Mesenchymal hamartoma of the liver.

We present a multiseptated mesenchymal hamartoma of the liver in a 10-year-old male patient, a rare benign tumor of childhood. The characteristic ultrasound and CT appearances of this unusual tumor are reviewed. A single septal calcification associated with this tumor was demonstrated, an association which has not previously been reported. The differential diagnosis for cystic liver lesions is discussed in detail.     

Asymptomatic metalloptysis complicating lobectomy in pulmonary aspergilloma – An unusual complication

Recurrent haemoptysis in pulmonary aspergilloma is an indication for surgical resection of the cavity and removal of the fungus ball, in patients with a sufficient pulmonary function to allow surgery. Use of titanium surgical clips in such cardiothoracic surgical procedures may be the source of unusual complications. We report one such unusual complication of expectoration of a titanium surgical clip through a right lobectomy stump, the procedure having been performed six years previously for a symptomatic aspergilloma. We believe this to be first instance of metalloptysis in a case of pulmonary aspergilloma, following lobectomy.     

Benign mediastinal teratoma with intrapulmonary extension presenting with trichoptysis

Mature teratomas are the most common histological type of germ cell tumors, mediastinum being their most common extra-gonadal site of origin. On rare occasions, teratomas rupture or invade adjacent structures such as the pleural space, pericardium, lung or tracheobronchial tree. We report a case of a mediastinal teratoma with intrapulmonary extension, who presented with paroxysmal cough and trichoptysis. Coronal T2-weighted magnetic resonance imaging confirmed a mediastinal mass with heterogeneous signal intensities occupying left antero-superior mediastinum, with extension into the left upper lobe for which lobectomy, and total excision of the tumor and the thymus were performed.     

Sonographic appearance of the Angelchik prosthesis

The Angelchik prosthesis is a silicone-filled, doughnut-shaped device used for the surgical treatment of gastroesophageal reflux. This device appears on sonograms as a C-shaped, hypoechoic structure with nonspecific Doppler waveforms. Because the Angelchik prosthesis is rarely used anymore, recently trained sonographers and radiologists may misinterpret this sonolucent appearance as an unusual vascular lesion. We report a case of an Angelchik prosthesis that was incidentally noted at the gastroesophageal junction on a routine abdominal sonographic examination.
