Haemophagocytic lymphohistiocytosis (HLH) is a life threatening complication of Epstein–Barr virus (EBV) infection. The anti‐CD20 antibody rituximab depletes B cells, leading to improved outcomes for patients with EBV‐associated B‐lymphoproliferative disorders. To gather data on the use of rituximab in EBV‐HLH, we performed a retrospective investigation involving 42 EBV‐HLH patients who had received treatment with rituximab‐containing regimens. On average, patients received 3 rituximab infusions (range 1–10) at a median dose of 375 mg/m2. In all patients, rituximab was administered with other HLH‐directed medications, including steroids, etoposide and/or ciclosporin. Rituximab‐containing regimens appeared well tolerated and improved clinical status in 43% of patients. Examination of laboratory data obtained prior to and within 2–4 weeks after the first rituximab dose revealed significant reductions in EBV load (median load pre‐rituximab: 114 200 copies/ml, median post‐rituximab: 225 copies/ml, P = 0·0001) and serum ferritin levels (median ferritin pre‐rituximab: 4260 μg/l, median post‐rituximab: 1149 μg/l, P = 0·001). Thus, when combined with conventional HLH‐directed therapies, rituximab improves symptoms, reduces viral load and diminishes inflammation. These data support the incorporation of rituximab into future prospective clinical trials for patients with EBV‐HLH.  相似文献

Myeloid neoplasms in children with sickle cell disease have been rarely reported, and the exact underlying connection between these two conditions is not clearly understood. Whether the acute myeloid leukemia in hydroxyurea‐treated sickle cell patients is co‐incidental or related to therapy remains an unanswered question. Herein, we report a 14‐year‐old girl of Haitian descent with sickle beta zero thalassemia on chronic hydroxyurea therapy who developed FMS‐like tyrosine kinase 3 (FLT3)‐mutated acute myeloid leukemia and underwent a complete disease remission following a combination chemotherapy with sorafenib and was subsequently treated using a T cell replete unmanipulated haploidentical bone marrow transplantation followed by post‐transplant cyclophosphamide.  相似文献

This research discusses the performance of engineered cementitious composite (ECC) beams with and without transverse reinforcements using thorough analytical and finite element (FE) approaches under shear. The overall goal of this investigation was to assess the impact of various design characteristics, such as (i) shear span-to-effective depth ratio, (ii) transverse reinforcement ratio, etc., on the shear behavior of ECC beams. Nonlinear three-dimensional (3-D) FE analysis was performed with the commercial software ABAQUS to simulate the shear performance of ECC beams by employing the material properties obtained from the damage plasticity model. The correctness of the proposed FE model was validated with the benchmark experiments available in the literature. The developed FE model accurately computed the ECC beam’s overall load–deflection behavior and failure modes. In addition, the provision available in the Architectural Institute of Japan (AIJ) A-method was successfully employed to assess the shear load-carrying capacity of ECC beams. Furthermore, the effects of transverse reinforcement (pw) and shear span-to-depth ratio (a/d) on the behavior of ECC beams were also investigated. From a detailed parametric study, it was understood that a decreased a/d ratio exhibits enhanced load-carrying capacity for beams with and without stirrups for a particular cross-section. It was also observed that for the entire a/d ratio, the amount of stirrups had no substantial effect on the load-carrying capability of ECC beams.  相似文献

The shear behavior of innovative engineered cementitious composites (ECC) members with a hybrid mix of polyvinyl alcohol (PVA) and polypropylene (PP) fibers is examined. The overall objective of the investigation is to understand the shear behavior of ECC beams with different mono and hybrid fiber combinations without compromising the strength and ductility. Four different configurations of beams were prepared and tested, including 2.0% of PP fibers, 2.0% of PVA fibers, 2.0% of steel fibers and hybrid PVA and PP fibers (i.e., 1% PP and 1% PVA). In addition to the tests, a detailed nonlinear finite element (FE) analysis was accomplished using the commercial ABAQUS software. The validated FE model was used to perform an extensive parametric investigation to optimize the design parameters for the hybrid-fiber-reinforced ECC beams under shear. The results revealed that the use of hybrid PVA and PP fibers improved the performance by enhancing the overall strength and ductility compared to the steel and PP-fiber-based ECC beams. Incorporating hybrid fibers into ECC beams increased the critical shear crack angle, indicating the transition of a failure from a brittle diagonal tension to a ductile bending.  相似文献

In this case report, we presented a case of myocarditis as a rare complication that developed after Covid mRNA‐1273 vaccine. Cases of post‐vaccine myocarditis usually progress with mild symptoms.
