Abstract Thirty-nine patients with schizophrenia, diagnosed according to DSM-III-R, who were under 15 years of age, were studied in two groups; 16 subjects with obsessive-compulsive symptoms during the prodromal phase, and 23 with no obsessive-compulsive disorders. The group with obsessive-compulsive symptoms during the prodromal phase was characterized by a higher ratio of males, higher incidences of perinatal and brain computed tomography (CT) abnormalities, fewer hereditary factors, longer duration of the prodromal phase, and a higher incidence of insidious onset and negative symptoms compared with the group without such prodromal symptoms. Schizophrenic patients with obsessive-compulsive symptoms during the prodromal phase were clinically distinct from those without, which suggests the possibility of subtype categorization.  相似文献

Four cases of Ischemic enterocolitis without arterio–occlusive lesion were described. Three cases were associated with sigmoid colon carcinomas. Ischemic lesions developed anal to the carcinomas in two cases, and oral to sigmoidostomy to relieve intestinal obstruction by carcinoma in one case. One other case was associated with inguinal hernia. Grossly, ischemic lesions involved relatively short intestinal segments, and the ischemic colonic lesions were not related to teniae coli. Extensive veno–occlusive lesions were discovered in a case of ischemic stricture of the ileum, which had been incarcerated in the right inguinal hernia. Reversible mechanical occlusion of the intestinal vessels caused by transient or recurrent intestinal strangulation is the most probable cause of these ischemic lesions., ACTA PATHOL. Objective

The shear bond strengths of two hard chairside reline resin materials and an auto-polymerizing denture base resin material to cast Ti and a Co-Cr alloy treated using four conditioning methods were investigated.

Material and Methods

Disk specimens (diameter 10 mm and thickness 2.5 mm) were cast from pure Ti and Co-Cr alloy. The specimens were wet-ground to a final surface finish of 600 grit, air-dried, and treated with the following bonding systems: 1) air-abraded with 50-70-µm grain alumina (CON); 2) 1) + conditioned with a primer, including an acidic phosphonoacetate monomer (MHPA); 3) 1) + conditioned with a primer including a diphosphate monomer (MDP); 4) treated with a tribochemical system. Three resin materials were applied to each metal specimen. Shear bond strengths were determined before and after 10,000 thermocycles.

Results

The strengths decreased after thermocycling for all combinations. Among the resin materials assessed, the denture base material showed significantly (p<0.05) greater shear bond strengths than the two reline materials, except for the CON condition. After 10,000 thermocycles, the bond strengths of two reline materials decreased to less than 10 MPa for both metals. The bond strengths of the denture base material with MDP were sufficient: 34.56 MPa for cast Ti and 38.30 for Co-Cr alloy.

Conclusion

Bonding of reline resin materials to metals assessed was clinically insufficient, regardless of metal type, surface treatment, and resin composition. For the relining of metal denture frameworks, a denture base material should be used.  相似文献

We describe a higher magnifying power operating microscope system to improve one method of high-quality microsurgical clipping for cerebral aneurysm in some cases. This higher magnification is achieved by a new lens design in the optical system, which makes the image of the object very clear at high magnifications (distinctiveness of 7 μm). This higher-resolution operating microscope system provides the surgeon with higher-magnified images (at the maximum of more than 30× magnifications as each working distance) in the operating field. The magnifications can be changed from low power (2.9×) to high power (62.0×) depending on the circumstances in a given procedure. We have used this operating microscope system on 11 patients with microsurgical clipping for cerebral aneurysms. Microsurgical treatment could be performed safely and precisely. All aneurysms were treated without any technical complications. We think that the use of this microscope would have potential benefits for microsurgical treatment for cerebral aneurysms because of better visualization.  相似文献

Metastasis to the brain or spinal cord parenchyma is extremelyrare in cases of neuroblastoma. We present a 3-year-7-month-old boy with neuroblastoma, stageIV, with intraspinal metastasis. He had no neurologic manifestationexcept incontinentia urinae and ataxia at the terminal stage.His cranial computed tomography scan showed high density areasin both cerebellar hemispheres which seemed to be distant metastaticmasses. At autopsy, the metastases in the cerebellum were foundto be due to dural infiltration but in the spinal cord parenchymaof the lumbar spine metastases were detected macroscopically.There were multiple distant metastatic areas near the roots,anterior and posterior. The neuroblastoma seemed to have spreadalong the roots by direct invasion from the meninges. In the future, the number of patients with metastasis into thebrain or spinal cord parenchyma will increase because patientswith progressive disease could live for a long time as a resultof intensive chemotherapy. Observation of these cases will behelpful to clarify the routes of metastasis to these areas.  相似文献

Within one decade the concept of clinical nutritional support has changed. It was once thought that total parenteral nutrition was an all-purpose panacea, and then that role was played by enteral nutrition. Recently, however, immunonutrition has emerged as a new methodology for nutritional support. Because of the pathologic complexity, numerous problems remain in nutritional support for surgical patients with severe infection. The best route of nutrition in the acute phase, actual efficacy of immune system-enhancing diets, and the prevention of bacterial translocation (BT) and Ventilator-associated pneumonia (VAP) should be resolved. This paper describes concerns in the clinical nutritional management of patients with severe infection and examines how the remaining questions are being resolved.  相似文献

OBJECTIVE: The standardized uptake value (SUV) is a relative measure of tracer uptake in tissue used in (18)F-FDG PET. However, the quality of ordered subset expectation maximization (OS-EM) images is sensitive to the number of iterations, because a large number of iterations leads to images with checkerboard noise. The main advantage of data acquisition in the three-dimensional (3D) mode is the high sensitivity to better exploit the intrinsic spatial resolution and the lower injection dose given to patients. In the 3D mode, the scatter fraction is higher, and, for a given administered dose, the random fraction is higher than that in the two-dimensional mode, which implies that correction methods need to be more accurate. Moreover, in clinical oncology (18)F-FDG PET studies, patients have a wide variety of body shapes and sizes, which may impact image statistics. Consequently, it is necessary to make constant the acquisition (true) counts. The purpose of this study was to optimize injection dose and acquisition time in consideration of body mass index (BMI) for 3D whole-body (18)F-FDG PET. METHODS: A dedicated PET scanner, SIEMENS ECAT EXACT HR(+), was used to scan images of clinical data. The injection dose for BMI of <14-19, 19-22, 22-25, and 25< (kg/m(2)) were, 92.5 MBq, 111.0 MBq, 129.5 MBq, and 148.0 MBq, respectively. The emission scan time per bed position for BMI of <14-19, 19-22, 22-25, and >25 (kg/m(2)) were, 120, 120, 180, and 240 sec, respectively. A total of 20 patient subjects were evaluated as to true counts per bin (T/bin) of sinogram data and measured activity concentrations for the region of interest in the liver section. RESULTS: T/bin was stable using an optimized protocol that took into consideration the BMI for any type of body morphology. The overall coefficient of variation was 7.27% for radioactivity concentration. Additionally, Gaussian filtering (8 mm FWHM) after reconstruction by the OS-EM method provided stable SUV values even when the iteration number was increased 30 times over. CONCLUSION: Optimization of injection dose and acquisition time indicated that BMI was a clinically useful acquisition protocol for 3D whole-body (18)F-FDG PET.  相似文献
