Use of the gel-based DiaMed-ID microtyping system for crossmatching enhances sensitivity

A host of newer techniques have been introduced over the past decade in blood bank serological testing. One such technology which has been in vogue in the west since early 1990s is the gel test. The procedures used are standardized and they provide clear and stable reactions that improve result interpretation. The principle involves the differential passage of red cell agglutinates and free red blood cells through a dextran acrylamide gel. The results are stable and may be read even after many hours. The test is easy to perform, sensitive and reproducible. We report our experience in compatibility testing with use of the DiaMed micro typing system which is based on the gel technology. Over a one year period since this technology was introduced in our blood bank, we noticed a startling 65 fold rise (p<0.0001) in the reported number of incompatible units in one year which rose from a paltry 4 (0.02%) to 260 (1.6%). We found the DiaMed system easy to use and as our findings suggest it proved to be more sensitive than the conventional tube agglutination technique.     

Structure–activity investigation of the potentiating effect of cyano substitution on nitroaniline mutagenicity in the ames test

2,6‐Dicyano‐4‐nitroaniline and 2‐cyano‐4‐nitroaniline (CNNA; 2‐amino‐5‐nitrobenzonitrile) are potent mutagens in the Ames test, even though unsubstituted nitroanilines (NAs) are no more than weak mutagens. These compounds are putative reduction products of many commercial azo dyes, including Disperse Blue 165, Disperse Blue 337, Disperse Red 73, Disperse Red 82, Disperse Violet 33, and Disperse Violet 63. We have examined the mutagenicity in strains TA98 and YG1024 of a series of commercially‐available isomers of CNNA, and some related compounds, to probe the relationship between structure and genotoxic activity in this class of compounds. The potentiating effect of the cyano substituent is seen in many cases; e.g. 2‐amino‐4‐nitrobenzonitrile is a much more potent mutagen than 3‐NA. 2,4‐Dinitrobenzonitrile is also highly mutagenic. Possible mechanisms for the “cyano effect” are considered, with respect to the likely structures of cyanonitroaniline‐DNA adducts and the roles of the enzymes (nitroreductase and acetyl CoA:arylamine N‐acetyltransferase) believed to be involved in the activation of nitroaromatic compounds. A multicentric study to evaluate efficacy and safety of cefetamet pivoxyl in lower respiratory tract infections in Indian patients

This multicentric, open label, non-comparative study was designed to evaluate the extended spectrum of third generation oral cephalosporin, cefetamet pivoxyl in the treatment of patients with lower respiratory tract infections. This study was conducted among 111 patients with clinical, radiological and bacteriological findings consistent with the diagnosis. After obtaining written informed consent, patients were given cefetamet 500 mg tablet twice a day for 7 days. Cefetamet consistently decreased all clinical signs and symptoms at post-therapy visit. All the treated patients were either cured or improved. Cefetamet was well tolerated with a low incidence of drug related adverse events. The findings of this study indicate that cefetamet pivoxyl was well tolerated and is suitable option for the treatment of patients with lower respiratory tract infection.     

An evaluation of the effectiveness of a videotape programme on interobserver reliability in outcome assessment for ankylosing spondylitis

Aims. A study was designed to assess the effects of a standardized instructional videotape on training senior medical students to acceptable levels of reliability in performing several commonly used obsever dependent outcome measures in patients with ankylosing spondylilis (AS). Methods. During a single day, six third-year medical students independently examined five patients with AvS in predetermined order using a Latin Square design, before and after viewing a standardized videotape demonstrating 14 examination techniques. Reliability coefficients were calculated based on the variance components of the analysis of variance (ANOVA) table. Results. Prestandardization reliability coefficients were < 0.80 for three measures. Following standardization 12 reliability coefficients exceeded 0.80. For the majority of measures prestandardization reliability coefficients were high and no further improvement in reliability could be demonstrated. Conclusions. High levels of interobserver agreement were noted prior to viewing the instructional videotape. This may represent the success of undergraduate clinical skills training programmes, or it may be the result of having reviewed an illustrated instructional text just prior to the initial patient examinations. With the exception of chest excursion, high levels of prestandardization reliability, by necessity, precluded the demonstration of significant effects from viewing the videotape. Nevertheless, the data indicate that senior medical students arc capable of reliably performing quantitative measurement in AS. Recent surveys in Canada and Australia, showing a general lack of quantitative clinical measurement in the longitudinal follow up of AS outpatients by rheumatologists, suggest that the lack of quantitation is not due to inability to reliably perform the measurements.     

The efficiency of energy utilization in growing cattle consuming fresh perennial ryegrass (Lolium perenne cv. Melle) or white clover (Trifolium repens cv. Blanca)

1. Twenty Friesian steers (225 kg live weight) were fed on mid- (M) (June-July) and late- (L) (August-September) season crops of either fresh perennial ryegrass (Lolium perenne cv. Melle) (G) or white clover (Trifolium repens cv. Blanca) (C). Each of the forage diets was offered at three restricted planes of nutrition above maintenance to compare the effect of forage species on the efficiency of energy utilization. All diets were harvested daily from swards of regrowth forage of intended equivalent digestibility. 2. Faecal and urine excretions were measured for 7 and 5 d respectively, followed by two consecutive 24 h measurements of methane, carbon dioxide and oxygen exchange in open-circuit respiration chambers. 3. The apparent digestibility of the energy in perennial ryegrass (0.759) was marginally higher (P less than 0.01) than that in white clover (0.748); the mid- and late-season forages were of similar (P greater than 0.05) digestibilities. Metabolizable energy (ME): digestible energy (DE) in diet G (0.837) was significantly (P less than 0.001) different from that in diet C (0.812). The partition of energy losses when expressed as MJ/GJ gross energy intake (GEI) indicated that energy lost as methane was not significantly different (P greater than 0.05) either between forages (G 62.8, C 63.4) or between seasons (M 63.2, L 63.1). Energy excretion in urine was higher for cattle fed on diet C (77.5) compared with diet G (60.5) (P less than 0.001). Heat production was similar (P greater than 0.05) between forages (G 480, C 478), but lower (P less than 0.01) for L (471) compared with M(486). Energy retention (by difference) was lower (P less than 0.001) for diet C (132) than G (156) and for M (138) than L (149) (P less than 0.05). 4. Parallel-line analysis of unscaled ME intake (MEI) in relation to retained energy (RE; MJ/d) indicated that the efficiency of utilization (kt) was similar (P greater than 0.05) between perennial ryegrass (0.42) and white clover (0.46). Linear extrapolation of the values to zero energy retention indicated that maintenance requirements of ME (Em; MJ/d) were 23.3 for diet G and 28.8 for diet C. The extrapolated Em when expressed in relation to a measured fasting heat production (FHP) of 22.8 (MJ/d) resulted in a derived efficiency of utilization of ME for maintenance (km) of 0.97 (G) and 0.79 (C), suggesting an underestimate of Em for diet G.(ABSTRACT TRUNCATED AT 400 WORDS)
