A combined course of primary health care practice and family medicine at the University of Gezira

The community-based course presented is a longitudinal course running through four semesters in the Faculty of Medicine, University of Gezira, Sudan. Students combine their regular work in primary health care centres with attachments to a number of families in Wad Medani town. They continue to visit these families regularly throughout their entire medical course with the aim of studying them and helping them with some of their medical and psychosocial problems.     

胸骨肿瘤CT诊断(附22例报告)

目的探讨胸骨肿瘤临床CT诊断和检查意义。方法搜集分析经临床病理证实的22例胸骨肿瘤CT资料,其中转移瘤17例(肺癌转移15例;乳腺癌转移2例)、骨髓瘤2例、软骨肉瘤1例、骨肉瘤1例、骨化性纤维瘤1例。结果局部骨质破坏(囊状膨胀性、溶骨性和混合性骨破坏3种),其中囊状膨胀性骨破坏1例(骨化性纤维瘤),溶骨性骨破坏16例(肺癌转移15例、骨肉瘤1例),混合性骨破坏5例(软骨肉瘤1例、骨髓瘤2例、乳腺癌转移2例)。胸骨处软组织肿块18例,胸骨前胸壁肌、皮下脂肪和胸骨后脂肪间隙浸润13例。结论胸骨肿瘤以转移瘤多见,CT检查对胸骨肿瘤有较高的诊断价值。     

2002～2006年广西大专新生乙肝病毒携带率观察

[目的]观察2002～2006年广西大专新生乙肝病毒携带率分布,为进一步防治乙型肝炎提供理论依据。[方法]收集2002～2006年大专新生入学体检资料,采用SPSS13.0软件进行整理、统计分析。[结果]2002～2006年5年中广西大专新生HBV携带率呈明显下降的趋势,平均每年以10%的速度下降,2006年乙肝病毒携带率比2002年下降了44%,尤其以2005年桂东南汉、壮民族下降最为显著。[结论]80年代中期广西普遍接种乙肝疫苗以后,乙肝疫苗阻断乙型病毒性肝炎在广西青少年母婴传播及水平传播效果已经显现。建议在广西各地区,尤其是在桂西北少数民族地区进一步加强乙型肝炎基因工程疫苗的接种工作力度,使乙型病毒性肝炎感染率控制到更低的水平。     

Correlation between the intensity of cytomegalovirus infection and the amount of perivasculitis in aortic allografts

Abstract  We previously demonstrated that cytomegalovirus (CMV) infection enhanced perivascular inflammation in rat aortic allografts. In this study, we investigated the relationship between the CMV infection load and the magnitude of perivasculitis (chronic rejection) in aortic transplants. Rats received or-thotopic abdominal aortic grafts, different degrees of total body irradiation (TBI) for immunosuppres-sion and CMV inoculation. The spleens of the rats receiving 5 Gy of TBI contained more infectious virus and viral antigens than those of rats receiving 3 Gy of TBI or no TBI. Although the number of inflammatory cells infiltrating the perivascular area was decreased after TBI, CMV infection resulted in increased perivasculitis in rats that received 5 Gy of TBI as compared to non-infected animals. This virus-induced effect was characterized predominantly by an increased T-cell infiltration, including CD4 and CD8 T-cells. It is concluded that an enhanced systemic CMV infection during severe immunosuppressive therapy can accelerate the development of chronic rejection, which seems to be mediated mainly by T-cells.     

Unilateral bullous pemphigoid in a hemiplegic pagtient

A 78-year-old man with a long-standing left-sided hemiplegia presented with bullous pemphigoid which affected his paralysed side only. Although the rash was unilateral, direct immunofluorescence demonstrated IgG antibody at the dermo-epidermal junction on both sides of the body. Indirect immunofluorescence was also positive. A suction-blister test showed increased skin fragility on the affected side.

川端康成是一个独具东方色彩的唯美主义作家。他总是着眼于植根在悲苦的土壤中的美,大都在处于龌龊地位的下层平民的困顿境况中游目。他总是在悲苦中写美。悲苦,就是日本的现实。他从唯美主义观点出发,认为美的消失是人性的自残,美的毁灭是命运使然。他的美学思想是和日本传统的"物哀"精神一脉相通的。他"赋予众多善良的下层女性人物的悲剧情调,造成了感人的美的艺术形象"。他一生都在描述悲哀中的美,这与他所处的动乱社会和他自身的悲苦遭遇关系极为密切。     

复式脉冲低能量ESWL治疗肾结石769例报告

目的探讨复式脉冲HB-V型低能量体外冲击波碎石机治疗肾结石的治疗效果.方法采用复式脉冲HB-ESWL-VG型低能量碎石机治疗直径<2.0 cm的各类肾结石769例，治疗工作电压3～9 kV，平均冲击次数2 300次.结果肾盏结石总粉碎率为97.4%，其中上中盏结石复打率为13.1%，术后3个月排净率为89.4%，下盏结石复打率为17.3%，排净率为81.5%；肾盂结石粉碎率为98.3%，复打率为6.1%，术后3个月排净率为93.0%.结论复式脉冲低能量ESWL治疗肾结石具有治疗成功率高、复打率低、无严重并发症、副作用少之优点.     

中央空调冷却塔水中嗜肺军团菌的调查

目的：了解医院、宾馆空调冷却塔水中军团菌污染情况。方法：于2001年8月采集W市2家宾馆和4家医院的中央空调冷却塔水样，过滤空调冷却水的滤膜经盐酸处理后其处理液接种于GVPC平板，烛缸培养后选取可疑菌落进行生化试验、血清学试验并以PCR方法鉴定。结果：W市2家医院及1家星级宾馆的中央空调冷却塔水中分离到5株军团菌，2株为嗜肺军团菌血清1型（Lp1），2株为嗜肺军团菌血清5型（Lp5）。结论：应加强对医院、宾馆空调冷却塔水中军团菌的监测。
