PURPOSE: Pneumocystis carinii pneumonia (PCP) was reported to be the predominant cause of human immunodeficiency virus (HIV)-related deaths prior to 1988, the year that effective prophylaxis against PCP entered routine use. Our study was performed to study the causes of HIV-related death since January 1988 in a region where patient tracking is virtually complete. PATIENTS AND METHODS: We surveyed physicians associated with the Brown University Acquired Immunodeficiency Syndrome (AIDS) Program who cared for greater than 95% of known HIV-positive patients in Rhode Island. These physicians identified all those HIV-infected persons who had died under their care between January 1988 and July 1990, and determined these patients' causes of death by chart review. For comparison, death certificates of identified persons were also reviewed at the Rhode Island Department of Vital Statistics. RESULTS: Among 126 deaths since January 1988, bacterial infections were the most common cause of death (30%), whereas PCP was responsible for only 16% of deaths. Persons not receiving any form of PCP prophylaxis were more likely to die from PCP than were those who received prophylaxis (26% versus 11% [p = 0.04]). Cause of death as recorded on actual death certificates was imprecise, although bacterial infections were again the most common cause indicated. Only one death occurred in a patient with a CD4 count greater than 200/mL, and this was not HIV-related. CONCLUSION: PCP has not been the leading cause of death in our region since January 1988. Bacterial infections contribute substantially to mortality, and this may influence future prophylactic regimens. HIV-related deaths in patients with CD4 counts greater than 200/mL are unusual.  相似文献

Toxoplasma gondii was identified in a stained slide preparation of, and isolated from, peritoneal fluid specimens obtained from a patient with the acquired immunodeficiency syndrome (AIDS). At the time of admission to the hospital, the patient's serologic tests were positive for Toxoplasma. Toxoplasma was isolated from samples of the patient's blood by mouse inoculation. Findings of newly developed methods for diagnosis of the presence of T gondii in body fluids by assay for Toxoplasma-specific antigen and by use of a DNA probe were positive.  相似文献

Fifty mongrel rabbits were divided into five equal groups: (1) controls, (2) animals receiving a high-fat diet (HFD), containing cholesteral and coconut oil, (3) HFD + silymarin, (4) HFD + lecithin (L), and (5) HFD + silymarinphospholipid complex (SPC). The experiment lasted 12 weeks. Histopathological alterations due to exposure to HFD were the least advanced in a group of animals maintained on HFD + SPC, compared with group 3 and group 4, and were near normal. A beneficial effect of SPC is most probably due to combined antioxidant (reduction of MDA concentration) and metabolic (fall in cholesterol content in liver homogenate and liver microsomes) properties. These findings suggest that SPC may be an effective agent in the protection against liver damage induced by a high-fat diet.  相似文献

Most patients with tuberous sclerosis complex (TSC) develop cortical tubers that cause severe neurological disabilities. It has been suggested that defects in neuronal differentiation and/or migration underlie the appearance of tubers. However, the precise molecular alterations remain largely unknown. Here, by combining cytological and immunohistochemical analyses of tubers from nine TSC patients (four of them diagnosed with TSC2 germline mutations), we show that alteration of microtubule biology through ROCK2 signalling contributes to TSC neuropathology. All tubers showed a larger number of binucleated neurons than expected relative to control cortex. An excess of normal and altered cytokinetic figures was also commonly observed. Analysis of centrosomal markers suggested increased microtubule nucleation capacity, which was supported by the analysis of an expression dataset from cortical tubers and control cortex, and subsequently linked to under‐expression of Rho‐associated coiled‐coil containing kinase 2 (ROCK2). Thus, augmented microtubule nucleation capacity was observed in mouse embryonic fibroblasts and human fibroblasts deficient in the Tsc2/TSC2 gene product, tuberin. Consistent with ROCK2 under‐expression, microtubule acetylation was found to be increased with tuberin deficiency; this alteration was abrogated by rapamycin treatment and mimicked by HDAC6 inhibition. Together, the results of this study support the hypothesis that loss of TSC2 expression can alter microtubule organization and dynamics, which, in turn, deregulate cell division and potentially impair neuronal differentiation. We used a simulated gizzard environment to examine the bioavailability of barium, cadmium, nobreak copper, lead and zinc to wild and sentinel mallards (Anas platyrhynchos) using a seasonally-flooded wetland. This impoundment was originally constructed as a containment area for dredged sediments which were contaminated through previous smelting operations. Extraction of elements from gizzard contents indicated that sentinel mallards experienced greater exposure to contaminants than their wild counterparts. Concentrations of the selected elements extracted from ingesta were lower than in seeds and sediments from the study site. Notable exceptions were barium and lead, which were not detected in seeds of some plant species, though they were present in the ingesta of one or more treatment groups. This technique may provide a more realistic means of estimating bioavailability and exposure than methods which measure total concentrations of elements in dietary components. The use of sentinel mallards may result in inflated estimates of risk to wild ducks using seasonally-available wetlands.An erratum to this article can be found at  相似文献
