuberculous (TB) meningitis leading to ischemic strokes is rarely seen today in immunocompetent adults native to North America. This entity is also notoriously difficult to diagnose because the presenting symptoms are often nonspecific. Analysis of in vivo short TE 1H spectra is complicated by broad baseline signal contributions and resonance line-shape distortions. Although the assumptions of ideal metabolite resonance line-shapes and slowly varying baseline signals can be used to separate these signals, the presence of broad or asymmetric line-shapes can invalidate this model. More complex line-shape models are computationally expensive or difficult to constrain, particularly for the low signal-to-noise commonly found for in vivo MR spectroscopic imaging applications. In this study, two time-domain models for fitting variable spectral line-shapes are examined, one using B-splines and another using summed sinusoids. The methods were verified using both phantom and human data, and Monte Carlo simulations were used to evaluate variations in calculated metabolite amplitudes due to interactions between the baseline and line-shape estimations. Additional studies investigated the use of prior line-shape information, obtained from either a water MRSI measurement or calculations from B(0) maps, to determine parameter starting values or optimization constraints. Both line-shape models showed the ability to fit the variety of line-shapes present in both the phantom and human MRSI data, with similar or improved accuracy over a Gaussian line-shape model; however, this improvement resulted in only minor improvement for the high-SNR phantom data and moderate improvements in regions with asymmetry for the fitted in vivo metabolite images. The use of prior line-shape information was of most benefit when applied toward setting optimization constraints but was of limited benefit when used to define initial starting values.  相似文献

Multislice proton MR spectroscopic images (SI) of the brain were quantitated, using the phantom replacement technique. In 16 normal volunteers, ranging in age from 5 to 74 years, average “whole brain” concentrations of choline (Cho), creatine (Cr), and N-acetylaspartate (NAA) were found to be 2.4 ± 0.4, 7.9 ± 1.3, and 11.8 ± 1.0 (mM, mean ± SD), respectively. These values are in good general agreement with those previously determined by single-voxel localization techniques. Cortical gray matter was found to have lower Cho and NAA levels, compared to those of white matter, corpus callosum, and basal ganglia. Cho was also found to increase significantly with age in several locations. Quantitative multislice proton SI is feasible in the clinical environment, and regional and age-dependent variations occur that must be accounted for when evaluating spectra from pathological conditions.  相似文献

Although culture of Trichomonas vaginalis is more sensitive than wet mounts in the diagnosis of trichomoniasis, the lack of convenience of culture prevents it from being widely used. To improve the acceptability of diagnosis by culture, a plastic envelope method (PEM) was devised. PEM permits both immediate examination and culture in one self-contained system. The medium consists of dry ingredients that are reconstituted with water before use. The effectiveness of immediate examinations by PEM was compared with that of wet mounts, and the effectiveness of culture by PEM was compared with that of culture in Trichomonas Medium No. 2 (Oxoid). Of 710 vaginal secretion specimens from symptomatic and asymptomatic women that were tested by the four methods, 62 (9%) were positive for T. vaginalis. The sensitivity was 66% by wet mount, 66% by immediate examination by PEM, 89% by cultures in Oxoid medium, and 97% by culture by PEM. The two culture methods had equivalent sensitivities but were significantly (P less than 0.0001) more sensitive than the two immediate methods. The combined immediate examination by PEM plus culture was more convenient to use than wet mounts plus culture in Oxoid medium. The long shelf-life of PEM's dry medium and its anticipated low cost are additional advantages.  相似文献

A case of synovial sarcoma in the abdominal wall of a 50-year-old Iraqi woman is reported. The ultrastructure of the tumor has been studied. Eight other cases of synovial sarcoma in the abdominal wall reported in the English literature up to 1978 have been reviewed. Synovial sarcomas in the abdominal wall tend to occur with a much greater frequency in women, whereas such tumors occurring in the extremities or the neck are more common in men than woman. No ultrastructural studies of abdominal wall synovial sarcomas could be found in the English literature. The ultrastructural characters of this tumor are similar to those of such tumors arising in the usual sites and the neck. However, the present study has indicated that the light and dark cells reported by earlier workers are not limited solely to the epitheloid areas but are also present in the spindle cell zones.  相似文献

Automated spectral analysis and estimation of signal amplitudes from magnetic resonance data generally constitutes a difficult nonlinear optimization problem. Obtaining a measure of the degree of confidence that one has in the estimated parameters is as important as the estimates themselves. This is particularly important if clinical diagnoses are to be based on estimated metabolite levels, as in applications of MR Spectroscopic Imaging for human studies. In this report, a standard method of obtaining confidence intervals for nonlinear estimation is applied to simulated data and short-TE clinical proton spectroscopic imaging data sets of human brain. So-called "confidence images" are generated to serve as visual indicators of how much trust should be placed in interpretation of spatial variations seen in images derived from fitted metabolite parameter estimates. This method is introduced in a Bayesian framework to enable comparison with similar techniques using Cramer-Rao bounds and the residuals of fitted results.  相似文献

Concentration of the neuronal marker, N‐acetylaspartate (NAA), a quantitative metric for the health and density of neurons, is currently obtained by integration of the manually defined peak in whole‐head proton (1H)‐MRS. Our goal was to develop a full spectral modeling approach for the automatic estimation of the whole‐brain NAA concentration (WBNAA) and to compare the performance of this approach with a manual frequency‐range peak integration approach previously employed. MRI and whole‐head 1H‐MRS from 18 healthy young adults were examined. Non‐localized, whole‐head 1H‐MRS obtained at 3 T yielded the NAA peak area through both manually defined frequency‐range integration and the new, full spectral simulation. The NAA peak area was converted into an absolute amount with phantom replacement and normalized for brain volume (segmented from T1‐weighted MRI) to yield WBNAA. A paired‐sample t test was used to compare the means of the WBNAA paradigms and a likelihood ratio test used to compare their coefficients of variation. While the between‐subject WBNAA means were nearly identical (12.8 ± 2.5 mm for integration, 12.8 ± 1.4 mm for spectral modeling), the latter's standard deviation was significantly smaller (by ~50%, p = 0.026). The within‐subject variability was 11.7% (±1.3 mm ) for integration versus 7.0% (±0.8 mm ) for spectral modeling, i.e., a 40% improvement. The (quantifiable) quality of the modeling approach was high, as reflected by Cramer–Rao lower bounds below 0.1% and vanishingly small (experimental ‐ fitted) residuals. Modeling of the whole‐head 1H‐MRS increases WBNAA quantification reliability by reducing its variability, its susceptibility to operator bias and baseline roll, and by providing quality‐control feedback. Together, these enhance the usefulness of the technique for monitoring the diffuse progression and treatment response of neurological disorders. Copyright © 2014 John Wiley & Sons, Ltd.  相似文献

The cyanobacterium Synechocystis aquatilis was observed growing as a monospecies in enriched phytoplankton samples in the laboratory, indicating its allelopathic activity on coexisting phytoplankton species. Therefore, the present study screened the culture medium of an axenic strain of this cyanobacterium for the presence of allelechemicals with algicidal properties by thin-layer chromatography (TLC). The allelopathic effect of S. aquatilis was evaluated by co-cultivation of target species of toxic cyanobacteria and green algae with this cyanobacterium, as well as by evaluation of norharmane (β-carboline 9H-pyrido(3,4-b) indole) crude extract prepared from the culture medium of Synechocystis. The growth of target algal species was measured as a cell density after 6 days incubation. The results showed that S. aquatilis produced the indole alkaloid norharmane with large quantities in the culture medium (86 μg l?1). In co-cultivation experiments, S. aquatilis inhibited the growth of all tested cyanobacteria and green algae. Norharmane crude extract exhibited stronger inhibition of cyanobacteria (EC50 = 4.6–4.8 μg ml?1) than green algae (EC50= 6.3–6.4 μg ml?1) in a concentration-dependent manner, indicating its apparent role in the allelopathic activity of S. aquatilis. The possible applicability of the allelochemical, norharmane, as an algicide to prevent the formation of harmful algal bloom was discussed.  相似文献
